5. What is a PCN (Primary Care Network)
Primary care networks were introduced into the NHS in England as part of
the ‘NHS Long Term Plan’, published in January 2019. The 2019 General
Practitioner contract gave the opportunity for GP practices to join networks,
each with between 30,000 and 50,000 patients.
As part of the NHS 5 year long term plan, primary care networks will bring
general practices together to work at scale. (Across Manchester 220, 000
patients) There are other networks geographically based and cover all
practices within a clinical commissioning group. (CCG)
Our PCN is made up of the following
The Docs |The Arch Medical Practice |Cornbrook Medical Practice.
6. • What are they?
What are
they?
Primary care networks form a key part of the NHS plan
The link between general practices and the wider teams.
Created to work and collaborate with federations (Finance/service
managers) and CCG
Improve
Services
What will they
do?
Deliver 7 of the national Specifications: structured medication reviews,
enhanced health in care homes, anticipatory and personalised care,
early cancer diagnosis, cardiovascular disease case finding and tackle
inequalities.
Join up the delivery of urgent care in the community
Improve the ability of practices to recruit and retain staff, manage
finances, provide a range of services to patients and to easily integrate
with the wider health care system.
Deliver high
quality care
7. Working at Scale
Finding new ways of working that will enable us to meet the increasing challenges and
demands placed in the NHS. Services can be approved and deployed across a number of different
towns/districts.
Will be able to offer improved services
Extend the services available to patients
Integrating primary care with wider health and community
services
More joined up preventative and personalised care
What are the benefits to our patients? What are the benefits to Practices?
Connecting large numbers of people together and ensures
there are robust routes for face to face & digital
interaction, feedback and learning, to support the
development of centralised training and education.
Ability to build on the strengths of practices, allowing them
to share resources, expertise and services.
Forming an organisation that can tender for services
Improving integration of services across practices and
other providers

• Moving our systems to online forms has given patients easier access
to support.
• Our phone lines are notoriously busy and the landline demand means
that people can struggle to get through to us.
• Patients completing online forms means that care can be steered in
the right direction, triaged at an earlier opportunity.
12. Registrations
• We currently have 9174
patients registered at The
Docs.
• 491 OUT OF AREA patients
• 218 patients currently
waiting to be registered
with us.

Please be aware that registrations currently take 3-4 weeks to process. This is
due to the incredibly high volume of applications and is unfortunately beyond
our control.
13. Prescriptions
• Still receiving paper prescriptions in bulk,
from Pharmacies.
• We will often see duplicate requests,
from patients and Pharmacy.
• We have stopped processing Scripts after
11am, to ensure the GP’s have time for
Admin tasks.
15. Status at The Docs
We are still asking the patients wear masks on site and when seeing a clinician.
We have been lucky enough to only have had a couple instances of staff with COVID.
We have reacted quickly to positive tests. We have a rigid clean down protocol so have
been able to carry on with our service ensuing staff and patients are safe.
Patients booked in for appointments are sent a Covid questionnaire before attending
the practice, this will remain in place.
We have in place a team of staff, processing and ensuring all patients eligible for the
vaccine have been offered 1st & 2nd doses.
