From the perspective of the integrative medical care consultant. What type of evidence would you propose to show that an integrative approach produces the best medical outcomes in medical care? As the consultant, how would you address the criteria put forth by your peers? Considering Integration For the purpose of this discussion, I have been asked to meet with an integrated medical care consultant whose job is to propose that my consortium incorporate CAH practitioners into its clinics. As a Chief Medical Officer of a specialty clinic consortium that is dedicated in delivering the highest level of cancer care treatment, I would have several concerns and questions for the consultant. First, what types of therapies will we be offering to our patients? Are there scientific studies available that prove both the efficacy and safety of these therapies? What is the internal cost of making the transformation to integrative care? What are the licensure requirements for the CAH practitioners who will be joining our staff? Are they responsible for the cost of maintaining licensure, or is that our responsibility? This is the most important information I would like to gain from the consultant’s presentation. Moreover, the most important clinical criteria that I would want to consider before I made a contractual decision to integrate CAH practitioners into the consortium clinics are as follows: An outcome assessment of this integrative clinical model that includes concurrent measurement of biologic, psychologic, social, spiritual, and contextual dimensions of whole-person health as well as patterns of change in medical conditions, risk factors, and/or health behaviors that occur during the integrative medical treatment (Greeson, Rosenzweig, Halbert, Cantor, Keener, & Brainard, 2008, p. 4) An assessment of current studies that prove both efficacy and safety in CAH therapies to be integrated An assessment of current policies and procedures, as well as any adjustments that would need to be made for integration An assessment of key concerns of integration for patients and staff alike A risk assessment and liability analysis (Ventola, 2010, p. 3) An assessment of the integrative model of care’s scope of practice An assessment of the ethical considerations of integration I feel that next to efficacy and safety, cost effectiveness and ethical implications are the most important elements to consider when discussing making the transformation to an integrative care facility. As a manager in Ford Rehabilitation centre, I have encountered several challenges in both external environment and internal environment that have greatly challenged the increasing demands of my patients’ services as well as failure of the reimbursements of funds by the insurance providers. Environmental conditions normally affect human health in varied means. Interactions between the environment and human health usually lead to very complex ethical queries that are related to health policy decisions. There are various factors in the environment that can lead to risks and the same time benefits. They include genetically modified plants, nanotechnology, bio fuels and other technology. There is a body of evidence that have emerged saying that environment can affect the health of human being and at the same time human health can have impact to the environment.
The external factors are factors in the environment that cannot be controlled by an organization. There are several external factors that affect many health organizations; these factors include political conditions, government policies and regulations, technological environment and social environment. In my organization the two key external factors affecting my company are the social environment and technological environment. Social factors have developed challenge in the Ford rehabilitation centre. This is because many patient customers have varied and different types of beliefs which make the relations in the health centre challenged. It have become problematic to deal with some patients since it is difficult to know the type of services they need based on where they have come from. Various patients have diverse transformation in attitude towards health care. The patients are however very demanding in my organization because each one of them needs to be handled differently based on community variations. In order to curb this, as manager I have decided to implement several programs that will promote cooperation between my patients as well amendments that will bring in suitable services to each patient. Implementation of this programs will enable my organization to continue being indispensible and financially stable despite the social challenges affecting the availability of patients in the organization.
Another external factor in the environment that will have a great impact in my company is technological environment. Implementation of more advanced methods to serve my customers is likely to improve patients’ attendance and this will boost the compan.

Measures of consumer satisfaction can serve an important role in monitoring quality and improving health care. Oftentimes overshadowed by measures of clinical process and outcomes in monitoring health care quality, consumer satisfaction has emerged as an important indicator of quality (see Chapter 5). At one time relegated to service improvement efforts by hospitals, measures of patient—or consumer—satisfaction are recognized as the provider’s best source of information about “communication, education, and pain-management process, and they (patients) are the only source of information about whether they were treated with dignity and respect” (Cleary, 2003, p. 33). Consumers’ experiences can stimulate important insights into how a provider is operating and suggest changes that may “close the chasm between the care provided and that care that should be provided” (Cleary, 2003, p. 33). Furthermore, the marketplace in which the providers operate is demanding that data on patient satisfaction be used to empower consumers and foster provider accountability and consumer choice. Measuring consumer satisfaction provides a comprehensive, systematic, and patient-centered approach for analysis, implementation, monitoring, and improving both the perceived and the clinical quality aspects of care (Ford et al., 1997).
This chapter provides an overview of key issues and methods related to measuring consumer satisfaction. The rationale for measurement is discussed and followed by a series of issues: measurement, data capture, timing, and functional responsibility. An example applying patient satisfaction measures as part of the Balanced Scorecard (a measurement system that adds customer and other dimensions to the customary financial measures [Kaplan and Norton, 1996]) is presented. We conclude with a brief overview of the special issue of case-mix adjustment of reported consumer satisfaction measures.

“Not only do people bolster beliefs in their ability to control in response to successful control of an event but also they hold an unwarranted belief that they can control chance events,” states Yamaguchi (Matsumoto (Ed.), 2001, pp. 226–227) in the course text. While members of all cultures have the goal of protecting self-image following failures, differences exist among cultures in terms of the attributions made for the failure and success of a task. Thus, while the self-serving bias is universally applied, the specific attributions made differ cross-culturally. In some cultures, it is assumed that failure is attributable to situational factors while others assume dispositional factors.
Differences also exist in how the failure or success of another individual is attributed. Consider the relevance of attributions for success and failure for the scholar-practitioner working in a multicultural environment or in a global company. How would knowledge of how individuals’ attribute their own or others failure impact a team, classroom, or organization?

