- A nurse forgot to connect an IV antibiotic to a patient's line during their morning shift, missing the scheduled dose. This clinical encounter highlighted a lack of one of the five prerequisites for person-centered care.
- The identified prerequisite was commitment to the job. Missing the IV dose posed risks to the patient's treatment process.
- The student reflected on how they will ensure their future practice consistently applies commitment to the job, such as being more attentive to tasks and double checking medications are administered.

Brief overview of situation/clinical encounter
Provide a succinct overview with adequate information in which
to contextualise the pre-requisite being discussed(150 words).
When we started the shift at 2 pm and during checking on our
patients on handover round we have found an IV antibiotics
medication connected to infusion pump, but not connecting to
the patient iv line since 8 am. The responsible nurse realised
that he forgets to connect the iv medication to the patient
10. because he was so busy during the shift and he thought that he
has done it, but he is not.
This medication was two times a day and now we have missed
morning dose which will affect the patient treatment process.
Identify relevant pre-requisite/s
Your encounter identifies several pre-requisites that
impacted/contributed to the clinical encounter. Please list these
below in dot points (not included in word count).
Identify and discuss ONE priority pre-requisite
Relate this to the literature on person-centred practice and
outcomes, highlighting its importance as an attribute for health
professionals and how this should be demonstrated with
reference to the Registered Nurse Standards for Practice (2016)
(300 words).
Discuss what risk/impact there was/could have been
The chosen pre-requisite was not being enacted to its full
capacity during the situation/clinical encounter nominated.
What risks/impact did this/could this have/had? Was it
eventually corrected? (100 words).
Consider your own nursing practice
11. Reflect on what you have learnt and what you will now do to
ensure your future practice is able to consistently apply the pre-
requisite discussed (250 words).

Pre-requisites for person-centred care: reflective paper
Task Description:
During PEP, you will regularly encounter clinical episodes that
both challenge and enhance your developing professional
practice. The purpose of this task is to generate a reflective
paper focussed on ONE of the five ‘prerequisites’ of the person-
centred nursing framework. These ‘prerequisites’ are considered
the essential attributes for health professionals to facilitate
person-centred outcomes (McCormack, Manley & Titchen
2013). These are identified as follows:
1. Professionally competent
2. Developed interpersonal skills
3. Commitment to the job
4. Clarity of beliefs and values
5. Knowing ‘self’
From your PEP , consider a clinical encounter (either through
direct observation of other health professional/s or your own
12. actions), where you felt a pre-requisite may have been lacking
or questionable. Briefly outline the situation and identify which
pre-requisite/s may have impacted/contributed to the patient
outcome.
Choose ONE of these as your focus and briefly relate this to the
literature regarding person-centred outcomes and why this pre-
requisite is considered an important attribute for practitioners.
You will also be required to consider your own practice to
ensure you adequately meet this pre-requisite as you progress
through the final stages of your degree and continue to establish
your professional identity.
Additional resources including a template to populate with your
reflection is attatched.
