Introduction
Public health improvement initiatives (PHII) provide invaluable data for patient–centered care, but their research is often conducted in a context
different from the needs of any individual patient. Providers must make a conscious effort to apply their findings to specific patients' care.
In this activity, you will learn about a PHII, and explore its application to a particular patient's care plan.

At last week's conference I spoke with Alicia Balewa, Director of Safe Headspace. They're a relatively new nonprofit working on improving outcomes
for TBI patients, and I immediately thought of Mr. Nowak. At his last biannual cholesterol screening he mentioned having trouble with his balance. This
may be related to his hypertension, but he believes it's related to the time he was hospitalized many years ago after falling out of a tree, and
expressed distress that this might be the beginning of a rapid decline.

My father came home from Vietnam with a kaleidoscope of mental health problems. That was the 1970s, when treatment options for things like PTSD,
TBI, and even depression were very different. Since then there has been a lot of investment in treatment and recovery for combat veterans. That's
excellent news for veterans in treatment now, but they're not looking at my dad, and how his TBI and PTSD have affected him through mid–life and
now as a senior. That's why I started Safe Headspace: to focus on older patients who are years or decades past their trauma, and find ways to help
them.
Which treatments showed the strongest improvement?

Scenario
The charge nurse in your clinic has contacted you to assume primary care for a patient and develop a plan for follow-up care. The plan should be personalized for him based on evidence-based research provided by a community expert as well as your own research on the condition. You will also be challenged to determine which aspects of the traumatic brain injury (TBI) approach could be applied to similar situations and patients.
Your Role
You are a nurse who has been requested to provide primary patient care, including a follow-up care plan. You will revisit the interview with the community TBI expert and prepare a personalized health pl.

This is a much better paper, but the main thing is to identify a successful PHII. For example others have used Triple Aim but there are many others. If you have questions please let me know.

Population health improvement initiative is meant to bring some improvement in a certain health issue in the community (Hack et al., 2017). The initiative must have some outcomes at the end of the improvement process. These are the set goals or achievements that the initiative wishes to achieve at the end of the process.In the case at hand, the expected achievements of the population health improvement plan for elderly people with traumatic brain injury (TBI) and post traumatic stress disorder (PTSD) include; improvement in moods, memory, and muscle control. Most elderly people with TBI and PTSD have frequent cases of loss of memory. They tend to forget things easily (Winter et al., 2016). In addition, they also have issues with their moods. Their muscle control is also very low.

Introduction
Normally, PICOT format is helpful in formulation of questions in an evidenced based clinical practice. PICOT generated questions generally fall under for main categories of clinical practices. These include; therapy, prevention, diagnosis, etiology as well as Prognosis. The essential elements in PICOT questions. The PICOT format is valuable in addressing research questions comprehensively. Five elements are normally addressed including; population, intervention, comparison, outcome and time as well (Riva, Malik, Burnie, Endicott, & Busse, 2012).
Summary of Case Study
The ever increasingly high incidence of breast cancer conditions has posed serious challenges in the nursing profession. Provision of appropriate healthcare to the cancer patients has been lacking leading to adverse effects of the proliferation of cancerous cells which further worsen the conditions of the patients. As primary care, clinicians have the responsibility to stressing providing healthcare services within healthcare facilities as well as monitoring treatment in home based facilities to help manage cancer condition. Most cancer patient need clinicians who practice evidence-based clinical practices (Riva, Malik, Burnie, Endicott, & Busse, 2012).
Research Question
In cancer patients receiving chemotherapy, will they have better white blood cell count monitoring with a follow-up at home versus follow-up at a health care facility during their treatment?
PICOT Format
1) P-Population: Patients aged 18-60 years-old, breast cancer who have not received chemotherapy in the past six months are subjected to the treatment. Patients with other serious health conditions such as heart diseases were excluded in the study. 30 patients, with 15patients stationed at the healthcare facility while the other 15 patients receiving home-based care, are expected to take part in the study.
2) I -Intervention: The patients will receive dosage based on the age, sex and health general body health as well as the stage of cancer cells proliferation in the body. The patients are required take the prescribed drugs at regular intervals. The subjects will be subjected to treatment under the same during the research study.
3) C-Comparison: All the subject regardless of variations in their level of dose requirement will be subjected to the same treatment for the same duration, 3months. Standardized treatment will be given to subjects with no extreme variations in their level of dose requirement and would be used as an active control group. Using this strategy, it will be possible to minimize the non-specific effects due to a group of the patient receiving treatment within the healthcare.

