Therapist Case Report
Case report is a detailed narrative description of a real life problem or situation as well as a way to
apply theoretical knowledge gained from the academic literature to real life situations that may serve
as basis for future research. Case report can be done on any subject or topic. It is designed to
analyze and understand the factors important to the etiology, care and outcome of the subject's
problems, through a detailed description of a patient's background, present status and response to
treatment. In my case report from JSPTR "Undetected Pectoralis Major Tendon Rupture in a Patient

The Effects Of News Media On Today 's Society
The news media's role in today's society is interlocked and essential to the conveying of
information. The news media has access to television broadcasting and has people to go find stories
to broadcast. However, as time has progressed, the news media has become more and more
opinionated. Consequently, the bias of the news media in their reporting's has tainted the opinions of
the American people. By creating a strict set of laws pertaining to the reporting of news, bias within
reports will have to decrease; causing a less opinionated audience, people feeling better about
themselves and humanity, and the United States growth as a stronger country.
The news media in today's world is far too opinionated. On a regular basis, the news media and
other news outlets are often discussing opinions, their own opinions, and other's opinions about
stories and news reports rather than the reports themselves. During a discussion about Donald
Trump's immigration policy on August 25th, 2016, on the CNN newsroom, one of the reporters said
that "we heard this shift on his immigration policy." However, one of the other reporters defended
Mr. Trump by saying. "First, let me correct the record" and goes on to do so and they argue back and
forth (Person Discusses). In this report, what Donald Trump said nor any part of his speech was
shared. The report was wholly opinion based. Although, the news media will share clips and snips of
stories in many of their stories.
However, many news

Integrated Reporting And An Understanding Of A Company 's...
Integrated Reporting is a more holistic approach to gaining an understanding of a company's
performance. It takes into consideration factors including the environment, society and governance
when evaluating a company's performance and strategizing on a future course of action. One of the
most recent and promising developments in Integrated Reporting has been the release of the
Integrated International Reporting Framework by the International Integrated Reporting Council.
This paper assesses the opportunities, benefits, challenges and limitations of this framework.
Introduction
The Integrated International Reporting Framework was introduced by the International Integrated
Reporting Council (IIRC) to guide the entire integrated reporting process and also to clearly define
key ideas and concepts of this framework. Given some of the confusion surrounding this framework
among companies and accountants in New Zealand with respect to what precisely the Integrated
<IR> Framework is and how it might affect accounting practice, this essay purports to clearly
explain the framework. Further, this paper will also critically examine the framework, highlighting
some of the salient opportunities, benefits, challenges and limitations of this approach. Finally, it
will briefly reflect upon the implication of the framework for accounting practice.

Company Complexity Of Ge And Mcvay
Company Complexity
According to Ge and McVay research study findings, business complexity is positively correlated
with firm disclosure of material weaknesses. They hypothesized that the "firms with more
complicated transactions likely to have greater chance for a disconnect in the financial reporting
process, and thus a material weakness (Ge et al., 148)." To measure business complexity, Ge and
McVay used the number of operating segments reported in the annual 10–K and the existence of
foreign currency exchange translation. Ge and McVay had two subgroups within their study: firms
that disclosed material weaknesses and a control group of 2003 Compustat firms who did not report
any material weaknesses.

In comparison to Doyle et al. study, Doyle and his colleagues hypothesized that the "complexity of
the firm is a driver of internal control weaknesses (Doyle et al., 9)." To measure the business
complexity of a firm, Doyle et al. used three measures to include the number of special purpose
entities, number of operating segments and the existence of foreign currency adjustment. The
findings of Doyle et al. were as predicted. The three complexity measures were higher in firms that
reported material weaknesses in comparison to the 2003 Compustat firms, who did not report any
material weaknesses. This indicates that company complexity is positively correlated to the
disclosure of material misstatements.
Company Profitability
In relation to the Ge and McVay research study findings, company profitability is negatively
correlated with firm disclosure of material weaknesses. Ge and McVay hypothesized that "poorly
performing firms simply might not be able to adequately invest in proper internal controls (Ge et al.,
151)." This would result in a greater chance of disclosing material weaknesses. In order to test their
hypothesis, they used two profitability metrics consisting of return on assets (ROA) and cash from
operations scaled by assets (CFO/A).
In reference to the Ge and McVay research findings, as presented in Figure B1 , material weakness
firms had lower ROA and CFO/A compared to the control group of 2003 Compustat firms

The Role Of Media Management On Crisis Communication
The importance of media management in crisis communication
Introduction
Crisis according to Laura (2013, p. 51), can be defined as any situation that threatens harms or cause
panic among people. Crisis may occur due to accidents, war, and flood or fire attack. During this
period, the public, who may be affected by the crisis because they are found in the situation or their
next of kin are involved in it, rely on a lot of information that is relayed by the media. Because of
this, the media must be careful of what they report. The information that the media convey to the
public according to Laura (2013, p. 50) may cool the situation, and reduce the tension that has been
built by the parties concerned. However, if they are not mindful of ... Show more content on

also researched on how the media should play this role. For instance, Taffel, A., in the year 2007 did
research on how to enhance Public Relations During the crisis (Taffel 2011, p. 112). In his research,
Taffel observed that, it is necessary to monitor the information that is relied on the public so that, the
image of the agency that is in crisis is not tarnished. In his research, Taffel had suggested that the
organization must balance the information that o is relayed the public (Taffel 2011, p. 112). He
observes that, it is necessary to balance between truths and lies so that the level of tension is
reduced. In the research, taffel adds that, if the media is allowed to report on everything, they may
kill the reputation that the company has.
In 2011, Yan, J., Brooke, F. L and Lucinda, L. A. did a research on the role of the social media in
effective crisis management. The research had been prompted by the manner in which a lot of
propaganda was spread via the social media during the Arab spring (Yan, Brookes and Lucinder
2011, p.3). In the paper, the researchers observe that, crisis can be best managed by the medal if they
can control the information that they give to the public. The researchers observe that, if the media
gives information that is contradicting, the public may become more anxious.

