Total Hip Arthroplasty
Despite contrary belief, hip replacement is not necessarily negative. Many Americans believe that in
order to have a hip replacement, your condition must be terrible. A survey conducted in 2012 said
that almost 1/3 of respondents to the hip replacement survey felt they would have benefited from
having their surgery done earlier. This procedure has and continues to change the lives' of many
people in our world. Hip replacement, or arthroplasty, is a surgical procedure in which the diseased
parts of the hip joint are removed and replaced with new, artificial parts. These artificial parts are
called the prosthesis. The goals of hip replacement surgery include increasing mobility, improving
the function of the hip joint, and relieving pain. According to the Centers for Disease Control and
Prevention, 332,000 total hip replacements are performed in the United States each year. "Hip
Replacement." Questions and Answers about. N.p., n.d. Web. 21 Oct. 2013. What were once
considered high–tech, joint replacements are now a common operation. Surgeons replace more than
a million hips and knees each year in the U.S. Studies show joint replacements can significantly
relieve pain and increase mobility in about 90% of people who get them. Feature, R. Morgan,

Physical Therapy And Specific Surgical Approach Essay
The objective of this research is to determine whether physical therapy and specific surgical
approach will affect the recovery period among the patients who underwent the total hip
replacement. The results which describe hip mobility and function after the surgery, as well as
overall patients' satisfaction, are compared between various randomized and non–randomized
studies, usually with the presence of experimental and control groups. Major common outcomes
reported were pain, muscle strength, mobility, range of motion, and gait limitations. Based on these
criteria it was determined that post–operative physical therapy exercise program significantly
increases the time of recovery and improves the results of the surgery compared with patients
receiving usual care. At present time, no study showed enough evidence to prove absolute advantage
of anterior approach over the posterior as improvements were observed in both cases. This research
contains important information that can be useful for patients awaiting total hip replacement.
Introduction
Total hip replacement (THR), also known as total hip arthroplasty (THA), is commonly performed
surgical procedure for treatment of the late stage of osteoarthritis of the hip joint throughout the
world. Arthritis causes damage to the articular cartilage of the femur and leads to reduced joint
mobility and pain. Total hip arthroplasty involves replacement of worn cartilage with an artificial
ball–and–socket joint. The surgery provides

The Effect Of Glass On Everyday Life
3. Discussion
3.1. Effect of Glass on Everyday Life
Mirrors and their applications were pivotal in the development of the sciences of optics, chemistry
and astronomy (Rasmussen, 2012). It has even been said that the Renaissance and the Scientific
Revolution might not have happened if not for the invention of mirrors (MacFarlane, 2002).
Eyeglasses have been said to be one of the greatest inventions ever because without them, people
born with poor vision not only find performing daily tasks much more difficult, but would also be
severely constrained in terms of the amount of education they can receive and thus in their life
opportunities.
This is further underlined when one takes into account the fact that approximately 6 out of 10 people
in the world today either wear glasses, contact lenses, or have previously undergone corrective eye
surgery. Even for those born with good vision, they typically lose their ability to focus by their mid–
40s, which means that eyeglasses have considerably lengthened the intellectual lifespan of a person.
The impact of eyeglasses on society thus cannot be understated.
The impacts of the microscope and telescope are massive, especially to the fields of medicine and
science. The modern discoveries ... Show more content on Helpwriting.net ...
Bioglass is deemed more suitable to be implanted in bodies because of their inert qualities as
compared to metals' corrosive and reactive nature. Compared to the way metals replace tissue,
Bioglass value–adds the replacement process through the ability to bond damaged tissues, resulting
in healing functions of bones ("Work advances on bioglass replacements for metal implants", 2013).
As a result, Bioglass is widely adopted for healthcare purposes in terms of tissues replacement as
well as for repairing of bones due to their better suitability than metal

Personal And Professional Nursing Practice
Clinical Question Paper The nursing profession is one that is ever changing. As a profession that is
created and based upon scientific research, the way in which nursing care is delivered is continually
being modified and altered to reflect the latest and most up–to–date evidence based research
findings. However, not all research findings are going to be applicable or appropriate for integration
into nursing care. Consequently, the ability for nurses to be able to interpret and understand these
research findings is also important, in order to make the most informed, educated, and
knowledgeable nursing decisions before integrating such research findings into clinical practice.
Therefore, the purpose of this paper is to reflect on how nursing knowledge is dispersed and
disseminated in both personal and professional nursing practice. Clinical Question When in comes
to enacting a possible change in nursing practice, a clinical problem or issue needs to be identified.
Once a problem has been acknowledged, the process towards change is one that involves a
multifaceted approach. The development of a clinical question is the first step in the process. In the
nursing profession, this question is typically constructed using the PICOT format. Developed by
Fineout–Overholt and Johnston, this five–letter acronym stands for: P = patient or population, I =
intervention or issue of interest, C = comparison intervention or current practice, O = outcome
desired, and T = time need to achieve

The New Revolution Of Food
Is Soylent bringing an end to our everyday food? Even though this answer is not clear, there are
many positive and negative speculations about new discoveries in nutrition such as Soylent. Many
people have found ways to improve the quality of our diet and have completely innovated the world
of food. This new revolution of food seeks to enhance our quality of life and to make food as
beneficial as possible. However, this dream can also be perceived as a nightmare, for this revolution
may completely take over our diet and change the way we use food. This change causes much
discomfort among people who have grown accustomed to the food they see everyday, and they do
not want to fix something that has not been broken. Products such as Soylent represent a future of
food fueled by hopes and dreams, but if we are not careful, this dream can quickly turn into a
nightmare. The story Soylent represents an example of how an invention so simple can make an
impact so immense. To clarify, Rhinehart broke down the complicated world of food to the basics
which he turned into a cheap yet efficient product. Widdicombe states, "food costs had dropped
from four hundred and seventy dollars a month to fifty." Hence, this steep drop in monthly expenses
spent on food has enticed many people to believe in the Soylent dream. Not only does this method
of creating cheap food help address our problem of worldwide hunger, it also helps those in tough
financial situations like college students. In

Essay On Total Joint Replacement
Background of Total Joint Replacements and Wear Debris: Total joint replacements are very
common in the United States, and are becoming even more common with increasing obesity rates
and an increase in life expectancy. Long–lasting and high–performance implants are needed for the
younger, active population that receives them. The Agency for Healthcare Research and Quality
states more than 285,000 total hip arthroplasties (THA) and 600,000 total knee arthroplasties (TKA)
are performed in the United States alone each year (2). Longevity of these joint replacements is
dependent on several factors: the durability of the fixation of the implant surface to the bone, the
bearing surface's wear rate, and the accuracy of the surgeon in implantation of the total joint (2). Of
these total joint replacements, periprosthetic osteolysis and aseptic loosening occurs in about twenty
percent of cases, and aseptic loosening is responsible for about seventy–five percent of total joint
replacement failures (2). This is ... Show more content on Helpwriting.net ...

Suggestions For Choosing Replacement Windows For Your Cabin In The Woods
If you're planning to renovate a cabin in the woods so you'll have a place for weekend getaways,
you'll probably want to replace the windows with more energy efficient versions. You'll also want
the windows to be durable and require little maintenance in the years to come. Here are some some
suggestions for choosing the ideal replacement windows for your cabin.
Consider Aluminum Clad Wood Frames
Aluminum clad wood frames are a good match for cabins because they give your place the
traditional appearance of wood on the inside, but the outside has a protective layer of aluminum.
These windows are made with wood on the side that faces indoors and metal on the exterior side.
The metal is available in different colors, which is a plus if you want to match the color to your
metal roof or add a touch of color to accent the cabin's wood exterior.
The biggest advantage to having these windows is that they require much less maintenance than full
wood frames. The exterior metal surface won't rot or attract ... Show more content on

However, you want the windows to be as secure as possible when you are away. While it is difficult
to make your windows 100% burglar proof, there are some things you can do to make the glass
difficult to break. You could choose glass that is impact resistant or you could have security film
applied to the glass. Another option is to buy windows made of acrylic that is transparent like glass
but much stronger. You don't only have to worry about intruders breaking into the windows. If the
cabin is surrounded by trees, a storm could knock a branch into the glass and break it if you don't
have security glass. Animals may even bang against the glass trying to get inside the cabin. For
these reasons, you want glass that resists breaking, or glass that can't be knocked out of the frame
even if it

