6. QUALITY POLICY
Pangasinan Provincial Hospital is committed to sustain its
mission of delivering responsive, efficient and equitable
quality health care services to its stakeholders.
In support of this commitment, it shall:
Provide relevant health policies, programs and services
consistent with standards at minimum cost;
Continuously enhance the competencies of its employees;
Upgrade service capabilities by adopting new technologies;
and
Comply with applicable statutory and regulatory requirements
7. CONTEXT OF THE ORGANIZATION
The PPH determines external and internal issues, relevant to
the purpose and strategic direction of the organization in
achieving its objectives. Monitoring and reviewing activities
about the said external and internal issues is very relevant
in the success of planning.
The hospital also determines interested parties that are
relevant to the Management. The parties’ requirements
(needs and expectations) are likewise being determined by
the organization. Information about these interested
parties shall also be monitored and reviewed annually.

CONTEXT OF THE ORGANIZATION
10. PERFORMANCE EVALUATION
It is the policy of the Pangasinan Provincial Hospital to
conduct a review of the suitability and effectiveness of its
Management.
In monitoring, measurement, analysis and evaluation, PPH
determines:

The methods for monitoring, measurement, analysis and
evaluation needed to ensure valid results
When the monitoring and measuring shall be performed
When the results from monitoring and measurement shall
be analyzed and evaluated
11. Different process owners submit an Objective Performance
Report on a regular basis. This report will identify if
targets per area are being reached. Likewise, non-
attainment of targets is being subjected to Corrective
Action in order to improve service delivery.
Identified problems and opportunities and its accompanying
strategies, plans and actions are also being evaluated
for its effectiveness. For a given action plan, its
effectiveness shall be evaluated after at least 6 months
of implementation, preferably on a biannual basis of 6
months interval. This is subject to change depending on
the severity of the problems identified during evaluation.