Ethical Considerations At Hand It May Preserve The Company...
The intent of this paper is to critically analyze ethical considerations to support whether or not to

The issue at hand is determining whether or not newly hired Ben should notify his supervisor and

This compares to critical errors in Ben's case.
Similarly to this definition, the definition of a "small error" begs a number of questions:
– Where do these errors come from?
– What is Ben's proficiency in Microsoft Excel?
– Can anyone outside the company audit the company's books?
Ben is conflicted as to how to handle errors found in his work because it may jeopardize either his
job, or the future of the entire company. This is his first job and he realizes that his responsibilities
are overwhelming at times. His reports are an important component of the company's business, and
part of the CFO's work relies on it.
On several instances he has found errors in his reports, and decided to only revise and report those
errors he believes to be critical. Small errors, on the other hand, are not as important to require
reporting according to him.
The case however suggests that Ben is also considering whether or not he should in fact overlook
the small errors, which is what I will attempt to answer in this critical thinking analysis.
Ben's position to disregard small errors is understandable in some aspects. In fact, as a newly hired,
in his first job, he must be proud of being given such job with great responsibilities. Submitting
frequent revisions due to small errors, will make him look incompetent, unreliable, and jeopardize
his job. This could mean running the risk of being fired or demoted. In his situation, the most
important is to prove himself and show that

The Utah And Minnesota Incident Reporting
Preventable health care errors contribute to at least 44,000 deaths per year, increasing the cost of
health care and limiting public trust. The Adverse Health Event Law passed in 2003 requires
disclosure and examination of specific unfavorable events with corrective action plans with some
aspects shared publically in order to educate consumers about health care facilities issues and
improvements (MDH 2015).
Review of the Utah and Minnesota Incident reporting mandates provided various state statutes and
reporting responsibilities to the state's government and regulatory agencies. Reports of adverse
events must be reported in various methods among states. For example, Utah and Minnesota require
each individual facility, hospital, outpatient centers, and clinics, to report the adverse events that
occur at the particular facility. Specifically, in Minnesota if the Boards that regulate physicians,
nurses, podiatrist, physician assistants, and/or pharmacists are aware of an adverse event, the
specific board holds the responsibility to report the events to the Minnesota Department of Health.
Adverse events consist of defined elements within states in the categories of surgical, product or
device, patient protection, case management, environmental, and potential criminal events,
essentially providing events that produce irreversible patient harm. However, differences among
these two states exist as Utah provides more specific descriptions in the case management events

the 'Supporting Files and Links' section within AMP.
Test Steps:
1. Inquire with the business regarding Financial Elder abuse and determine the following:
a. What criterion is used within the Bill Pay claims group to identify possible instances of Financial
Elder Abuse?
b. How does the banker report suspected Financial Elder Abuse?
c. Does management obtain any information/data from the Elder Financial Abuse (EFA) team
regarding reports submitted by the Online Claims team? This information would be used for training
and analysis purposes.
Audit emailed James L. Holloway (Operational Risk Consultant) with a list of the questions above.
Jim's response can be found in the 'Supporting Files and Links' section under . In his response he
confirmed that there are no procedures outside of what was provided and reviewed in step 2.
Additionally, Bonnie Brooke provided information in the email chain stating that the VOS group
does not receive any reporting regarding submitted elder abuse claims from the Elder Financial
Abuse (EFA) team, as it is not required. Further, in the email chain Bonnie mentions the rarity that a
financial elder abuse case is reported referring to one "case we reported 1.5–2 years ago" as the most
recent incident to date.

Peter has been working for with the Bigness Oil Company's local affiliate and established a trusting
relationship with Jesse, manager of the local facility. Jesse was pleased with Peter's work and

Some concepts involved through the issue are risk, confidentiality, environmental health, conflict of
interest, and loyalty. First, the company and peter's career will be in risk if he reported the oil spill to
the state. Second, the engineering law requires confidentiality therefore it is irrelevant that peter
report such an issue that is confidential in the company. Third, the tests were only taken 400 feet
from the surface of the groundwater, however maybe there is contamination deeper or other marine
life were affected. Four, most important concept is the conflict of interest since Peter had to either
obey the engineering law that requires confidentiality and neglect his morals or obey the state law
and lose his friendship and career. Five, Jesse trusted Peter to keep this mysterious spill between
them; he should be loyal to him since they have a trusting relationship. Additionally, Jesse noted that
he cannot have a consulting engineer who doesn't value client loyalty. On the other hand, there are
some unknown facts in the issue. According to state law all spills must be reported but what about
spills that occurred years ago? Where the effects of spill seem to have dissipated? Is there any
gasoline concentration beyond the 400 feet of gasoline? Is the marine life affected by the spill? Will
the gasoline contaminate the ground water years later? Who is responsible to