Obamacare Persuasive Essay
Several of us are watching the news right now waiting to see if Senate Majority Leader Mitch
McConnell and Congress get the 50 out of 52 votes to repeal legislation and to begin the Obamacare
debate. After the votes are counted the debate will start for Obamacare to be repealed or to not be
repealed. Obamacare, also known as Patient Protection and Affordable Care Act, was signed in
March 2010 by Obama to "Include multiple provisions that would take effect over a matter of years,
including the expansion of Medicaid eligibility, the establishment of health insurance exchanges and
prohibiting health insurers from denying coverage due to pre–existing conditions." ("Affordable").
Therefore, this debate will show what Obamacare does for the people of the U.S, what will replace
Obamacare if it is repealed, and if it is worth all the hassle of repealing and replaces it with Trump
Care or to not replace it. Obamacare should not be repealed because several Americans are relying
on Obamacare's cost assistance to afford health care. Many citizens often ask each other, "What is
Obamacare?" To be quite honest, not many people could explain to each other what Obamacare is.
The main goal of Obamacare is to give access to more affordable health insurance and to reduce the
growth of overall spending on health care in the U.S (DeMichele, "What is Obamacare?").
Obamacare may seem like every other bill that can be repealed or replaced but it is not. Citizens
lean on Obamacare to give them the

Nursing Assessment And Comprehensive Post Operative...
Hip fractures are one of the most common causes of extended hospital stay among the elderly, and
approximately one third of patients undergoing hip arthroplasty surgery will die within the first year
(Gregersen et al., 2012). The specialised nature of the orthopaedic ward is a complex environment
for not only nurses, but for all health care professionals. It is the role of the registered nurse within
the orthopaedic ward to provide holistic assessment and comprehensive post–operative planning to
facilitate client centred care for an elderly patient undergoing a total hip arthroplasty (Walker, 2012).
The purpose of this essay is to identify and prioritise appropriate nursing assessments and care
provided for an elderly man (Mr. Simons) transferred to the Orthopaedic ward from the Emergency
Department via operating theatres. The discussion will initially consider the environment of the
orthopaedic ward and the role that the nurse must assume to provide high quality care. Focusing
briefly on the ageing population and the impact that hospitalisation has on the elderly, the essay will
then rationalise and prioritise current nursing assessments considering the primary and secondary
assessment strategy and consideration of major body systems. Moreover, the discussion will detail
and rationalise the appropriate management of patient safety, comfort and communication with
substantiated planning of care by the registered nurse in the first eight hours of stay. The post–

Osteoarthritis Essay
Osteoarthritis, the most common type of arthritis, is most prevalent in older patients. Osteoarthritis
is a degenerative bone disease due to the gradual loss of cartilage. A primary type of osteoarthritis is
hip arthritis where it is caused by joint injury, increasing age, and being overweight ("Hip
Osteoarthritis," n.d.). However, osteoarthritis can also be caused by immature joints, inherited
defects in cartilage, and extra stress on a patient's joints (Hip Osteoarthritis," n.d.). As a result, hip
arthritis becomes a huge detriment in patient's social, emotional, physical lifestyles. In order to treat
hip arthritis, doctors choose from a variety of non–drug treatments, medications, and surgeries.
Uniquely, I was inspired to research about the treatments of hip arthritis because my very own
brother was pronounced with hip arthritis a few years back. Therefore, his determination to battle
this disease encouraged me to investigate about the treatment of hip arthritis.

Case Study On Knee Replacement
1. My mother has knee pain for many years and recently it has worsened. She has some deformity
around the knees and can barely walk more than 10–15 minutes. What should be done?
Recent worsening of chronic knee pain and deformity probably indicates that knee arthritis has
reached advanced stage i.e. the protective lining of the joint is nearly completely worn out. At this
stage, pain medication often doesn't help much. Rather, one may suffer from dangerous side effects
of prolonged pain medication therapy on heart, liver, kidney, stomach and intestines. It is
recommended that one should consult a joint specialist and get certain X–rays. If it is advanced
arthritis, then one should consider knee replacement to get rid of the problem.
2. What do you mean by knee replacement?

Joint replacement is entering an era of greater minimally invasive alternatives, which provide
patients with options to minimize recovery time and financial expense. The three major costs of
joint replacement surgery include the price of the implant, extraneous hospital fees, and
postoperative rehabilitation expenses [1]. Although minimizing length of stay would reduce these
costs, several factors traditionally necessitate overnight hospitalization for arthroplasty patients,
including pain control, blood loss, and monitoring of comorbidities. However, with recent

Titanium Alloy Implants and Joint Replacements
The advancement in materials science has paralleled with human ability to make improvements in
the fields of engineering, manufacturing and medical fields. Specifically in the medical field, as
major joints or sometimes organs begin to fail, foreign materials must be introduced into the body to
perform the functions of the failing component. Not all materials can be used in the human body
however, therefore materials science is directly correlated to the success of implants. This report
discusses some of the challenges faced when selecting the most ideal material for an operation, and
investigates many of the material properties of common joint replacement/implant materials.
Understanding further the certain requirements of proper implant materials used in practice will
permit greater advancement in the field of medical implants and joint replacement once other
materials are created to better fit those requirements. There are three main areas which a certain
material must prove to be superior in a given application in order to be considered for use in
implants and joint replacements. First and most importantly, the material in question must be
compatible with the human body. Introduction of foreign materials into the human body causes a
reaction between the foreign material and human tissue, we consider the material compatible so long
as this reaction is not producing negative or harmful

Observation of a Failed Hip Replacement
Case involves a 60yr old man who underwent an uncemented total hip arthroplasty or a total hip
replacement, who was referred to a home PT upon d/c instructing the patient to start home PT &
begin ambulation using the 'toe touch weight bearing' which was incorrectly mentioned in the d/c
summary as 'weight bearing as tolerated'. For the initial 6 weeks, a patient with uncemented
prosthesis should begin with 'toe touch weight bearing' and not 'weight bearing as tolerated' unlike
the cemented prosthesis1. Toe touch weight bearing is instructed to the patient to protect the joint &
also give bone the time to grow into the porous coating of the implant2. During the patient's visit,
the PT developed a plan of care from his evaluation & examination and handed–over the case to the
PTA to carry out the therapy interventions. PTA (Physical Therapy Assistant) is defined as an
educated individual who works under the direction & supervision of a physical therapist3. From the
mentioned case, it is clear that the PT has not read the d/c summary and also that he has not
mentioned the type of prosthesis used whether a cemented or an uncemented which highlights the
PT's failure of proper documentation. PTA has elected to follow a plan of care that she felt was
consistent with that of a cemented prosthesis, highlighting the PTA's mistake because the PTA is not
allowed to make any modifications and follow the plan of care as directed by the supervising PT.

Reflection Paper On Writing And Writing
Writing has consistently been a shortcoming throughout my life. The content of my paper was
typically not the issue, the organization of the ideas was. I always had good ideas to write about, but
I never knew how to organize those thoughts into a well–organized paper. Although in Writing 101,
I used the multiple steps of the writing process to insert my ideas into a well–written paper, I
continued to struggle in areas such as grammar and mechanics. In Writ 102, I was able to realize that
the writing process develops over time, and the best papers result from revision over time. The
multi–step writing process that was required for each paper this semester allowed me to develop
fundamental revision skills over time that made my writing reach its full potential. When I was first
assigned the analysis essay, I was relieved because I had completed a similar paper in Writing 101
the previous semester. I assumed I had this paper in the bag because it was a familiar topic. When
composing the first rough draft, I was focused on reaching the required page count, not the
mechanics or content of my paper. I realized, when I reexamined the first draft, that I lacked the
primary content of the paper. I did not include an analysis of my product's advertisement. Since I did
not analyze my product's advertisement at all in my first draft, in my second draft I added two
adequate paragraphs that analyzed the ad. By adding the analysis, I rectified the lack of content in

She was scheduled for total hip arthroplasty three days following admission. The daughter wanted to
proceed with the surgery, and the son did not. The ethical concern in this case hung on two issues.
First, the patient demonstrated a sufficient degree of dementia to ask "will the patient be able to
adhere to instructions and hip precautions following surgery." Additionally, given her degree of
debility due to her non–ambulatory status before the injury "would it be in her best interests to
proceed with the surgery?" As a team, the nurse, social worker, and I discussed these concerns with
the orthopedic PA who agreed that perhaps the fracture should just be allowed to heal on its own
instead of going forward with a costly, painful surgery and extended rehabilitation that may or may
not produce the desired outcome. The PA, nurse, social worker, and I met with the orthopedic
trauma surgeon and the head of orthopedics and presented the problem. Using a duty–based,
deontological framework for ethical problem solving, and the principle of beneficence (Marquis and
Huston, 2012), it was agreed that in this particular instance it would be best to let the fracture heal.
As a team, we spoke with the family and explained why this choice was the best for their mother.

