• any sch 3 4 5 drug rx should be stamped with ‘Red C’ one inch tall in lower right
corner.keep on file 2 years.
•also to be kept--
•biennial inventory of narcotics
•RpH in charge inventory
•sch II invoices attached with Form 222
•sche II invoices are filed away from sch 3 4 5
•exempt narcotic log---name and address of buyer, drug, date, price--RpH must see
transaction.
•master formula sheet---compounding paperwork
•MSDS
•MAR--medication administration record
•nonsterile compounded records
•poison log--name address, drug, use, buyer signature
•rx hard copy ( has initials of whoever filled rx)
•repacked meds
2. paperwork?
• rx records can be electronic or paper
• if all electronic rx system then no C required
• if paper there are 3 ways to store records
• system 1= sch II drugs goes to file#1 sch 3 4 5
goes to file#2 all other rx goes to file#3
• system 2=sch II goes to file#1 sch 3 4 5 and all
other legend drugs goes to file #2
• system3= sch 2 3 4 5 goes to file#1 all other rx
goes file #2

must override this.