News Reporting Bias Analysis
People who watch the daily news usually don't watch a variety of other news stations therefore
whichever the station reports the news it usually tends to be biased. The truth can be obscure with all
the biased views in media that's why it is important to learn how to tell the difference between the
misleading information and truthful information. Due to this reason the viewpoints of a person can
be biased due to the influence of biased news. Viewpoints such as those can be avoided if the truth
can be detected easily, such as by simply watching several news stations with different stances in
politics, and etc. Learning and studying the viewpoints of the different media outlets can help you

scanning articles to find the truth in them. There is now outlets being created to help the viewers
find the truth in journalism reportings therefore helping improve journalism because it is checking
on journalist to make sure they don't report lies due to their biases. McManus in his article talks
about how now in modern days there is too much biased views in news and has lead to the creation
of small news outlets that are online who try to report news but without all the biased writings.
There are smaller news stations that focus on finding the truth and not being biased on their
reporting but more of showing both views to a situation. Shows like these are appearing online
nowadays such as one called The Young Turks. Although they seem biased they will help the viewer
on detecting the truth from the lies in news by taking different viewpoints basically going through
the trouble to find the truth in different reportings and just reporting the facts. One method people
can use to determine the truth is by using the SMELL test. What the SMELL test is basically

Week 3 – Case Study #1
Cecellia Dantzler
Synopsis: Frank, manager of corporate reporting at Amalgamated Forest Products, has threatened to
go public with information regarding a falsified report on the effect of effluent controls on the
discharge of wastewater from pulp and paper companies, which has angered his boss, Jim McIntosh
and the company's president, Jim Letourneau. Letourneau was to testify before a legislative
subcommittee the following week and use the report, "Endangered Species: The Pulp and Paper
Industry in the Upper Peninsula", to give the industry's perspective on proposed legislation. The
section of the report which contained the falsified financial information was prepared by Tina ...

The section of the report addressing the monetary impact of Amalgamated installing emission
control equipment had been falsified. The numbers used in the report were over exaggerated in order
to show that Amalgamated would suffer severely financially should this legislation be passed.
Recommendation #1: Amalgamated has three mills in the community. These three mills and the
corporate offices provide jobs to many of the 10,000 people of Northlake. The company is a major
contributor to the economy of the community. The recommendation is for Amalgamated to be
ethical and show corporate responsibility. No amount of money can be given for human life. The
aboriginal community is suffering from the effects of emission waste from the previous competitor
and from Amalgamated as well. Amalgamated should forego its own financial gain and do what is
ethically right to clean up their waste so that it does not affect the community. The company has had
the emissions problem for so long and it would only benefit them in the long run to do the right
thing and stop the pollution now.
Finding of Fact #2: Frank is being bullied, more specifically, being mobbed, by his vice president
and the president of the company. When Jim McIntosh confronts Frank about publically disclosing
the truth about the report, he is yelling and calling Frank stupid. He's threatening Frank with all the
things he could lose if he were to go forward with the truth. His face is bright red and he is

Scientific Reports And Discoveriesalways Attract The Media.
Scientific reports and discoveries always attract the media. Multiple cases have been turned into

With this case there are numerous reports written as well as scientific research. There is a very
distinct difference in news reports on the issue and actual scientists reporting their tests and
hypotheses. In order to understand the differences one must first consider the rhetorical model. The

The reason for doing this is the news reporter's exigence, or reason for writing the article is to just
provide the public with a story and explain the most important details to try to get the public
interested in their stories. The news reporter also has a deeper exigence beyond just writing a story,
the news reporter gets paid accordingly to how popular their reports are, therefore in reality, news
reporters write to get paid, rather than to make a difference in the scientific community. The article
goes on to say that the research "may help improve understanding" , in this news report stating that
it may help, just provides the reader with the thought that the writer doesn't clearly know the
findings just what they have read on the issue, whereas the scientific report intelligibly states the
facts because the news reporter is in a discourse community of just writing and reporting, no
scientifically based knowledge is needed in their field, so reporters don't need to know much about
what they are reporting on, as long as they know the most important details.
There are other substantial contrasts between the two as well. In a news report the writer or reporter
is only skilled in the expertise of media and whatever path of schooling and research they took, in
contrast the scientist has an educational background and knows more about studies and science in
general. Therefore the scientist knows more about how

Task 2 Failures to Protect Individuals Essay
TASK 2
Identify two reports on serious failures to protect individuals (ADULTS) from abuse. Write an
account that describes the unsafe practices in the reviews. Look on Google under UK failures in
adult care
Report 1.
In March 2002, a 30–year–old woman with learning disabilities was admitted to Borders General
Hospital in Scotland with multiple injuries as a result of sustained physical and sexual assaults. The
abuse had been carried out at home and was perpetrated by three men, one of whom was her carer.
'The case of the vulnerable adult' is the term that the individual involved has asked to be used. Her
identity is protected under rules giving anonymity to victims of rape.
The woman had made allegations against one of the ... Show more content on Helpwriting.net ...
If all seemed well they could have arranged visits for support for the girl for example.
Knowing or finding out weather a person is able to fully understand when they have a disability is a
difficult thing to do. When the girl's mother passed away the perpetrator was allowed to become the
girls carer and the abuse continued. Before this man was allowed to become the girls carer
agencies/social services should have fully investigated the man's past, he should have gone through
interviews and the agencies could have observed/investigated the man's and girl's
relation/friendship. There would have been no harm for the agencies to go and check on the woman
years later and keep on top of visits because if they did this the signs of abuse would have been clear
such as dehydration and lack of food, poor clothing, torn genital area and marks on her face and
body.
Report 2.
NHS staff persistently abused a blind and deaf man with a low IQ by tying him up for 16 hours a
day, government inspectors revealed today in a damning report on services for people with learning
disabilities.
Carers employed by the Cornwall Partnership NHS trust bound his arms together with cloth
bandages and fastened them to his bed or wheelchair, to stop him slapping himself in the face.