Total Hip Arthroplasty Essay
During the weeks of February 7 through February 17, I observed a total hip arthroplasty on a 56–
year–old Caucasian female patient who suffered from a femoral neck fracture and damage to the
acetabulum. The fracture was a result from a car accident where the patient's knees collided with the
dashboard, forcing the femur into the hip and breaking the femur. For this report I will concentrate
on total hip replacement, its components, main surgical technique, and complications. Sir John
Charnley first developed total joint arthroplasty in the 1960s (Skinner 395). In a total hip
replacement "the articular surfaces of the acetabulum and femoral head are replaced" (Lemone
1241). A prosthesis is then used to replace the entire head of the femur ... Show more content on

The acetabular components have a "spherical outer surface with at least one hole to permit the
surgeon to determine if the prosthesis is fully impacted into place" (Skinner 399). The inner surface
of the acetabular component then locks the UHMWPE to limit rotation and dissociation (Skinner
399). There are two crucial factors when choosing the implants. First, if the prosthesis is too stiff
then the patient may suffer from proximal osteopenia. Second, the stiffer the prosthesis is the more
likely the patient will experience pain in the thigh. To avoid these two factors the surgeon should
elect to use titanium ally instead of the cobalt chromium alloy.
There are several surgical techniques used for total hip replacement, including the posterolateral
approach, the lateral approach, the anterolateral approach, and others. For this paper I will
concentrate on the posterolateral approach since it is the most common (Skinner 396). The patient is
put into the lateral decubitus position with the affected side superior after anesthesia is administered
and a compression stocking is placed on the unaffected limb. The skin is covered with an adhesive
drape after the incision is outlined (Skinner 396). Then the hip is flexed to 45 degrees so the
"incision can be made in line with the femur from approximately 10 cm proximal to the tip of the
trochanter to 10 cm distal to the tip of the trochanter" (Skinner 396). The incision

"Wow" My mom said glancing to the side of the road and behind a large grey building. "It seems so
barren now." she said shifting her eyes back onto the road, her hair fluffing up as she shakes her
head. "I can't believe they're chopping down so many trees." I heard sadness bubbling up in her
voice as the traffic light turned green and my mom drove the car forward. I look back at the empty
plot of land, a place that used to be packed with trees, trees that had been there for hundreds of
years. I watch the desolate yard of stumps fade off behind us as we continue on home. The thoughts
still roam around in my head. How many trees does it take before they stop? One more? Ten more?
A hundred more? Or will the only reason to stop be the extinction of them as a whole? My brain
pans back a couple years. I think back to the fluffy memories, back when my mom was a babysitter.
When our house used to be jam packed full of kids, some laughing, some crying. It would get so
crowded that we would have to migrate out of the house like air flying out of a balloon. Sometimes
we would go to the park or walk around the block, but most of the time we would go play in our
backyard. Our backyard has never been the most very interesting on its own, but it does have one
noteworthy feature. Behind the chain link fence in our backyard, the world opens out into a lush
green space. This space has always been full of trees, black berry bushes, and plenty of squirrels. So
as young kids, me and the other

Total Hip Arthroplasty ( Tha )
Introduction:
Total hip arthroplasty (THA), commonly known as hip replacement, is a reconstructive orthopedic
procedure that involves the surgical excision of the head and proximal neck of the femur and
removal of the acetabular cartilage and subchondral bone(A). The damaged joint is replaced with an
implant that mimics the motion of the natural joint and is made from combinations of metal, plastic
and/or ceramic components(D).
Joint replacement is one of five priority areas targeted for shorter wait times in Canada. Ontario's
current wait time is 207 day.(F) The number of THAs in Canada increased 11% between 2006–2007
and 2010–2011. In 2013–2014, there were 49,503 hospitalizations for hip replacements(B). THA has
been used to manage conditions in which the hip joint has responded poorly to less–invasive
treatments(D). The most common of which is severe osteoarthritis of the hip joint, which affects
more than 10% of Canadians aged 15 or older(G), and accounts for 70% of cases. The procedure has
proved to be remarkably successful in eliminating pain and restoring function in hips severely
involved with diseases, improving mobility, daily functioning and quality of life(A,D,G). THA
provides pain relief and improves physical function and quality of life in patients with end–stage hip
osteoarthritis. The incidence of THA is expected to increase due to the growing elderly population.
Nevertheless, THA is becoming more common in younger populations because of the improvements

Flat Irons-Personal Narrative
One summer ago, I discovered the truth behind the old saying, "Forever doesn't last too long these
days." I felt as if forever had been cut short like the strands of life were by the Furies.
I remember it like it was yesterday I had just bought a new flat iron. We were walking through the
outlet mall in Orlando, Florida. Lost and confused by all the hustle and bustle of it, I tried to remain
optimistic. I always try to steer clear of the booths in the middle of a mall as the salespeople are
incorrigibly relentless. Suddenly, I was seized by an Italian–speaking woman. Immediately, she
hauled me to her booth, and I could not escape. For fear of us leaving, she forcefully seated me.
Consequently, my family was forced to follow and participate. ... Show more content on

Almost finished, I looked up to note my accomplishments. I was impressed. My thick, coarse hair
was silky and had body. I couldn't wait to show it off. Every time I needed my hair to look salon

next day and was depending on Royale with my life. I absolutely could not go to school with my
hair half afro, half straight. A ponytail simply would not do. I started crying because the flat iron
would not turn on. I tried everything I could imagine. I cursed the Italian saleswoman, and the
creators of the Royale for filling my innocent mind with lies about a better future. I was about to
give up on life when I remembered I still had an old flat iron. It may not have been new and
improved, but it got the job done.
When I returned home from picture day, I immediately phoned the Royale company about the
lifetime policy. I did not know where my mother brought it from nor where her warranty paper was.
The Representatives were not helpful and could not assure me a new flat iron. I was heartbroken.
After all the promises and guarantees of no hassle replacement, the company did not follow

Evidence-Based Practice
PSYC40014 Advanced Research Methods in Psychology assignment 1

University of Melbourne
Word count: 1,546 What is Evidence–Based Practice and why we should promote it
Evidence–Based Practice (EBP) was initially a movement in medicine, dating back to early 20th
century (Spring, 2007). It was described as a careful decision making process about the care of
individual patients using best available evidence by Sackett, Rosenberg, Gary, Haynes, and
Richardson (1996). Greenhalgh (2010) gave a similar definition but with more emphasis on the
mathematical assessment of the potential benefit and harm. By now, EBP has been widely adopted
by many health disciplines, including psychology. In 2005, the American ... Show more content on

Klein (2014), a research psychologist, argued in his article on Edge that EBP should be abandoned.
This article will address each of the four points he made in the article.
The first point Klein (2014) made was that research evidence is not always a completely reliable,
and therefore shouldn't be trusted. This is an invalid argument for several reasons. It is true that
research has limitations due to a number of reasons: the nature of the research questions, funding
restriction, random sampling availabilities, ethical restrictions, and limitations of statistical
techniques. Science is not perfect, some studies are less reliable than others, and we will never be
certain if the conclusions are true or not, but what is the better alternative? Without EBP, the clinical
decisions can only be based on some novel ideas or / and clinical observations, but they are often
wrong. Prefrontal lobotomy, a surgical procedural that cuts off neural connections in the prefrontal
cortex, was based on a novel idea; it was thought to be effective in managing undesirable symptoms
of patients with mental disorders, and it seemed to work as some patients did have reduced
symptoms. However, it improved some of the patients' symptoms but left profound long–term
damage to most of the patients. Those patients had personality change, inability to function in the
society, apathy, lost of initiation and inhibition (Freberg, 2010; Szasz, 2007). No research evidence
will ever be completely

