referrals.
“This new approach has provided a more
in-depth understanding of the arrangements of
our local services and a greater cohesiveness
between the roles of the optometrist and the
service aims of the PCT.”
Nerinda Evans, Head of Commissioning and Development at
NHS Suffolk
7. The way things are now
During the course of our research, we’ve
heard several common goals mentioned
by PCTs:
Improve lines of communication between PCTs,
secondary care and community care
Increase efficiency of flow of patients referred from a
general practitioner to a specialist / hospital
Ensure patients do not spend time in the system when
they could be treated appropriately in the community at a
faster rate
8. We believe in driving
efficient eyecare
through innovation
9. What we can do for you
• We have created a revolutionary new system
that is driven by the four pillars that are
important to you:
– Quality
– Innovation

eyecare in order to enhance patient outcomes,
improve efficiency and release budget.
10. We are different and we can
prove it

specifically tailored to your local patient
pathways and clinical thresholds
We are able to offer support to all parties
involved in the eyecare referral process,

technology, but what really makes us unique
is the fact that we combine this with skill and
expertise that only human beings can deliver
All our clinicians are dedicated
professionals who will ensure our system
helps you and your patients save time
and money

database of hospital
clinics and community
providers that are available
to a patient within their
local area

mainly in their community, having been
accredited by the local CCG
They deliver high-quality service in
specific clinical areas such as glaucoma
or paediatrics

review, evaluate and refer patients to the
community where appropriate

hospital referrals

specialist knowledge is leveraged within
your local optometric community
27.
28. ebill
We can also offer activity audit,
compliant with minimum data set
standards
We can alleviate your administrative
burden by:
– Cross-checking all patient data
– Confirming care costs per patient
– Managing the community billing audit
– Provide a single point billing service
29. Data tracking
We are able to track the volume and nature
of referrals, categorising each in terms of
condition, locality, urgency and age
We can use these data to forecast annual
budget and then offer subsequent control of
this, managing all local referral payments
31. Tried and tested
The Suffolk pilot showed that our whole
system considerably impacts hospital
appointments, and hence substantially
reduces costs to the local CCG
33. Volume reduction
When clinicians know that their referrals
are being studied and double-checked by a
peer clinician, this results in more care
being taken when making referrals
This improvement in standards at primary
care level – combined with the enhanced
services being offered by a community
provider – contributes to the significant
reduction in hospital referrals
34. Reduced waiting times
The knock-on effect of referral reduction is
that patient waiting times are dramatically
reduced
In Suffolk, all patients sent to the community
were seen within 2 weeks
35. Glaucoma analysis
To show the impact our system has had on a
single disease area, results were investigated
in suspect glaucoma
Of the inbound referral activity, 38% were sent to
a community provider
36. Glaucoma (cont.)
Of those patients sent to a community
provider, 73% were safely discharged to
the original referrer

nuances of your trust

system, we make eyecare as efficient as

Editor's Notes
Say hello, introductions, etc.Pose the question, “What are your key goals as a PCT?”.
With any luck, the PCT will have mentioned most/all of these points before the slide is shown.
Hard benefits
Soft benefits
Just one of many quotes from happy customers!
How do things work in your area? Are there aspects which you would like to improve at all?Building on their answers, review these points with the QIPP goals in mind: Can you think of ways to bring care closer to home? Do you think we can intervene earlier? Can we increase standardisation? Can we make patients feel more empowered? Can we do all of this and still reduce unit costs? We want to work with you to make life even better for patients, and help meet your own goals along the way.
We want to improve the standards of optometry in the UK, and we believe we can help you meet your QIPP goals – everyone wins!
Quality = Evidence-based, practice-informed best practice options which allow clinicians to do what is right for their patients. High-quality, individualised treatment, which means consistently improved patient experiences and outcomes.Innovation = The latest knowledge and technology is crucial to offering the best healthcare with limited resources.Productivity = Within areas of limited resource, this requires a holistic approach that thinks about an entire patient journey, not just segments here and there.Prevention = Early diagnosis and comprehensive follow-up can really limit complications, and the resources needed to deal with them. Together, these can reduce waiting times and cut costs. It isn’t a compromise.
The patient is at the centre of everything we do – but there are proven benefits for everyone involved.
Other people might have developed and honed the technology required, but no-one else has the human angle covered like we do.
Let us show you exactly how it works. Our system can be broken down into different stages and you can choose which elements of the system you feel could benefit you, in order to create your own bespoke package.
Where we are now…
All referral data captured and referral criteria enforced with standardised data and clinical content
Our erefer team offer support, help and information at every step along the way – with this system you get the latest technology, backed up by dedicated specialists.Referring clinicians can simply log-on to our service, enter the relevant patient details and submit the form, and then we take it from there. The referral is then visible at all stages of the process, and can be accessed by both primary and secondary care.erefer dramatically streamlines the normal referral process – which saves patients time, and saves you money.
This is the patient support arm of the Evolutio offer. We give patients the care and support they need at every step along the way.We are able to provide patients with any support they need, either online or in printed form.
Glaucoma patient pathway, using local community provider for conducting key tests.
The clinical enhanced services within any given community will be run by community providers. They are able to deliver an enhanced service within their core specialty, demonstrating appropriate core skills, rigorous equipment standards, and competencies to perform these extra services without direct supervision.Referring back into enhanced community providers also cuts down on hospital appointments, and reduces time to care.
All of our centres are manned by accredited and trained clinicians, with annual performance examinations to ensure all our staff are making the same clinical judgements for each condition time after time.An enormous number of optometry services have now been specialised to the extent that some opticians may not be aware of the appropriate specialist, diagnostic test or treatment for their patient. As such, our team can really make a difference in guiding traffic and helping patients get what they need as fast as possible.By referring eyecare patients back into their local relevant CP, they can hopefully be seen faster, by the most-suitable local clinician, without the need for specialist help.Leveraging specialist knowledge in the local community means continued local professional development, and also maximises available resources. Patients get in-house clinical expertise, to ensure they get exactly what they need in a timely fashion.evaluate ensures a more satisfying and cost-effective process.
Due to the fact that we already hold all of the necessary patient referral data within our database from erefer, we can do what no-one else can.All of this means CCGs can easily validate how money has been spent. Our data outputs are vital for future planning of services, as well as analysing local trends and the quality of clinical practice.
We chose glaucoma because: In 2009 the NICE guidance actually lowered the threshold for glaucoma risk, and therefore we have seen a significant increase in hospital referrals in this therapeutic speciality ever since. However, many of these patients could be seen by a CP instead.