The Un Sanctions Committee 's Guidelines Prescribe Too...
Some would say that the UN Sanctions Committee's guidelines prescribe very loose standards of
evidence for their Monitoring Groups. This is often followed with the claim that inconsistent
standards of investigations and evidentiary standards can lead to poor harmonization and unfair
sanctioning, which can even go so far as discrediting of the Sanctions Committees of the Security
Council. While these claims may be true to some extent, the argument can be made that by
comparing reports, Experts have already formed a degree of consensus in their evidentiary standards
and due processes of investigations. The purpose of exemplifying the high burden of proof across
the Panels of Experts without a common guidelines is to demonstrate that due ... Show more content

Looking at this policy, it is clear that it would be impossible under the current framework to provide
this type of due process for all individuals and entities sanctioned by the UN. Furthermore, the
Security Council would most likely not provide the resources necessary to pursue this process.
Therefore, to the standard of due process used for charging misconduct on a UN employee would
not be satisfied with the due process used for investigations and impositions of UN sanctions. While
this example may not be a realistic comparison, the value in its mention is intended to shed light on
the double standards inherent in the due process of imposing UN sanctions (having detrimental
effects), and the due process for imposing a charge of misconduct on a UN staff member (having
much less of an effect than targeted sanctions). This double standard perhaps signifies a need for
reform.
After viewing the evidentiary standards and investigative methods of the different Experts Panels of
the Security Council Monitoring Teams/Groups – as will be done in the remainder of this essay– it
will be demonstrated that almost all Panels of Expert more or less conform to the recommendations
made in The Report of the Informal Working Group of the Security Council on General Issues of
Sanctions (2006) already, without a mandate or common standard outlined by the Security Council.

Solving Ethical Dilemmas in the Accounting Profession
The Dilemma of an Accountant
Baker Greenleaf was one of the Big Eight accounting firms. Daniel Potter was a highly ethical
accountant that placed a lot of value on integrity. He was hired to work as an accountant for the
firm. Baker like many other firms was faced with a dilemma that tested the firm's ethical standings.
Even though a firm may be faced with a difficult decision, the firm's ethical standing helps
determine the best ethical decision for each situation. To reach the best ethical decision it is best to

Dan is a guy that has high ethics and integrity who bases his decisions on those aspects.
Baker Greenleaf is the firm handling the account. Baker's reputation is at stake, if the wrong
decision is made the firm's reputation may be ruined. The firm could lose clients, revenue, and face
lawsuits if the wrong decision is reached. The wrong decision may also cause the firm to lose
investors.
Oliver, who is the project senior and has about six years of experience with Baker risks losing a
promotion. He also risks not gaining exclusive control over the account. This could cause Baker to
lose profit.
The client (SUB) has it reputation at risk. The clients may lose potential buyers if people discovered
the SUB falsifies its reports. The clients may also face lawsuits if the reports are not corrected.
Gene is assisting Dan with the account and she may have a promotion at risk too. She has her
reputation, ethics, and integrity on the line. Participating with the wrong decision could also put her
job at Baker at risk.
Clients/customers of the SUB may be over charged if the reports are falsified. The clients/customers
may also choose to do business with another company because of the situation.
Solution for the Dilemma
Dan's decision to refuse to falsify the reports was the right decision not only for himself but the firm
as well. Dan's decision was based on the deontology approach; he was more concerned about his
duties, obligations, morals,

The New Police Report Manual Devallis Rutledge
WHAT'S THE PROBLEM?
A Field Training Officer (FTO), Rutledge (Author), is on patrol with a patrol officer. He has to type
his first police report and is not doing a good job. His officer was disappointed at what he had read
in his report; he's going to show the rookie how it's done. The officer's report was not only 50%
longer, but full of jargon. The rookie cop stated, "Why do we write like that?" "Because that's the
way we've always done it," replied the officer. After years of being a cop and writing real police
reports, Rutledge became a prosecutor. After numerous trails, Rutledge was still boggled at the way
police reports were written. So he decided to ask ... Show more content on Helpwriting.net ...
1 and spit on Victim No. 2. So the question to the puzzle is who's who; now you're going back to the
first page of the report to find out. Another problem is placing a label in front of a name; only
adding unnecessary wording and needless work. For example, there's no need to say Suspect Lynn
told Victim Cole to, shut up or I'll punch you again. State, "Lynn told Cole to..." Once you've
documented who participated and to who did what, refrain from using labels. This will make the
readers' and your job easier; document all the important facts. Rutledge suggests, "Don't use labels if
you can avoid it."
WHODUNIT?
If someone reading your police report has to play a guessing game as to who did what; then an
important piece is missing from your report. When the trial date approaches, are you really going to
remember who collected what, who searched what if you don't put a name in front of the action
done. For example, by stating Officer Killinger saw cocaine in plain view on the floor of the car;
instead of saying the The Assigned Officer saw cocaine...., you know Officer Killinger found the
cocaine in the car. Then the attorneys know who to subpoena to the fact that cocaine was found in
the car. Sketches at scenes are common and are useful as to who and where certain evidence was
found; sketches can paint a picture not only for jurors and judges, but it will also be beneficial for
you and your colleagues to remember who did what, when they