How A Buyer Has Numerous Benefit Options For The Procedure...
A buyer has numerous benefit options to choose from. The following are the suggested options:
1. A Center of Excellence (COE) Only Benefit:
Under this benefit design, patients are signified to a COE for evaluation and determined to see
whether they are applicable aspirants for a knee or hip replacement procedure. If the candidate
qualifies for the procedure, the benefit is enclosed and the provider is paid a bundled payment. If an
individual does not qualify for the procedure, there is no insurance benefit for the procedure at any
facility. 2. Reference Pricing:
Under reference pricing, a benefit price is established for a set of services. Some providers will bill
at or below the reference price; some providers will bill ... Show more content on Helpwriting.net ...
These approaches give the patient a broad choice of providers and some financial incentive – in the
form of lower cost–sharing – to seek care from a provider participating in the bundled payment
program. However, to create enough "steerage" to these providers, the incentives will have to be
significant; providers may think twice about participating in bundled payment if patients still have a
broad choice of providers.
Purchasers are likely to be more successful at getting providers to accept bundled payment if
providers can expect greater patient volume when they do. To achieve maximum savings and better
patient outcomes by driving volume to high–value providers, CPR recommends benefit design
approaches that create incentives for enrollees to seek care from them. Options 1 and 2 above are
examples of benefit designs that are more likely to yield higher results than those strategies that
offer modest benefit design differentials between a bundled payment provider and a non–bundled
payment provider. In the end, determining your benefit design strategy comes first; designing and
developing the network will follow.
Employee education and benefit designs that either encourage or require the use of treatment
decision support (TDS) tools can help consumers feel better about their treatment decisions and
prepare them for more aggressive benefit strategies that steer patients

The Incidence Of Aki Between Two Perioperative Antibiotic...
Anecdotal evidence suggested an increase incidence of AKI when gentamicin was used in
combination with cefazolin and vancomycin for perioperative prophylaxis of elective hip and knee
replacements. As post–operative AKI is associated with increased morbidity and mortality, the aim
of this study was to compare the incidence of AKI between two perioperative antibiotic regimens in
patients undergoing total hip arthroplasty (THA) and total knee arthroplasty (TKA).
Methods
This was a retrospective cohort that evaluated 300 patients who underwent elective hip or knee
replacements that received cefazolin, vancomycin and gentamicin or cefazolin and vancomycin for
perioperative antibiotic prophylaxis. The RIFLE classification was utilized to ... Show more content

There was no significant difference in rates of PJI, CDAD, length of stay, need for acute
hemodialysis, or readmission related to AKI.
Background
There were more than 1,000,000 total hip and knee replacements in the United States in 2010.1
Prosthetic joint infections (PJIs) are a rare complication of joint replacements, estimated to be less
than 2% of cases, but result in significant morbidity and increased healthcare costs.2 Frequently, PJI
requires removal of the prosthesis and a prolonged course of IV antibiotics. It has been estimated
that the cost of one PJI is approximately $100,000 after a hip arthroplasty and $60,000 after a knee
arthroplasty.2 Perioperative prophylactic antibiotics for total joint replacements have been proven to
reduce rates of surgical site infections.2–3 It is recommended that prophylactic antibiotics are
administered within one hour prior to surgical incision and are discontinued within 24 hours
following the end of surgery.4 First generation cephalosporins, such as cefazolin, are the agents
most commonly utilized for antimicrobial prophylaxis in joint replacement procedures.2,5
Vancomycin may be used in addition to a first generation cephalosporin in institutions considered to
have a high rate of methicillin–resistant staphylococcus aureus (MRSA) surgical site infections. 2 If
there is local data to suggest gram–negative pathogens as a cause of PJI then it is recommended to

Nursing : A Total Knee Replacement ( Tka ) Is The Most...
Perioperative Nursing Written Paper
Robyn Weiner
New York University College of Nursing
Spring 2015
A total knee replacement (TKA) is the most common joint surgery performed in the United States
(Turner, 2011, pp. 27–32). Each year, over 650,000 Americans undergo this surgery (Wittig–Wells,
2015, pp. 45–49). It is an invasive surgery that involves an incision on top of the knee and replacing
damaged parts of the knee with artificial parts that are either metal, ceramic or plastic. Someone
would get a total knee replacement for damage of the joint, osteoarthritic, posttraumatic, or
inflammatory arthritis. The cartilage is damaged, wears away and then you develop bony deformity
and contracture of ligaments but it starts out with specific defects or wear of cartilage. The top
nursing priorities for a total knee arthroplasty is to "prevent complications, promote optimal
mobility, alleviate pain, and provide information about diagnosis, prognosis, and treatment needs"
(Doenges, 2014, pg. 627). A possible nursing diagnosis from the patient who is undergoing a TKA
might be 'impaired physical mobility related to pain and discomfort as evidenced by reluctance to
attempt movement.' Another one could be 'acute pain related to chronic joint disease as evidenced
by reports of pain' (Vera, 2014). The preoperative phase begins when the patient is scheduled for
surgery. The role of the perioperative nurse is to first determine any risks the patient may

Assessment And Management Of Leaky Wounds Following...
Assessment and management of leaky wounds following prosthetic joint replacements: a
retrospective study of prospective surveillance.
Authors: Sudhir Kannan, Jason Wilson, Grahame Taylor
University Hospitals Of Leicester.
Department of Musculoskeletal Surgery (Leicester General Hospital).
Introduction:
Persistent wound discharge following joint replacement is a known complication. Any
discharge from the wound for > 4 days is defined as persistent leakage. Our literature
showed that the incidence of persistent discharge from the wound ranges up to
14.3%(Gaine et al), wound complication which require surgical treatment is
0.33%.(Galat et al).
Galat et al (2009) conducted a retrospective analysis of 17,784 primary total knees
replacements and report that the incidence of wound complications requiring surgical
treatment was 0.33%. Gaine et al reported that the incidence of superficial infection and
early deep infections following joint replacement were 14.3% and 1.1% respectively.
We could identify several classification systems for leaky wounds. However, these
classification systems have been devised by individual authors, which have not been
repeated between papers. To be specific there is no universally accepted classification
system.
The leaky wounds often lead to treatment dilemma, prolonged hospital stay and
delayed return to activity (Gaine et al). Besides, it causes stress to the patient and the
operating surgeon. There is also

Joint Replacement: A Case Study
Mandatory Medicare Bundled Payment Plans
I. In July 2015 there was a proposal issued for a new Comprehensive Care for Joint Replacement
program. This proposal is designed to establish bundled payments fir total hip and knee
replacements. The program for Comprehensive Care for Joint Replacement (CCJR) is a crucial
development for Medicare and Medicaid holders.
II. Bundled payments create financial incentives for providers and benefits patients. Providers will
be influenced to coordinate care for the entire service provided, pre–operation, during, and post–
operation. This program will benefit the patients in eliminating costs that do not benefit them or
relate to their care. For example, patients can be directed to use home care as opposed to
institutional care if suitable.
III. It is the opinion of medical professionals that bundled payments such as the CCJR proposal were
inevitable. "Hip and knee replacements are some of the most common surgeries that Medicare ...

The proposal for bundled payments (CCJR) will force hospitals and other health care facilities to
change and adapt. The proposal would include medical severity diagnostic–related groups which
would help calculate targeted prices for each severity group and each hospital separately. Several
controversial components would be included in the proposal. Mandatory participation is one of the
key requirements to the proposal. Another controversial component to the CCJR program is that
hospitals would be exclusively responsible for the bundled payment program and any financial
excess. However, these controversial components are key features to ensuring the proposal's success
which will help patients and providers in the future. Another reason the CCJR proposal will force
hospitals to adapt is that the hospitals would be financially accountable for the quality of care. If the
hospitals fail to meet three specifically designed protocols for quality, the hospital(s) would be

Dane Miller– The Maverick CEO
Great leaders are all defined by different attributes, successes, and contributions made to their
professions, communities, and stakeholders. The term "Leader" is subjective and varies greatly in
the opinions among various individuals. In this paper, Dr. Dane Miller, my selected CEO, will
undergo a leadership assessment based on the ROCC of Trust and various other qualities. Biomet

This seems extremely simple but the fact that a CEO like Miller was concerned with this degree of
detail and the fluidity of the procedures reflects how dedicated and authentic he was as a leader.
Humility is a personal favorite trait that I seek in others and that I see in Miller. During his early
career he took a job at Arby's for one reason; their policy gave each employee two sandwiches a day.
He would eat one and take one home to his wife. Dane Miller has followed up this statement by
saying "Behind every successful man is a great woman–and a surprised mother–in–law!" (Adams).
Although he was extremely successful, he continuously gives credit to other people for his
achievements. He applauds his wife for her encouragement but also gives credit to community. It
takes a courageous leader to recognize his or her blessings and multiply those back to others. He has
given over 30 million dollars of his own money to the restoration of his community through various
nonprofits, community organizations, churches, and educational scholarships. In The Maverick
CEO; Dane Miller and the Story of Biomet, he quotes William James, "The best use of life is to
invest in something which will outlast your life" (Adams).
The success of Biomet as an organization can be traced to Dane Miller and his contributions through
his leadership. Miller enhanced Biomet socially, demographically, and