In this course, I have done extensive research about several nursing and science-based theories and how these theories can be applied in solving a nursing problem. The problem that needed a solution in this course was chronic non-cancer pain. In this course, I was able to demonstrate how nursing theories can be integrated into efforts to solve chronic non-cancer pain, which happens to be an old-age problem in nursing practice. In particular, I demonstrated how the biopsychosocial model of chronic pain is applied to the management of chronic non-cancer pain. The model provided a clear framework for steps that needs to be taken to manage chronic non-cancer effectively.
Of particular importance was the application of the model in determining factors that determine the speed at which one recovers from chronic non-cancer pain. Some of the factors that I described using the model included coping strategies, distress, illness, and physical dysfunction (Kaiser et al., 2013). Given the multiple factors that determine the speed of recovery from chronic non-cancer pain, it is arguable that it is not possible to manage this problem using a single intervention. The biopsychosocial model of chronic pain came in handy in describing the “cycle of pain”, given the fact that an individual develops pain due to interaction of factors such biological, psychological, and social factors (Kaiser et al., 2013). Explaining the interaction of the aforementioned factors helped in deriving sense from the fact that people who develop chronic pain are unable to engage in social and occupational activities. It also became evident that other factors such as depression and isolation contribute immensely to the exacerbation of pain symptoms.
The Biopsychosocial model of chronic pain was also useful in my course because it helped me in researching deeply on treatment of chronic non-cancer pain. By applying the model, it was possible to gain a deeper understanding of the interaction among different factors that contribute to the exacerbation of chronic non-cancer pain symptoms. Fundamentally, the model was quite useful in understanding that pain has a negative impact on emotions of an individual when it persists. Patients start developing negative beliefs about the negative effects of chronic non-cancer pain (Kaiser et al., 2013). Basing on this information, it was possible to choose pain management approaches that only relieve the pain but also change the negative beliefs that patients develop about pain.

Study problem
There are several studies that have shown that stigmatization towards mental health patients have been present throughout history and even despite the evolution in modern medicine and advanced treatment. For example, Verhaeghe et al., (2014), captures in a publication in reference to a study that he conducted that stigmatization towards mental health patients has been there even as early is in the 18th Century. People were hesitant to interact with people termed or perceived to have mental health conditions.
Stigmatization has resulted from the belief that those with mental problem are aggressive and dangerous creating a social distance (Szeto et al., 2017). Also, mental health-related stigma has become of major concern as it creates crucial barriers to access treatment and quality care since it not only influences the behaviour of the patients but also the attitude of the providers hence impacting help-seeking. Timmermann, Uhrenfeldt and Birkelund (2014), have identified stigma as a barrier that is of significance to care or help seeking while the extent to which it still remains a barrier have not been reviewed deeply. Therefore, this study will assess the role contributed by stigma in help seeking in depth. 1. Purpose

On a daily basis, healthcare providers are faced with an array of clinical decisions to be made in an efficient and timely manner. Translating evidence into best practices is one way to achieve this. Without current best evidence, practice is rapidly outdated, often to the detriment of the patient. Evidence based practice is the conscientious use of current best practice in making decisions about patient care (Sackett, Richardson, Rosenberg, & Hayes, 2000). It is important for health care professionals to ask questions about their current clinical practice. You are working in an NHS psychological therapies service in an ethnically diverse area of London, however the service has noticed that patients from marginalised ethnic groups are under-represented in the patient numbers. Consultation with local community groups and existing patients suggests that a choice of a mind-body therapy, alongside the standard talking therapy currently offered would be welcomed. You have been asked by your Service Manager to investigate
two suitable mind-body therapies and to write a report on these so that the service can determine which therapy to offer to its patients.