Lab Report : Science Or Engineering Course Essay
A full laboratory report is usually required in a science or engineering course that has a laboratory
section. Believe it or not, a lab report (as they are commonly referred to) is actually a learning tool
to help you get more out of your lab and classroom experience. You typically enter a lab session
with some hypothesis, such as "copper conducts heat more quickly than steel." You will review the
basic concepts behind the experiment, such as the definition of conductivity and whether heat flows
from hot to cold. Then, you follow a defined procedure for performing a test to verify whether your
hypothesis is correct or not. A key part of that test is recording data. The data will be analyzed and
used to either support or reject your hypothesis. Usually one to two weeks later, you will submit a
laboratory report summarizing what you did and what you learned.
Before you leave the lab session, remember that it is a good idea to record all the equipment you
used (including manufacturer, model and serial number), make sure you have all the data you need,
take some pictures of your setup (to refer to later, or to include in your report), and make sure you
still have a copy of the procedure you used. Believe it or, if you have gathered all that information
then you have already started on your report.
A full lab report typically has six sections:
Introduction: What is your hypothesis? What is the scope of your report? In short, tell the reader
what you are running tests to

Advantages And Disadvantages Of Administrative Reports
Administrative Reports
Overview with tools, software, best practices and alternatives
What is an administrative report?
An administrative report is a document that presents the status and progress of certain work within a
team or a company.
Who does administrative reports?
An administrative report is usually done by a person with an administrative function. Common
examples are office secretaries and administrative delegates. Those with higher position such as
managers and supervisors are also eligible to create an administrative report.
Why do people write administrative reports?
Administrative reports are prepared to give the current status and progress of a certain project within
a team or a company. The report aims to note all aspects of the project within the team, including the
assessment ... Show more content on Helpwriting.net ...
Since the reports inform team members about these existing or rising issues, they will be able to
strategize and develop a plan to counter and resolve such issues.
Disadvantages of administrative reports
While administrative reports help in the workflow of a team's project, it also presents some flaws.
Some administrative reports have inaccurate or incomplete information, which may adversely affect
the assessment, planning, implementation, and evaluation of data. For instance, inaccuracy of the
numbers in an inventory report in an administrative report may affect the succeeding shipment of
products. Another example is a list of cases in an administrative report in a hospital. Failure to list
all kinds of cases may change the results when data are analyzed.
The other negative point in administrative reports is that some readers only read the end part – the
summary, conclusion, and recommendation. It may sometimes end up the reader asking questions,
instead of reading the whole paper.

Weaknesses Of Ir, It Is Not The Silver Bullet That Will...
Weaknesses of IR
Although there are a lot of strengths for IR, it is not the silver bullet that will completely change
accounting forever. One of the main weaknesses of IR is the expenses that come with measuring
such a comprehensive range of factors that include non–monetary factors that can be hard to
actually measure (Eccles & Krzus, 2010). It can be unappealing for smaller companies or
organisations to set up all of these systems in order to provide a wider perspective of their business
or organisation just to please stakeholders, when the costs will outweigh the potential profits of
attracting new investors.
Another issue would be potential information overload for less knowledgeable individuals who will
simply be overwhelmed by all the information provided by IR. There is a question of how important
is it to report more comprehensively if the end user will simply be unable to use all of the
information to their advantage, which raises another question of if it is worth reporting in an IR
format at all.
Time is also a major issue for some companies who may have less labour to spare in order to report
on what can be considered less important aspects of an organisation, especially if it is a small to
medium sized enterprise. There have been reports of some integrated reports being three times as
longer than estimated to be completed (Eccles & Saltzman, 2011). It can be very hard to justify
allocating that many hours to prepare a report that some may find unnecessary or

can also be poor where incidents are hidden or swept under the carpet and no action can be taken to
decrease the possibility of it from happening again. Tools have been developed to assess the safety
culture in many different organizations, usually those that have a high risk. Rules and regulations are
typically implemented following incidents resulting in fatalities. Nuclear, medical and aviation are
some of the main industries that have begun assessing safety culture within their organizations and
implementing SMS systems to help distinguish situations that could have potentially caused an
incident.
Over the years safety has been taken more and more seriously and the safety culture throughout
many organizations in general has been greatly improved upon. This has been mainly due to
mishaps and incidents in the past that have led to fatalities. Following these incidents have been
regulations and measures to ensure that the same thing is less likely to occur or possibly even
impossible to occur again. Since so many regulations are now in place incidents are few and far
between, which is a good

Improving Current System Of Police Reporting
5.5 Improvement to Current Situation
According to five factors that have been proposed, we had suggested some recommendations to
improve current system of police reporting in Malaysia which is the Police Reporting System (PRS)
to ensure that the function of the PRS will definitely helped to record and transfer the report in a
better and easier way.
5.5.1 Role
As mentioned above, role is very important factor in police reporting in order to make the report,
received statement from complainant, and most important thing is to transfer the report to another
police officers. As a recommendation, police organization in Malaysia can organize or provide a
training program for police personnel on duty at the enquiry office about the use and maintenance of
the PRS so that they can write the report quickly and also they know the function for every single
menu and button of the PRS. Plus, if the PRS are breakdown, the police personnel will know how to
fix the problem. Thus, it wills not wasting time to wait for the technical team to fix the problem and
also they no need to write the report manually in POL 55 and re–type it once the PRS was fixed.
Besides that, enquiry officer also are encourage to more than one language. For instance, if the
complainant who come over to lodge a report does not know to speak Bahasa Malaysia, the enquiry
officer are able to speak another language like English, Mandarin, Tamil or any language that easy
to understand by the complainant.