Robots During The Industrial Revolution
Innovations in technology from the Industrial Revolution to today have changed work and the
workplace. Robots and other automated machines do many human jobs. One study concluded that
about 50 percent of US workers could be replaced by robots in the next 10 to 20 years (Jones, 2016,
para. 3). Even though robots will replace some jobs done by humans, there will be new and higher
skilled jobs for workers. Some people worry that robots will have a negative impact on humans in
the future. However, people have had this concern for a long time (Shah, 2015, para.15). During the
Industrial Revolution, workers demolished machines in the factories because they worried that their
jobs would be replaced by machines. In the manufacturing industry, machines decreased or
eliminated the need for workers. Governments tried different ways to slow or stop the development
of new technology because of their concern over job loss. In 1638, England's government legally
prohibited certain machines. And during the Depression, the US government imposed a tax on
companies that replaced workers with machines. But new technologies do not necessarily mean that
workers who are replaced by machines will not have jobs (Volti, 2012, 80–81). History has shown
that even when jobs are lost to machines, new higher skill jobs are created. For example, even
though manufacturing jobs were lost in the new economy, new jobs were created in the service
industry (Volti, 2012, p. 56). Robots increase the efficiency of

Traction: The act of drawing or pulling an articular segment to produce separation from another
articular segment. Mobilization: the process of passively restoring capability of movement.
Combining the two, Traction mobilizations are an incredibly beneficial tool in helping individuals
move correctly and without pain by restoring physiological function and accessory motion of the
specific joint. That nagging hip pain that you've had from time to time would benefit from traction .
All those tight muscles could use some relaxation, they could use some traction. Some indications
for traction mobilizations include pain, hypomobility, and feelings of intra–articular pressure.
Traction mobilizations are performed passively by a trained professional and may sometimes require
the use of a strap. Although it is unclear exactly when traction mobilizations became prominent,
there has been an increased number of recent studies explaining the benefits and we will take a look

people the opportunity to remain active well into their golden years. (American) The American
Academy of Orthopaedic Surgeons estimates there are more than 300,000 total hip replacements
(THRs) and 600,000 total knee replacements (TKRs) performed every year–numbers that will only

One of the most significant factors in the patient's recovery is family involvement and support.
Family acceptance and support can help a patient deal with issues the patient may have during the
rehab process. Positive attitudes and reinforcement from loved ones often help the patient work
towards recovery. Family contribution, flexibility, and open communication can overcome various
barriers. Families who inspire hope can help the patient to adjust and become more confident in his
or her own abilities. Finally, family and friends can also offer practical support, such as rides to the
doctor or pharmacy, going to the grocery store, and offering to babysit during health care visits. The
encouragement of family and friends can motivate people who have been having difficulties in their
rehabilitation to take steps to be more active and get back to work more quickly than those who do

Jane Brody [2017] argues that there are million of Americans outliving their joints which are why
millions of people have surgery to replace worn out joints but the numbers are going to grow in
coming decades as people get older. Joint replacements are good when a person has lost mobility
because humans need to move in order to do exercise and be healthy. Surgery can get rid of the joint
pain but most patients in a study did not get a joint replacement. However, the numbers of patients
of getting joint replacements are increasing rapidly. In a study, it showed people who had surgery
recover equally well and typically experience less complication only if they go home directly from

Hip Replacement Recovery Guide

  • 1. Total Hip Arthroplasty Despite contrary belief, hip replacement is not necessarily negative. Many Americans believe that in order to have a hip replacement, your condition must be terrible. A survey conducted in 2012 said that almost 1/3 of respondents to the hip replacement survey felt they would have benefited from having their surgery done earlier. This procedure has and continues to change the lives' of many people in our world. Hip replacement, or arthroplasty, is a surgical procedure in which the diseased parts of the hip joint are removed and replaced with new, artificial parts. These artificial parts are called the prosthesis. The goals of hip replacement surgery include increasing mobility, improving the function of the hip joint, and relieving pain. According to the Centers for Disease Control and Prevention, 332,000 total hip replacements are performed in the United States each year. "Hip Replacement." Questions and Answers about. N.p., n.d. Web. 21 Oct. 2013. What were once considered high–tech, joint replacements are now a common operation. Surgeons replace more than a million hips and knees each year in the U.S. Studies show joint replacements can significantly relieve pain and increase mobility in about 90% of people who get them. Feature, R. Morgan, Griffin WebMD. "Should You Have Knee or Hip Replacement Surgery?" WebMD. WebMD, 30 Dec. 0089. Web. 21 Oct. 2013. • 3. Physical Therapy And Specific Surgical Approach Essay The objective of this research is to determine whether physical therapy and specific surgical approach will affect the recovery period among the patients who underwent the total hip replacement. The results which describe hip mobility and function after the surgery, as well as overall patients' satisfaction, are compared between various randomized and non–randomized studies, usually with the presence of experimental and control groups. Major common outcomes reported were pain, muscle strength, mobility, range of motion, and gait limitations. Based on these criteria it was determined that post–operative physical therapy exercise program significantly increases the time of recovery and improves the results of the surgery compared with patients receiving usual care. At present time, no study showed enough evidence to prove absolute advantage of anterior approach over the posterior as improvements were observed in both cases. This research contains important information that can be useful for patients awaiting total hip replacement. Introduction Total hip replacement (THR), also known as total hip arthroplasty (THA), is commonly performed surgical procedure for treatment of the late stage of osteoarthritis of the hip joint throughout the world. Arthritis causes damage to the articular cartilage of the femur and leads to reduced joint mobility and pain. Total hip arthroplasty involves replacement of worn cartilage with an artificial ball–and–socket joint. The surgery provides ... Get more on HelpWriting.net ...

  • 5. The Effect Of Glass On Everyday Life 3. Discussion 3.1. Effect of Glass on Everyday Life Mirrors and their applications were pivotal in the development of the sciences of optics, chemistry and astronomy (Rasmussen, 2012). It has even been said that the Renaissance and the Scientific Revolution might not have happened if not for the invention of mirrors (MacFarlane, 2002). Eyeglasses have been said to be one of the greatest inventions ever because without them, people born with poor vision not only find performing daily tasks much more difficult, but would also be severely constrained in terms of the amount of education they can receive and thus in their life opportunities. This is further underlined when one takes into account the fact that approximately 6 out of 10 people in the world today either wear glasses, contact lenses, or have previously undergone corrective eye surgery. Even for those born with good vision, they typically lose their ability to focus by their mid– 40s, which means that eyeglasses have considerably lengthened the intellectual lifespan of a person. The impact of eyeglasses on society thus cannot be understated. The impacts of the microscope and telescope are massive, especially to the fields of medicine and science. The modern discoveries ... • 9. Personal And Professional Nursing Practice Clinical Question Paper The nursing profession is one that is ever changing. As a profession that is created and based upon scientific research, the way in which nursing care is delivered is continually being modified and altered to reflect the latest and most up–to–date evidence based research findings. However, not all research findings are going to be applicable or appropriate for integration into nursing care. Consequently, the ability for nurses to be able to interpret and understand these research findings is also important, in order to make the most informed, educated, and knowledgeable nursing decisions before integrating such research findings into clinical practice. Therefore, the purpose of this paper is to reflect on how nursing knowledge is dispersed and disseminated in both personal and professional nursing practice. Clinical Question When in comes to enacting a possible change in nursing practice, a clinical problem or issue needs to be identified. Once a problem has been acknowledged, the process towards change is one that involves a multifaceted approach. The development of a clinical question is the first step in the process. In the nursing profession, this question is typically constructed using the PICOT format. Developed by Fineout–Overholt and Johnston, this five–letter acronym stands for: P = patient or population, I = intervention or issue of interest, C = comparison intervention or current practice, O = outcome desired, and T = time need to achieve ... • 11. The New Revolution Of Food Is Soylent bringing an end to our everyday food? Even though this answer is not clear, there are many positive and negative speculations about new discoveries in nutrition such as Soylent. Many people have found ways to improve the quality of our diet and have completely innovated the world of food. This new revolution of food seeks to enhance our quality of life and to make food as beneficial as possible. However, this dream can also be perceived as a nightmare, for this revolution may completely take over our diet and change the way we use food. This change causes much discomfort among people who have grown accustomed to the food they see everyday, and they do not want to fix something that has not been broken. Products such as Soylent represent a future of food fueled by hopes and dreams, but if we are not careful, this dream can quickly turn into a nightmare. The story Soylent represents an example of how an invention so simple can make an impact so immense. To clarify, Rhinehart broke down the complicated world of food to the basics which he turned into a cheap yet efficient product. Widdicombe states, "food costs had dropped from four hundred and seventy dollars a month to fifty." Hence, this steep drop in monthly expenses spent on food has enticed many people to believe in the Soylent dream. Not only does this method of creating cheap food help address our problem of worldwide hunger, it also helps those in tough financial situations like college students. In ... • 15. Essay On Total Joint Replacement Background of Total Joint Replacements and Wear Debris: Total joint replacements are very common in the United States, and are becoming even more common with increasing obesity rates and an increase in life expectancy. Long–lasting and high–performance implants are needed for the younger, active population that receives them. The Agency for Healthcare Research and Quality states more than 285,000 total hip arthroplasties (THA) and 600,000 total knee arthroplasties (TKA) are performed in the United States alone each year (2). Longevity of these joint replacements is dependent on several factors: the durability of the fixation of the implant surface to the bone, the bearing surface's wear rate, and the accuracy of the surgeon in implantation of the total joint (2). Of these total joint replacements, periprosthetic osteolysis and aseptic loosening occurs in about twenty percent of cases, and aseptic loosening is responsible for about seventy–five percent of total joint replacement failures (2). • 17. Replacement House Research Paper Suggestions For Choosing Replacement Windows For Your Cabin In The Woods If you're planning to renovate a cabin in the woods so you'll have a place for weekend getaways, you'll probably want to replace the windows with more energy efficient versions. You'll also want the windows to be durable and require little maintenance in the years to come. Here are some some suggestions for choosing the ideal replacement windows for your cabin. Consider Aluminum Clad Wood Frames Aluminum clad wood frames are a good match for cabins because they give your place the traditional appearance of wood on the inside, but the outside has a protective layer of aluminum. These windows are made with wood on the side that faces indoors and metal on the exterior side. The metal is available in different colors, which is a plus if you want to match the color to your metal roof or add a touch of color to accent the cabin's wood exterior. The biggest advantage to having these windows is that they require much less maintenance than full wood frames. The exterior metal surface won't rot or attract ... Show more content on Helpwriting.net ... However, you want the windows to be as secure as possible when you are away. While it is difficult to make your windows 100% burglar proof, there are some things you can do to make the glass difficult to break. You could choose glass that is impact resistant or you could have security film applied to the glass. Another option is to buy windows made of acrylic that is transparent like glass but much stronger. You don't only have to worry about intruders breaking into the windows. If the cabin is surrounded by trees, a storm could knock a branch into the glass and break it if you don't have security glass. Animals may even bang against the glass trying to get inside the cabin. For these reasons, you want glass that resists breaking, or glass that can't be knocked out of the frame even if it ... Get more on HelpWriting.net ...

