This document summarizes a clinical trial that compared targeted temperature management (TTM) at 33°C for 48 hours versus the currently recommended 24 hours in unconscious cardiac arrest patients. The trial involved 355 patients randomized to 48 hours or 24 hours of TTM cooling at 33°C. The primary outcome was favorable neurologic outcome at 6 months, and secondary outcomes included 6-month mortality, time to death, adverse events, and ICU resource use. The trial found no significant difference in neurologic outcomes or mortality between groups. Adverse events were higher in the 48-hour group. The study may have lacked power to detect clinically important differences and further research is warranted.