Shortcomings And Weaknesses Of The Mazars Report
What are the shortcomings/weaknesses that you are seeing in integrated reporting in general
(including the Mazars Report)?
From the Mazars Report, there is some shortcoming of integrated reporting (IR) but these
shortcoming are potential to be improved to be a better one as IR is a process of continuation and
improvement of current reporting approaches. Following are the shortcomings of IR from the
Mazars Report:
Risk and opportunities can be identified and explained, however, it is lack of details in analysis and
mitigation measures.
In the event that vital goals are frequently all around portrayed in various time scales, the connection
with resources allocation could be better in the IR.
The integrated performance indicators is weak in term of publication.
Complexity and lack of clarity has pointed out by Welch (2013), Associated of Chartered Certified
Accountants' head of policy, as a potential weakness of IR. Initial objective of IR is to reduce
complexity and increase clarity; however, it might distort the objective of IR while a set of non–
financial data/information have been recommended to embed to the report, and it could lead to
information overload in the report. Moreover, a full tested set of standards is not introduced or
announced even a ... Show more content on Helpwriting.net ...
It's vary with the previous reporting method which is strictly financial report or financial including
CSR report. It require the full support and commitment from the top management, leverage their
power to convey the message of IR adoption to all company personnel. The representative of all the
relevant disciplines within the business are the team who in–charged for the integrated reporting
process implementation, and also the reporting itself. The key of success to create an integrated
reporting process is to ensure that the multi–disciplinary team combines, cooperate and coordinates

The Art Of The Police Report By Lera Boroditsky
Ellen Collects "The Art of the Police Report" complements Lera Boroditsky article on "How does
language shape the way we think" by supporting the proposition constructed in her piece.
Boroditsky claims that language exhibits a substantial portion of constructing humanities mental
capacity and therefore is not only important but essential for proper communication amongst
individuals. Speaking on behalf of Collett's thesis, throughout his article, his writing affirms
Boroditsky premise by mentioning the significance of proper language use in the police academy.
Boroditsky mentions the fact that language plays a primal role in shaping an individual's cognitive
framework. Her arguments testify that without language, it would be nearly ... Show more content

Learning to report as a cop can be viewed as its own language, with its distinct set of norms and
expectations. In order for an individual to master a language, they must first grasp the ordinates
associated with it. Collets vindicate that as a cop "crime reports are written in neutral diction, and in
the dispassionate uni–voice that's testament to the academy's ability to standardize writing. They feel
generated rather than authored, the work of a single law enforcement consciousness rather than a
specific human being." The ability to detach opinion and bias is crucial in reporting. Only through
practice, is one competent to perform this reporting effectively. In the cop department you will have
officers who enjoy writing reports, carefully documenting their activities, and painting "word
pictures" of the events by adding their own distinctive style and trademark, as well as having the
officers who don't particularly enjoy writing reports and it is evident on the pages they complete.
The written reports include missing or incomplete information and poor interviewing/investigation
procedures. What those officers who turn in low quality reports fail to recognize is that they are
doing more harm than they perceive. Although a scenario may seem mundane, the report that is
written for the incident serves