  • 19. Obamacare Persuasive Essay Several of us are watching the news right now waiting to see if Senate Majority Leader Mitch McConnell and Congress get the 50 out of 52 votes to repeal legislation and to begin the Obamacare debate. After the votes are counted the debate will start for Obamacare to be repealed or to not be repealed. Obamacare, also known as Patient Protection and Affordable Care Act, was signed in March 2010 by Obama to "Include multiple provisions that would take effect over a matter of years, including the expansion of Medicaid eligibility, the establishment of health insurance exchanges and prohibiting health insurers from denying coverage due to pre–existing conditions." ("Affordable"). Therefore, this debate will show what Obamacare does for the people of the U.S, what will replace Obamacare if it is repealed, and if it is worth all the hassle of repealing and replaces it with Trump Care or to not replace it. Obamacare should not be repealed because several Americans are relying on Obamacare's cost assistance to afford health care. Many citizens often ask each other, "What is Obamacare?" To be quite honest, not many people could explain to each other what Obamacare is. The main goal of Obamacare is to give access to more affordable health insurance and to reduce the growth of overall spending on health care in the U.S (DeMichele, "What is Obamacare?"). Obamacare may seem like every other bill that can be repealed or replaced but it is not. Citizens lean on Obamacare to give them the ... Get more on HelpWriting.net ...

