A case report describes a patient who developed a local allergic reaction to chloramphenicol ointment applied to the eye to treat an adenexal infection. Chloramphenicol is commonly used topically for eye and skin infections due to its broad-spectrum antibacterial properties. However, local hypersensitivity reactions can occur, though they are uncommon. Diagnostic tests for drug allergies are often unavailable in developing countries. Therefore, the diagnosis in this case was made through a high index of clinical suspicion rather than confirmatory testing. The allergic reaction was diagnosed and treated successfully without diagnostic tests based on the clinical presentation and history of chloramphenicol use.