Therapist Case Report

Therapist Case Report Case report is a detailed narrative description of a real life problem or situation as well as a way to apply theoretical knowledge gained from the academic literature to real life situations that may serve as basis for future research. Case report can be done on any subject or topic. It is designed to analyze and understand the factors important to the etiology, care and outcome of the subject's problems, through a detailed description of a patient's background, present status and response to treatment. • 3. The Effects Of News Media On Today 's Society The news media's role in today's society is interlocked and essential to the conveying of information. The news media has access to television broadcasting and has people to go find stories to broadcast. However, as time has progressed, the news media has become more and more opinionated. Consequently, the bias of the news media in their reporting's has tainted the opinions of the American people. By creating a strict set of laws pertaining to the reporting of news, bias within reports will have to decrease; causing a less opinionated audience, people feeling better about themselves and humanity, and the United States growth as a stronger country. The news media in today's world is far too opinionated. On a regular basis, the news media and other news outlets are often discussing opinions, their own opinions, and other's opinions about stories and news reports rather than the reports themselves. During a discussion about Donald Trump's immigration policy on August 25th, 2016, on the CNN newsroom, one of the reporters said that "we heard this shift on his immigration policy." However, one of the other reporters defended Mr. Trump by saying. "First, let me correct the record" and goes on to do so and they argue back and forth (Person Discusses). In this report, what Donald Trump said nor any part of his speech was shared. The report was wholly opinion based. • 5. Integrated Reporting And An Understanding Of A Company 's... Integrated Reporting is a more holistic approach to gaining an understanding of a company's performance. It takes into consideration factors including the environment, society and governance when evaluating a company's performance and strategizing on a future course of action. One of the most recent and promising developments in Integrated Reporting has been the release of the Integrated International Reporting Framework by the International Integrated Reporting Council. This paper assesses the opportunities, benefits, challenges and limitations of this framework. Introduction The Integrated International Reporting Framework was introduced by the International Integrated Reporting Council (IIRC) to guide the entire integrated reporting process and also to clearly define key ideas and concepts of this framework. Given some of the confusion surrounding this framework among companies and accountants in New Zealand with respect to what precisely the Integrated <IR> Framework is and how it might affect accounting practice, this essay purports to clearly explain the framework. Further, this paper will also critically examine the framework, highlighting some of the salient opportunities, benefits, challenges and limitations of this approach. Finally, it will briefly reflect upon the implication of the framework for accounting practice. They hypothesized that the "firms with more complicated transactions likely to have greater chance for a disconnect in the financial reporting process, and thus a material weakness (Ge et al., 148)." To measure business complexity, Ge and McVay used the number of operating segments reported in the annual 10–K and the existence of foreign currency exchange translation. In comparison to Doyle et al. study, Doyle and his colleagues hypothesized that the "complexity of the firm is a driver of internal control weaknesses (Doyle et al., 9)." To measure the business complexity of a firm, Doyle et al. used three measures to include the number of special purpose entities, number of operating segments and the existence of foreign currency adjustment. The findings of Doyle et al. were as predicted. The three complexity measures were higher in firms that reported material weaknesses in comparison to the 2003 Compustat firms, who did not report any material weaknesses. This indicates that company complexity is positively correlated to the disclosure of material misstatements. Company Profitability In relation to the Ge and McVay research study findings, company profitability is negatively correlated with firm disclosure of material weaknesses. Ge and McVay hypothesized that "poorly performing firms simply might not be able to adequately invest in proper internal controls (Ge et al., 151)." This would result in a greater chance of disclosing material weaknesses. In order to test their hypothesis, they used two profitability metrics consisting of return on assets (ROA) and cash from operations scaled by assets (CFO/A). In reference to the Ge and McVay research findings, as presented in Figure B1 , material weakness firms had lower ROA and CFO/A compared to the control group of 2003 Compustat firms (excluding material ... The Role Of Media Management On Crisis Communication The importance of media management in crisis communication Introduction Crisis according to Laura (2013, p. 51), can be defined as any situation that threatens harms or cause panic among people. Crisis may occur due to accidents, war, and flood or fire attack. During this period, the public, who may be affected by the crisis because they are found in the situation or their next of kin are involved in it, rely on a lot of information that is relayed by the media. Because of this, the media must be careful of what they report. The intent of this paper is to critically analyze ethical considerations to support whether or not to report small errors to higher management. The issue at hand is determining whether or not newly hired Ben should notify his supervisor and CFO of small errors found after sending the reports he created for them. Similarly to this definition, the definition of a "small error" begs a number of questions: – Where do these errors come from? – What is Ben's proficiency in Microsoft Excel? – Can anyone outside the company audit the company's books? Ben is conflicted as to how to handle errors found in his work because it may jeopardize either his job, or the future of the entire company. This is his first job and he realizes that his responsibilities are overwhelming at times. His reports are an important component of the company's business, and part of the CFO's work relies on it. On several instances he has found errors in his reports, and decided to only revise and report those errors he believes to be critical. Small errors, on the other hand, are not as important to require reporting according to him. The case however suggests that Ben is also considering whether or not he should in fact overlook the small errors, which is what I will attempt to answer in this critical thinking analysis. Ben's position to disregard small errors is understandable in some aspects. In fact, as a newly hired, in his first job, he must be proud of being given such job with great responsibilities. The Adverse Health Event Law passed in 2003 requires disclosure and examination of specific unfavorable events with corrective action plans with some aspects shared publically in order to educate consumers about health care facilities issues and improvements (MDH 2015). Review of the Utah and Minnesota Incident reporting mandates provided various state statutes and reporting responsibilities to the state's government and regulatory agencies. Reports of adverse events must be reported in various methods among states. For example, Utah and Minnesota require each individual facility, hospital, outpatient centers, and clinics, to report the adverse events that occur at the particular facility. Specifically, in Minnesota if the Boards that regulate physicians, nurses, podiatrist, physician assistants, and/or pharmacists are aware of an adverse event, the specific board holds the responsibility to report the events to the Minnesota Department of Health. Adverse events consist of defined elements within states in the categories of surgical, product or device, patient protection, case management, environmental, and potential criminal events, essentially providing events that produce irreversible patient harm. First, the company and peter's career will be in risk if he reported the oil spill to the state. Second, the engineering law requires confidentiality therefore it is irrelevant that peter report such an issue that is confidential in the company. Third, the tests were only taken 400 feet from the surface of the groundwater, however maybe there is contamination deeper or other marine life were affected. Four, most important concept is the conflict of interest since Peter had to either obey the engineering law that requires confidentiality and neglect his morals or obey the state law and lose his friendship and career. Five, Jesse trusted Peter to keep this mysterious spill between them; he should be loyal to him since they have a trusting relationship. Additionally, Jesse noted that he cannot have a consulting engineer who doesn't value client loyalty. On the other hand, there are some unknown facts in the issue. According to state law all spills must be reported but what about spills that occurred years ago? Where the effects of spill seem to have dissipated? The section of the report addressing the monetary impact of Amalgamated installing emission control equipment had been falsified. The numbers used in the report were over exaggerated in order to show that Amalgamated would suffer severely financially should this legislation be passed. Recommendation #1: Amalgamated has three mills in the community. These three mills and the corporate offices provide jobs to many of the 10,000 people of Northlake. The company is a major contributor to the economy of the community. The recommendation is for Amalgamated to be ethical and show corporate responsibility. No amount of money can be given for human life. The aboriginal community is suffering from the effects of emission waste from the previous competitor and from Amalgamated as well. Amalgamated should forego its own financial gain and do what is ethically right to clean up their waste so that it does not affect the community. The reason for doing this is the news reporter's exigence, or reason for writing the article is to just provide the public with a story and explain the most important details to try to get the public interested in their stories. The news reporter also has a deeper exigence beyond just writing a story, the news reporter gets paid accordingly to how popular their reports are, therefore in reality, news reporters write to get paid, rather than to make a difference in the scientific community. The article goes on to say that the research "may help improve understanding" , in this news report stating that it may help, just provides the reader with the thought that the writer doesn't clearly know the findings just what they have read on the issue, whereas the scientific report intelligibly states the facts because the news reporter is in a discourse community of just writing and reporting, no scientifically based knowledge is needed in their field, so reporters don't need to know much about what they are reporting on, as long as they know the most important details. There are other substantial contrasts between the two as well. This is often followed with the claim that inconsistent standards of investigations and evidentiary standards can lead to poor harmonization and unfair sanctioning, which can even go so far as discrediting of the Sanctions Committees of the Security Council. While these claims may be true to some extent, the argument can be made that by comparing reports, Experts have already formed a degree of consensus in their evidentiary standards and due processes of investigations. Looking at this policy, it is clear that it would be impossible under the current framework to provide this type of due process for all individuals and entities sanctioned by the UN. Therefore, to the standard of due process used for charging misconduct on a UN employee would not be satisfied with the due process used for investigations and impositions of UN sanctions. While this example may not be a realistic comparison, the value in its mention is intended to shed light on the double standards inherent in the due process of imposing UN sanctions (having detrimental effects), and the due process for imposing a charge of misconduct on a UN staff member (having much less of an effect than targeted sanctions). This double standard perhaps signifies a need for reform. For example, there's no need to say Suspect Lynn told Victim Cole to, shut up or I'll punch you again. State, "Lynn told Cole to..." Once you've documented who participated and to who did what, refrain from using labels. This will make the readers' and your job easier; document all the important facts. Then, you follow a defined procedure for performing a test to verify whether your hypothesis is correct or not. A key part of that test is recording data. The data will be analyzed and used to either support or reject your hypothesis. Usually one to two weeks later, you will submit a laboratory report summarizing what you did and what you learned. Before you leave the lab session, remember that it is a good idea to record all the equipment you used (including manufacturer, model and serial number), make sure you have all the data you need, take some pictures of your setup (to refer to later, or to include in your report), and make sure you still have a copy of the procedure you used. Believe it or, if you have gathered all that information then you have already started on your report. A full lab report typically has six sections: Introduction: What is your hypothesis? What is the scope of your report? In short, tell the reader what you are running tests to ... Get more on HelpWriting.net ...