  • 21. Nursing Assessment And Comprehensive Post Operative... Hip fractures are one of the most common causes of extended hospital stay among the elderly, and approximately one third of patients undergoing hip arthroplasty surgery will die within the first year (Gregersen et al., 2012). The specialised nature of the orthopaedic ward is a complex environment for not only nurses, but for all health care professionals. It is the role of the registered nurse within the orthopaedic ward to provide holistic assessment and comprehensive post–operative planning to facilitate client centred care for an elderly patient undergoing a total hip arthroplasty (Walker, 2012). The purpose of this essay is to identify and prioritise appropriate nursing assessments and care provided for an elderly man (Mr. Simons) transferred to the Orthopaedic ward from the Emergency Department via operating theatres. The discussion will initially consider the environment of the orthopaedic ward and the role that the nurse must assume to provide high quality care. Focusing briefly on the ageing population and the impact that hospitalisation has on the elderly, the essay will then rationalise and prioritise current nursing assessments considering the primary and secondary assessment strategy and consideration of major body systems. Moreover, the discussion will detail and rationalise the appropriate management of patient safety, comfort and communication with substantiated planning of care by the registered nurse in the first eight hours of stay. • 23. Osteoarthritis Essay Osteoarthritis, the most common type of arthritis, is most prevalent in older patients. Osteoarthritis is a degenerative bone disease due to the gradual loss of cartilage. A primary type of osteoarthritis is hip arthritis where it is caused by joint injury, increasing age, and being overweight ("Hip Osteoarthritis," n.d.). However, osteoarthritis can also be caused by immature joints, inherited defects in cartilage, and extra stress on a patient's joints (Hip Osteoarthritis," n.d.). As a result, hip arthritis becomes a huge detriment in patient's social, emotional, physical lifestyles. In order to treat hip arthritis, doctors choose from a variety of non–drug treatments, medications, and surgeries. Uniquely, I was inspired to research about the treatments of hip arthritis because my very own brother was pronounced with hip arthritis a few years back. • 25. Case Study On Knee Replacement 1. My mother has knee pain for many years and recently it has worsened. She has some deformity around the knees and can barely walk more than 10–15 minutes. What should be done? Recent worsening of chronic knee pain and deformity probably indicates that knee arthritis has reached advanced stage i.e. the protective lining of the joint is nearly completely worn out. At this stage, pain medication often doesn't help much. Rather, one may suffer from dangerous side effects of prolonged pain medication therapy on heart, liver, kidney, stomach and intestines. It is recommended that one should consult a joint specialist and get certain X–rays. If it is advanced arthritis, then one should consider knee replacement to get rid of the problem. 2. Joint Replacement Research Paper Joint replacement is entering an era of greater minimally invasive alternatives, which provide patients with options to minimize recovery time and financial expense. The three major costs of joint replacement surgery include the price of the implant, extraneous hospital fees, and postoperative rehabilitation expenses [1]. Although minimizing length of stay would reduce these costs, several factors traditionally necessitate overnight hospitalization for arthroplasty patients, including pain control, blood loss, and monitoring of comorbidities. However, with recent advancements in analgesia technology, implant design, surgical technique, and postoperative management [2], many practices now offer outpatient surgery as an alternative to decrease ... Slipped Capital Femoral Epiphysis Essay Slipped capital femoral epiphysis (SCFE) and Legg–Calve–Perthes (LCP) disease are developmental conditions (1, 2) with SCFE being described as a common source of pain and disability, and the most common cause of degenerative hip disease in middle–aged patients (3, 4). Proximal femoral osteotomies following SCFE aim to restore more normal anatomy, improve function and reduce the morbidity and complications of pain, reduced range of movement, early arthritis and leg length discrepancy (3). Up to 50% of involved hips do well with no intervention but these hips are always at risk of developing secondary arthritis and similarly adult hip joints of patients with SCFE are more prone to develop advanced osteoarthritis (5). In this study we independently reviewed ten year outcomes in a multicentre cohort of 70 patients (73 hips) following hip resurfacing for end stage arthritis following SCFE and LCP. Patients and methods In 1997 the Oswestry Outcome centre was setup to serve as an independent international register for the collection, analysis and reporting of outcomes following hip resurfacing arthroplasty. Between 1997 and 2002, 4535 patients were recruited by 139 surgeons from different centres in 38 countries. We selected 70 patients (73 hips) which included 58 patients (59 hips) with SCFE and 12 patients (14 hips) with LCP. These patients were age and gender matched to a group of 73 patients with primary osteoarthritis whose outcomes were unknown to us. The selected patients in both groups were operated by 39 surgeons from 4 countries. The primary osteoarthritis group was labeled as group OA and secondary arthritis group was labeled as group LS. All patients were entered in this study after informed consent by the operating surgeon who provided the demographic and clinical details of each patient. These details were stored on a secure electronic database in the outcome centre and patients were followed annually in context of function, complications, revision and ... • 31. A Brief Note On Titanium Alloy Implants And Joint Replacement Erik Rasmussen 11–13–2016 ME 315 Titanium Alloy Implants and Joint Replacements The advancement in materials science has paralleled with human ability to make improvements in the fields of engineering, manufacturing and medical fields. Specifically in the medical field, as major joints or sometimes organs begin to fail, foreign materials must be introduced into the body to perform the functions of the failing component. Not all materials can be used in the human body however, therefore materials science is directly correlated to the success of implants. This report discusses some of the challenges faced when selecting the most ideal material for an operation, and investigates many of the material properties of common joint replacement/implant materials. Understanding further the certain requirements of proper implant materials used in practice will permit greater advancement in the field of medical implants and joint replacement once other materials are created to better fit those requirements. There are three main areas which a certain material must prove to be superior in a given application in order to be considered for use in implants and joint replacements. First and most importantly, the material in question must be compatible with the human body. Introduction of foreign materials into the human body causes a reaction between the foreign material and human tissue, we consider the material compatible so long as this reaction is not producing negative or harmful ... Observation of a Failed Hip Replacement Case involves a 60yr old man who underwent an uncemented total hip arthroplasty or a total hip replacement, who was referred to a home PT upon d/c instructing the patient to start home PT & begin ambulation using the 'toe touch weight bearing' which was incorrectly mentioned in the d/c summary as 'weight bearing as tolerated'. For the initial 6 weeks, a patient with uncemented prosthesis should begin with 'toe touch weight bearing' and not 'weight bearing as tolerated' unlike the cemented prosthesis1. Toe touch weight bearing is instructed to the patient to protect the joint & also give bone the time to grow into the porous coating of the implant2. During the patient's visit, the PT developed a plan of care from his evaluation & examination and handed–over the case to the PTA to carry out the therapy interventions. PTA (Physical Therapy Assistant) is defined as an educated individual who works under the direction & supervision of a physical therapist3. From the mentioned case, it is clear that the PT has not read the d/c summary and also that he has not mentioned the type of prosthesis used whether a cemented or an uncemented which highlights the PT's failure of proper documentation. PTA has elected to follow a plan of care that she felt was consistent with that of a cemented prosthesis, highlighting the PTA's mistake because the PTA is not allowed to make any modifications and follow the plan of care as directed by the supervising PT. • 37. Reflection Paper On Writing And Writing Writing has consistently been a shortcoming throughout my life. The content of my paper was typically not the issue, the organization of the ideas was. I always had good ideas to write about, but I never knew how to organize those thoughts into a well–organized paper. Although in Writing 101, I used the multiple steps of the writing process to insert my ideas into a well–written paper, I continued to struggle in areas such as grammar and mechanics. In Writ 102, I was able to realize that the writing process develops over time, and the best papers result from revision over time. The multi–step writing process that was required for each paper this semester allowed me to develop fundamental revision skills over time that made my writing reach its full potential. When I was first assigned the analysis essay, I was relieved because I had completed a similar paper in Writing 101 the previous semester. I assumed I had this paper in the bag because it was a familiar topic. When composing the first rough draft, I was focused on reaching the required page count, not the mechanics or content of my paper. I realized, when I reexamined the first draft, that I lacked the primary content of the paper. I did not include an analysis of my product's advertisement. Since I did not analyze my product's advertisement at all in my first draft, in my second draft I added two adequate paragraphs that analyzed the ad. Today advocacy extends beyond the individual patient to local and global populations, and nationally and globally to the nursing profession itself. Show more content on Helpwriting.net ... She was scheduled for total hip arthroplasty three days following admission. The daughter wanted to proceed with the surgery, and the son did not. The ethical concern in this case hung on two issues. First, the patient demonstrated a sufficient degree of dementia to ask "will the patient be able to adhere to instructions and hip precautions following surgery." Additionally, given her degree of debility due to her non–ambulatory status before the injury "would it be in her best interests to proceed with the surgery?" As a team, the nurse, social worker, and I discussed these concerns with the orthopedic PA who agreed that perhaps the fracture should just be allowed to heal on its own instead of going forward with a costly, painful surgery and extended rehabilitation that may or may not produce the desired outcome. The PA, nurse, social worker, and I met with the orthopedic trauma surgeon and the head of orthopedics and presented the problem. Using a duty–based, deontological framework for ethical problem solving, and the principle of beneficence (Marquis and Huston, 2012), it was agreed that in this particular instance it would be best to let the fracture heal. • 41. Total Hip Arthroplasty Essay During the weeks of February 7 through February 17, I observed a total hip arthroplasty on a 56– year–old Caucasian female patient who suffered from a femoral neck fracture and damage to the acetabulum. The fracture was a result from a car accident where the patient's knees collided with the dashboard, forcing the femur into the hip and breaking the femur. For this report I will concentrate on total hip replacement, its components, main surgical technique, and complications. Sir John Charnley first developed total joint arthroplasty in the 1960s (Skinner 395). In a total hip replacement "the articular surfaces of the acetabulum and femoral head are replaced" (Lemone 1241). A prosthesis is then used to replace the entire head of the femur ... Show more content on Helpwriting.net ... The acetabular components have a "spherical outer surface with at least one hole to permit the surgeon to determine if the prosthesis is fully impacted into place" (Skinner 399). The inner surface of the acetabular component then locks the UHMWPE to limit rotation and dissociation (Skinner 399). There are two crucial factors when choosing the implants. First, if the prosthesis is too stiff then the patient may suffer from proximal osteopenia. Second, the stiffer the prosthesis is the more likely the patient will experience pain in the thigh. To avoid these two factors the surgeon should elect to use titanium ally instead of the cobalt chromium alloy. There are several surgical techniques used for total hip replacement, including the posterolateral approach, the lateral approach, the anterolateral approach, and others. For this paper I will concentrate on the posterolateral approach since it is the most common (Skinner 396). The patient is put into the lateral decubitus position with the affected side superior after anesthesia is administered and a compression stocking is placed on the unaffected limb. The skin is covered with an adhesive drape after the incision is outlined (Skinner 396). Then the hip is flexed to 45 degrees so the "incision can be made in line with the femur from approximately 10 cm proximal to the tip of the trochanter to 10 cm distal to the tip of the trochanter" (Skinner 396). "It seems so barren now." she said shifting her eyes back onto the road, her hair fluffing up as she shakes her head. "I can't believe they're chopping down so many trees." I heard sadness bubbling up in her voice as the traffic light turned green and my mom drove the car forward. I look back at the empty plot of land, a place that used to be packed with trees, trees that had been there for hundreds of years. I watch the desolate yard of stumps fade off behind us as we continue on home. The thoughts still roam around in my head. How many trees does it take before they stop? One more? Ten more? A hundred more? Or will the only reason to stop be the extinction of them as a whole? My brain pans back a couple years. I think back to the fluffy memories, back when my mom was a babysitter. When our house used to be jam packed full of kids, some laughing, some crying. It would get so crowded that we would have to migrate out of the house like air flying out of a balloon. Sometimes we would go to the park or walk around the block, but most of the time we would go play in our backyard. Our backyard has never been the most very interesting on its own, but it does have one noteworthy feature. Behind the chain link fence in our backyard, the world opens out into a lush green space. • 45. Total Hip Arthroplasty ( Tha ) Introduction: Total hip arthroplasty (THA), commonly known as hip replacement, is a reconstructive orthopedic procedure that involves the surgical excision of the head and proximal neck of the femur and removal of the acetabular cartilage and subchondral bone(A). The damaged joint is replaced with an implant that mimics the motion of the natural joint and is made from combinations of metal, plastic and/or ceramic components(D). Joint replacement is one of five priority areas targeted for shorter wait times in Canada. Ontario's current wait time is 207 day.