I am Annie Jean Mae R. Camino taking up Master of Arts in Education Major in
Educational Administration. I am humbly asking for your assistancefor the validation of the survey
instrument of my thesis entitled: Factors Affecting the ICT Adoption ofSecondary School Teachers
of Prosperidad II in the Division of Agusan del Sur.
The purpose of this study is to determine the extent of ICT adoption in the classroom
instruction in the secondary schools of Prosperidad II in the Division of Agusan del Sur by
answering the following questions:
1. What is the profile of the secondary teachers of Prosperidad District 1 in the Division
of Agusan del Sur in terms of Age, Total number of years in teaching, Area of
specialization, Subject taught, Position and Number of trainings attended;
2. What is the level of capability of the teachers based on the Self-Assessment Tools –
Result-based Performance Management System (RPMS);
3. What are their attitudes towards ICT adoption;
4. What is the extent of practice in terms of decision, implementation, and confirmation
in the ICT adoption;
5. What challenges are encountered in the ICT adoption?
6. What interventions can be considered for implementation to address problems on ICT
adoption?