Some administrative reports have inaccurate or incomplete information, which may adversely affect the assessment, planning, implementation, and evaluation of data. For instance, inaccuracy of the numbers in an inventory report in an administrative report may affect the succeeding shipment of products. Another example is a list of cases in an administrative report in a hospital. Failure to list all kinds of cases may change the results when data are analyzed. • 73. Weaknesses Of Ir, It Is Not The Silver Bullet That Will... Weaknesses of IR Although there are a lot of strengths for IR, it is not the silver bullet that will completely change accounting forever. One of the main weaknesses of IR is the expenses that come with measuring such a comprehensive range of factors that include non–monetary factors that can be hard to actually measure (Eccles & Krzus, 2010). It can be unappealing for smaller companies or organisations to set up all of these systems in order to provide a wider perspective of their business or organisation just to please stakeholders, when the costs will outweigh the potential profits of attracting new investors. Another issue would be potential information overload for less knowledgeable individuals who will simply be overwhelmed by all the information provided by IR. There is a question of how important is it to report more comprehensively if the end user will simply be unable to use all of the information to their advantage, which raises another question of if it is worth reporting in an IR format at all. Time is also a major issue for some companies who may have less labour to spare in order to report on what can be considered less important aspects of an organisation, especially if it is a small to medium sized enterprise. There have been reports of some integrated reports being three times as longer than estimated to be completed (Eccles & Saltzman, 2011). It can be very hard to justify allocating that many hours to prepare a report that some may find unnecessary or ... Get more on HelpWriting.net ...

Safety culture is, however much of the same with a developed attitude towards safety. Safety culture can be strong, where people within an organization report incidents that occur or even when they had the opportunity to occur. On the other hand safety culture can also be poor where incidents are hidden or swept under the carpet and no action can be taken to decrease the possibility of it from happening again. Tools have been developed to assess the safety culture in many different organizations, usually those that have a high risk. Rules and regulations are typically implemented following incidents resulting in fatalities. Nuclear, medical and aviation are some of the main industries that have begun assessing safety culture within their organizations and implementing SMS systems to help distinguish situations that could have potentially caused an incident. Over the years safety has been taken more and more seriously and the safety culture throughout many organizations in general has been greatly improved upon. This has been mainly due to mishaps and incidents in the past that have led to fatalities. Following these incidents have been regulations and measures to ensure that the same thing is less likely to occur or possibly even impossible to occur again. From the Mazars Report, there is some shortcoming of integrated reporting (IR) but these shortcoming are potential to be improved to be a better one as IR is a process of continuation and improvement of current reporting approaches. Following are the shortcomings of IR from the Mazars Report: Risk and opportunities can be identified and explained, however, it is lack of details in analysis and mitigation measures.