(F) The number of THAs in Canada increased 11% between 2006–2007 and 2010–2011. In 2013–2014, there were 49,503 hospitalizations for hip replacements(B). THA has been used to manage conditions in which the hip joint has responded poorly to less–invasive treatments(D). The most common of which is severe osteoarthritis of the hip joint, which affects more than 10% of Canadians aged 15 or older(G), and accounts for 70% of cases. The procedure has proved to be remarkably successful in eliminating pain and restoring function in hips severely involved with diseases, improving mobility, daily functioning and quality of life(A,D,G). THA provides pain relief and improves physical function and quality of life in patients with end–stage hip osteoarthritis. The incidence of THA is expected to increase due to the growing elderly population. Nevertheless, THA is becoming more common in younger populations because of the improvements in ... Evidence-Based Practice PSYC40014 Advanced Research Methods in Psychology assignment 1 Qian Zha Student ID: 752852 University of Melbourne Word count: 1,546 What is Evidence–Based Practice and why we should promote it Evidence–Based Practice (EBP) was initially a movement in medicine, dating back to early 20th century (Spring, 2007). It was described as a careful decision making process about the care of individual patients using best available evidence by Sackett, Rosenberg, Gary, Haynes, and Richardson (1996). Greenhalgh (2010) gave a similar definition but with more emphasis on the mathematical assessment of the potential benefit and harm. Klein (2014), a research psychologist, argued in his article on Edge that EBP should be abandoned. This article will address each of the four points he made in the article. The first point Klein (2014) made was that research evidence is not always a completely reliable, and therefore shouldn't be trusted. This is an invalid argument for several reasons. It is true that research has limitations due to a number of reasons: the nature of the research questions, funding restriction, random sampling availabilities, ethical restrictions, and limitations of statistical techniques. Science is not perfect, some studies are less reliable than others, and we will never be certain if the conclusions are true or not, but what is the better alternative? Without EBP, the clinical decisions can only be based on some novel ideas or / and clinical observations, but they are often wrong. Prefrontal lobotomy, a surgical procedural that cuts off neural connections in the prefrontal cortex, was based on a novel idea; it was thought to be effective in managing undesirable symptoms of patients with mental disorders, and it seemed to work as some patients did have reduced symptoms. However, it improved some of the patients' symptoms but left profound long–term damage to most of the patients. Those patients had personality change, inability to function in the society, apathy, lost of initiation and inhibition (Freberg, 2010; Szasz, 2007). No research evidence will ever be completely ... A Center of Excellence (COE) Only Benefit: Under this benefit design, patients are signified to a COE for evaluation and determined to see whether they are applicable aspirants for a knee or hip replacement procedure. If the candidate qualifies for the procedure, the benefit is enclosed and the provider is paid a bundled payment. If an individual does not qualify for the procedure, there is no insurance benefit for the procedure at any facility. 2. Show more content on Helpwriting.net ... These approaches give the patient a broad choice of providers and some financial incentive – in the form of lower cost–sharing – to seek care from a provider participating in the bundled payment program. However, to create enough "steerage" to these providers, the incentives will have to be significant; providers may think twice about participating in bundled payment if patients still have a broad choice of providers. Purchasers are likely to be more successful at getting providers to accept bundled payment if providers can expect greater patient volume when they do. To achieve maximum savings and better patient outcomes by driving volume to high–value providers, CPR recommends benefit design approaches that create incentives for enrollees to seek care from them. Options 1 and 2 above are examples of benefit designs that are more likely to yield higher results than those strategies that offer modest benefit design differentials between a bundled payment provider and a non–bundled payment provider. In the end, determining your benefit design strategy comes first; designing and developing the network will follow. Employee education and benefit designs that either encourage or require the use of treatment decision support (TDS) tools can help consumers feel better about their treatment decisions and prepare them for more aggressive benefit strategies that steer patients ... Perioperative Nursing Written Paper Robyn Weiner New York University College of Nursing Spring 2015 A total knee replacement (TKA) is the most common joint surgery performed in the United States (Turner, 2011, pp. 27–32). Each year, over 650,000 Americans undergo this surgery (Wittig–Wells, 2015, pp. 45–49). It is an invasive surgery that involves an incision on top of the knee and replacing damaged parts of the knee with artificial parts that are either metal, ceramic or plastic. Someone would get a total knee replacement for damage of the joint, osteoarthritic, posttraumatic, or inflammatory arthritis. The cartilage is damaged, wears away and then you develop bony deformity and contracture of ligaments but it starts out with specific defects or wear of cartilage. The top nursing priorities for a total knee arthroplasty is to "prevent complications, promote optimal mobility, alleviate pain, and provide information about diagnosis, prognosis, and treatment needs" (Doenges, 2014, pg. 627). A possible nursing diagnosis from the patient who is undergoing a TKA might be 'impaired physical mobility related to pain and discomfort as evidenced by reluctance to attempt movement.' Another one could be 'acute pain related to chronic joint disease as evidenced by reports of pain' (Vera, 2014). The preoperative phase begins when the patient is scheduled for surgery. Robots and other automated machines do many human jobs. One study concluded that about 50 percent of US workers could be replaced by robots in the next 10 to 20 years (Jones, 2016, para. 3). Even though robots will replace some jobs done by humans, there will be new and higher skilled jobs for workers. Some people worry that robots will have a negative impact on humans in the future. However, people have had this concern for a long time (Shah, 2015, para.15). During the Industrial Revolution, workers demolished machines in the factories because they worried that their jobs would be replaced by machines. In the manufacturing industry, machines decreased or eliminated the need for workers. Governments tried different ways to slow or stop the development of new technology because of their concern over job loss. In 1638, England's government legally prohibited certain machines. And during the Depression, the US government imposed a tax on companies that replaced workers with machines. But new technologies do not necessarily mean that workers who are replaced by machines will not have jobs (Volti, 2012, 80–81). History has shown that even when jobs are lost to machines, new higher skill jobs are created. For example, even though manufacturing jobs were lost in the new economy, new jobs were created in the service industry (Volti, 2012, p. 56). Robots increase the efficiency of ... Mobilization: the process of passively restoring capability of movement. Combining the two, Traction mobilizations are an incredibly beneficial tool in helping individuals move correctly and without pain by restoring physiological function and accessory motion of the specific joint. That nagging hip pain that you've had from time to time would benefit from traction . All those tight muscles could use some relaxation, they could use some traction. Some indications for traction mobilizations include pain, hypomobility, and feelings of intra–articular pressure. Traction mobilizations are performed passively by a trained professional and may sometimes require the use of a strap. Although it is unclear exactly when traction mobilizations became prominent, there has been an increased number of recent studies explaining the benefits and we will take a look at some of these articles that specifically focus on the iliofemoral joint and the effects on pain, range of motion, and muscle strength. One of the most significant factors in the patient's recovery is family involvement and support. Family acceptance and support can help a patient deal with issues the patient may have during the rehab process. Positive attitudes and reinforcement from loved ones often help the patient work towards recovery. Family contribution, flexibility, and open communication can overcome various barriers. Families who inspire hope can help the patient to adjust and become more confident in his or her own abilities. Finally, family and friends can also offer practical support, such as rides to the doctor or pharmacy, going to the grocery store, and offering to babysit during health care visits. The encouragement of family and friends can motivate people who have been having difficulties in their rehabilitation to take steps to be more active and get back to work more quickly than those who do not have a strong support ... • 73. Pt2520 Week 1 Critical Thinking Assignment System Level Scripting #2 This week's critical thinking assignment explores the system–level scripting functionality of the Linux operating system. Documenting each step in the assignment will demonstrate how to use the awk and sed commands to display different sets of data from and file based on the commands used. The sed command will also be used to change the data within the test file. These steps will be discussed and illustrated with screenshots in the following paper. awk and sed After opening the shell prompt and logging in as the root user which is demonstrated in the first two screenshots below, the next involved checking the current directory location in the shell prompt. As seen from the second screenshot, the current directory is the /home/user directory which is the home directory for the user created in the test installation of Linux I am using. Step three in the assignment involves using the man command to view the documentation for the awk and sed commands. The awk command had quite extensive documentation of over 800 lines while the sed command had almost 300. These commands can perform a wide variety of tasks which make them very useful tools when writing scripts. The next item in the assignment gave instructions on how to ... After spending quite a bit of time scouring the internet and searching for a resolution, I determined that the format or font used in the document was not recognized by my installation of Linux. I opened the file and copied the information only to a new file and saved it in a .csv format. I then renamed it to match the original file name and I could then use the cat, awk, and sed command to return legible data. The screenshots show the garbled output from the downloaded file vs the output of the file after this ... Get more on HelpWriting.net ...

  • 75. Joint Replacement Research Paper Jane Brody [2017] argues that there are million of Americans outliving their joints which are why millions of people have surgery to replace worn out joints but the numbers are going to grow in coming decades as people get older. Joint replacements are good when a person has lost mobility because humans need to move in order to do exercise and be healthy. Surgery can get rid of the joint pain but most patients in a study did not get a joint replacement. However, the numbers of patients of getting joint replacements are increasing rapidly. In a study, it showed people who had surgery recover equally well and typically experience less complication only if they go home directly from the hospital.