As primary care, clinicians have the responsibility to stressing providing healthcare services within healthcare facilities as well as monitoring treatment in home based facilities to help manage cancer condition. Research Question
In cancer patients receiving chemotherapy, will they have better white blood cell count monitoring with a follow-up at home versus follow-up at a health care facility during their treatment?
PICOT Format
1) P-Population: Patients aged 18-60 years-old, breast cancer who have not received chemotherapy in the past six months are subjected to the treatment. Patients with other serious health conditions such as heart diseases were excluded in the study. 30 patients, with 15patients stationed at the healthcare facility while the other 15 patients receiving home-based care, are expected to take part in the study.
2) I -Intervention: The patients will receive dosage based on the age, sex and health general body health as well as the stage of cancer cells proliferation in the body. The patients are required take the prescribed drugs at regular intervals. The subjects will be subjected to treatment under the same during the research study.
3) C-Comparison: All the subject regardless of variations in their level of dose requirement will be subjected to the same treatment for the same duration, 3months. Standardized treatment will be given to subjects with no extreme variations in their level of dose requirement and would be used as an active control group. Using this strategy, it will be possible to minimize the non-specific effects due to a group of the patient receiving treatment within the healthcare.

In this course, I have done extensive research about several nursing and science-based theories and how these theories can be applied in solving a nursing problem. The problem that needed a solution in this course was chronic non-cancer pain. In this course, I was able to demonstrate how nursing theories can be integrated into efforts to solve chronic non-cancer pain, which happens to be an old-age problem in nursing practice. In particular, I demonstrated how the biopsychosocial model of chronic pain is applied to the management of chronic non-cancer pain. The model provided a clear framework for steps that needs to be taken to manage chronic non-cancer effectively.
Of particular importance was the application of the model in determining factors that determine the speed at which one recovers from chronic non-cancer pain. Some of the factors that I described using the model included coping strategies, distress, illness, and physical dysfunction (Kaiser et al., 2013). Given the multiple factors that determine the speed of recovery from chronic non-cancer pain, it is arguable that it is not possible to manage this problem using a single intervention. Without current best evidence, practice is rapidly outdated, often to the detriment of the patient. Evidence based practice is the conscientious use of current best practice in making decisions about patient care (Sackett, Richardson, Rosenberg, & Hayes, 2000). It is important for health care professionals to ask questions about their current clinical practice. You are working in an NHS psychological therapies service in an ethnically diverse area of London, however the service has noticed that patients from marginalised ethnic groups are under-represented in the patient numbers. Consultation with local community groups and existing patients suggests that a choice of a mind-body therapy, alongside the standard talking therapy currently offered would be welcomed. Artificial Intelligence (AI) technologies such as Generative AI, Image Generators and Large Language Models have had a dramatic impact on teaching, learning and assessment over the past 18 months. The most immediate threat AI posed was to Academic Integrity with Higher Education Institutes (HEIs) focusing their efforts on combating the use of GenAI in assessment. Guidelines were developed for staff and students, policies put in place too. Innovative educators have forged paths in the use of Generative AI for teaching, learning and assessments leading to pockets of transformation springing up across HEIs, often with little or no top-down guidance, support or direction.
This Gasta posits a strategic approach to integrating AI into HEIs to prepare staff, students and the curriculum for an evolving world and workplace. We will highlight the advantages of working with these technologies beyond the realm of teaching, learning and assessment by considering prompt engineering skills, industry impact, curriculum changes, and the need for staff upskilling. In contrast, not engaging strategically with Generative AI poses risks, including falling behind peers, missed opportunities and failing to ensure our graduates remain employable. Introduction
Public health improvement initiatives (PHII) provide invaluable
data for patient–centered care, but their research is often
conducted in a context
different from the needs of any individual patient. Providers
must make a conscious effort to apply their findings to specific
patients' care.
In this activity, you will learn about a PHII, and explore its
application to a particular patient's care plan.

