For the very first time in my life, I had executed a series of behavior changes in order to lose fifteen pounds by this semester. I am pleased and more
than grateful to see that my customized plan is successful, and took effect in different aspects of my well–being. "I am not what happened to me, I am
what I choose to become", I believe this quote by Carl Jung encapsulated why my health improvement plan would work – determination incorporated
into a personalized plan. Hereinafter, I would like to discuss in details what and why had my behavior changes improved both my short–term and
long–term health.
I followed the Copenhagen diet – a thirteen–day diet to lose weight and improve metabolism. I started this diet on the start of semester, giving myself
a week time to bid farewell to my ravenous appetite, before disciplining myself to go through the diet. It was extremely difficult for the first few
days, I was constantly hungry and was craving for snacks between meals. I wasn't able to strictly follow what is written on the menu, and I irresistibly
ate between meals for several days when I was famished. Fortunately, I had been able to resist the temptations of junk foods – foods containing
excessive calories, fat, sugar and salt. I only ate fruits or muesli when I was feeling hungry. These acts of violation subsided completely on the fourth

plan only has a $500 deductible, full premium, and $0 copay after the $500 dollar deductible for all of the services I currently use the most. In
addition, the estimated cost of this plan per month is $13.37.
Furthermore, for this plan there is no charge after the deductible for office visits to a primary doctor, office visits to a specialist, visits to the
emergency room, and visits to the urgent care facility; a $0 copay after deductible for brand and generic drugs, no charge for preventive care coverage,

Personal Health Improvement Plan Background In early January of 2016, I embarked on a new challenge to improve my overall health. Looking

Overall, I have only lost about 3 pounds which could be attributed more to diet than running. Running was great and I was able to follow 5 days

Following the theory of impression management, I found that I would put on a sort of mask as I began my shift each week. But not only would I
have a volunteering identity, I also found that I would change myself to fit the needs of each patient I spoke with. For one person I was silly and

There are a number of products and initiatives being used by health plans to control expenditures and maintain market competitiveness. Recent slowing
in health expenditures has largely been due to price changes, a shift from inpatient to outpatient care, and increased coverage. However, here I will
focus on operational changes within the health plan that can be made to decrease expenditures. I will discuss how investing in disease management
programs, wellness incentives, and value–based insurance design now can be used to control rising healthcare expenditures in the future while adding
value for consumers. I will also discuss the challenges involved with implementing these initiatives.
Disease Management Programs:
Background
A small portion of the patient population accounts for a disproportionate amount of health care expenditures primarily due to chronic illness (Shelton,
2002). Because of this, and the overall rise in healthcare costs, stakeholders have been searching for ways to target this small but expensive group of
people. One way is through disease management programs. It is a common belief that education and preventive measures make it easier to manage
chronic illness and reduce adverse outcomes including lengthy hospitalizations (Shelton, 2002). Although these programs are fairly new and there isn't
much data to support or refute their success, this is an area worth exploring to potentially curve skyrocketing healthcare expenditures.

Overview The first 6 months of the Lakeside Health Plan's costs have run above both the prior year's spend and expected budget. An increase in
unforeseen large claimant activity from dependents is contributing to this overestimated spend. Although this year's plan spend through the first 6
months is higher than anticipated, the past several years for the Lakeside plan saw reduced cost and surpluses to budget. For the 2014/2015 plan
year, the Lakeside Health Plan budgeted an $11,879 PEPY cost. The year finished at $11,236 PEPY or $132,236 below budget. Medical claims were
–3.1% below the prior year, and RX spend was a 14.1% increase; changes were made to the 2015/2016 plan year to counter the increased drug
spend. The plan saw large reductions in large claimant spend due to both stop loss negotiations resulting in the removal of lasers and DialysisPPO.
During the 2013/2014 plan year, the health plan total cost ended almost 25% below the expected budget. Net paid claims were down –7.4% or
$447,827 over the budget. On a PEPY basis, Lakeside had budgeted a $14,995 net cost per employee. The plan year ended at an $11,301 PEPY
cost or $676,350 below the expected total cost. This was largely due to a large claimant with $250,000 exposure in the form of a laser coming off the
plan with only $46k in claims. Stop loss reimbursements dropped 35%, which helped to secure a 3% total renewal. Lakeside also heavily invested in