At last week's conference I spoke with Alicia Balewa, Director
of Safe Headspace. They're a relatively new nonprofit working
2. on improving outcomes
for TBI patients, and I immediately thought of Mr. Nowak. At
his last biannual cholesterol screening he mentioned having
trouble with his balance. This
may be related to his hypertension, but he believes it's related to
the time he was hospitalized many years ago after falling out of
a tree, and
expressed distress that this might be the beginning of a rapid
decline.
Ms. Balewa will be on premises next week, and I'd like to set

you recently studied. What populations did your public health
improvement initiative
study?

My father came home from Vietnam with a kaleidoscope of
mental health problems. That was the 1970s, when treatment
options for things like PTSD,
TBI, and even depression were very different. Since then there
has been a lot of investment in treatment and recovery for
combat veterans. That's
excellent news for veterans in treatment now, but they're not
looking at my dad, and how his TBI and PTSD have affected
him through mid–life and
now as a senior. That's why I started Safe Headspace: to focus
on older patients who are years or decades past their trauma,
and find ways to help
them.
Which treatments showed the strongest improvement?
Exercise. We were able to persuade about half of our
participants — that's around 400 people, mostly men ages 45–80
— to follow the CDC's
recommendations for moderate aerobic exercise. Almost
everyone showed improvement in mood, memory, and muscle
control after four weeks. After
that a lot of participants dropped out, which is disappointing.
But of the 75 who stuck with it for another three months,
muscle control improved 15%,
mood improved 22%, and short–to–medium term memory
improved 61%. We didn't specify what kind of exercise, but we
did ask them to record
what they did every week, so that data is available.
4. Second was medication and therapy. Most of our participants
didn't receive any kind of psychotherapy in the years
immediately following their
trauma, so we had everyone assessed by a team of
psychotherapists. As a result of those assessments, 40% of
participants started on anti–
depressant medication and 9% started taking anti–psychotics.
Those who started taking medications now have regular contact
with a therapist to
manage that care. With some help at home to stick to the
regimen, all but a few have successfully followed their
treatment plans. They've reported a
26% improvement in mood over six months, and a 6%
improvement in memory.
The third treatment I want to mention is meditation. We only
had a small group interested in trying it, but the results were
dramatic. We prescribed
daily meditation at home, just 10 to 15 minutes, with a weekly
hour–long guided group meditation for all 23 participants. After
three weeks we lost two
to disinterest, but the other 21 showed improvements of over
70% in mood and memory, and 32% in muscle control.
Have you tried anything that hasn't worked?
Sure. There are memory exercises for patients in elderly care,
and things like Sudoku and crossword puzzles. We didn't see
any gains with those.
Some of our participants preferred strength training to aerobic
exercise, and the only improvement we saw in that group was in
muscle control, but
only 4%, which is significantly less than the aerobic group.
I should also say that we were working with a willing group of
5. participants. They knew they needed help, and were motivated
to get it. One of the
hurdles we see with veterans, especially in older generations, is
an unwillingness to acknowledge that they have a problem. We
haven't had to wrestle
with that because everyone who volunteers to participate wants
to be there.
Your organization is intervening with people who have TBI and
PTSD simultaneously.
We have a patient with moderate TBI suffered almost 40 years
ago, but no history of
PTSD. Have you separated your population and studied each
separately?
We haven't, no. In some cases we could, for those who come in
with previous diagnoses and medical records. But we have
participants who either
weren't diagnosed, were under–diagnosed at the time, or don't
have records to show us.
Conclusion
As you've seen, a PHII can apply to a patient under your care.
But it's not always a perfect fit, and it's
important to think carefully about how your patient's condition,
symptoms, background, and experience
compare to that of participants in a PHII.