After two years as a staff nurse, I was promoted to a clinical management position in a Post-Acute Care (PAC) facility. Since then, I have worked in a variety of milieus, including both free-standing and hospital-based facilities and served as a corporate nurse, a traveling nurse, and a private consultant. My nursing specialty has grown exponentially over the years. My role now encompasses reporting to many governmental agencies, on both a state and federal level, and most recently, I am involved in mandated Quarterly Reporting Program (SNF QRP). I am currently working as a Regional Clinical Reimbursement Specialist for a major Post-Acute Care Company.
I have been in the healthcare field since I was a junior in high school, working as a candy striper, certified nursing assistant, and home health aide before becoming an RN. So, I have been both exposed to and used critical thinking for a lot of years. In the recent past, the fee for service healthcare trend generated money through the volume of patients or census levels and viewed the patient as a source of revenue. Whereas, the new trend of value-based healthcare has changed healthcare, particularly in my industry of post-acute care. Our patients have now become an expense versus a source of revenue. This change has made it necessary for different types of healthcare providers to stop working in silos and instead communicate and collaborate with care providers across the continuum. Each provider will need to leverage each other’s strengths to provide the best possible outcomes for the patient. Also, with this new value-based payment model system, providers, and doctor’s need to begin to see cradle to grave care within a fixed premium payment.

An area of interest for a future career is hospital administration; therefore, the most relevant topic is hospital departments and allied professions. This profession includes interacting with all kinds of professions within the hospital, dentistry, pharmacy, security, and many others. Anytime an issue occurs in any of these areas, hospital and medical administrators must deal directly with the issue and solve the problems through the help of committees, boards, and potentially stakeholders. Overall, “healthcare professionals are held to the prevailing standard of care required in their profession, which include proper assessments, reassessments, diagnosis, treatment, and follow-up care” (Pozgar, 2016, p. 292). Due to high standards and expectations such as this, hospital administrators must be ready to assume responsibility when necessary, and work diligently to educate staff. Overall, there are many legal and ethical issues within this spectrum.
A current legal issues faced by hospital systems is HIPAA and data breaches (Becker’s Hospital Review, 2011). Due to the shift from hard copies of records to electronic record, there is a rise in the breach of personal data. Sharing electronic information between systems also increases the potential for more security breaches, directly impacting the patients served by hospitals and health systems (Becker’s Hospital Review, 2011). As a result of data breaches, lawsuits inevitably occur costing hospitals millions of dollars. One such case occurred in Maryland in 2011, and cost Cignet Health $4.3 million (Becker’s Hospital Review, 2011). While many benefits exist to using electronic sharing, there are also great risks and legal issues that can derive from it as well.
Another legal issue, and potentially the most common, is medical malpractice. When a patient feels they have been injured by their physician, they have the right to sue (Sabatino, 2017). While this may seem like a legal issues for the physician, and not the hospital, it can quickly become the hospital’s issue. Many cases like this result in a lawsuit against a hospital or health care system, who back their physician. Malpractice can include inappropriate treatments, unnecessary mistakes in surgeries, inappropriate transfers, and inappropriate discharge (Pozgar, 2016). These mistakes can directly cause or lead to the causation of personal injury in the patient, therefore resulting in legal issues. Many cases of medical malpractice are dismissed or found to be erroneous; however, as a hospital administrator there is the added pressure of protecting the image and reputation of the hospital or health care system.
There are also many ethical issues to consider as a hospital administrator. According to Larson, one of the top five ethical issues is balancing care quality and efficiency (2013). The e.

Measures of consumer satisfaction can serve an important role in monitoring quality and improving health care. Oftentimes overshadowed by measures of clinical process and outcomes in monitoring health care quality, consumer satisfaction has emerged as an important indicator of quality (see Chapter 5). At one time relegated to service improvement efforts by hospitals, measures of patient—or consumer—satisfaction are recognized as the provider’s best source of information about “communication, education, and pain-management process, and they (patients) are the only source of information about whether they were treated with dignity and respect” (Cleary, 2003, p. 33). Consumers’ experiences can stimulate important insights into how a provider is operating and suggest changes that may “close the chasm between the care provided and that care that should be provided” (Cleary, 2003, p. 33). Furthermore, the marketplace in which the providers operate is demanding that data on patient satisfaction be used to empower consumers and foster provider accountability and consumer choice. Measuring consumer satisfaction provides a comprehensive, systematic, and patient-centered approach for analysis, implementation, monitoring, and improving both the perceived and the clinical quality aspects of care (Ford et al., 1997).
This chapter provides an overview of key issues and methods related to measuring consumer satisfaction. The rationale for measurement is discussed and followed by a series of issues: measurement, data capture, timing, and functional responsibility. An example applying patient satisfaction measures as part of the Balanced Scorecard (a measurement system that adds customer and other dimensions to the customary financial measures [Kaplan and Norton, 1996]) is presented. We conclude with a brief overview of the special issue of case-mix adjustment of reported consumer satisfaction measures.

While some expressions of aggression are universal, cross-cultural differences exist in the type and level of aggression that are considered to be legally or socially sanctioned. There have been multiple reasons proposed by theorists to explain these cultural differences in the type (verbal, physical, etc.) and level of aggression expressed across cultures.

“Not only do people bolster beliefs in their ability to control in response to successful control of an event but also they hold an unwarranted belief that they can control chance events,” states Yamaguchi (Matsumoto (Ed.), 2001, pp. 226–227) in the course text. While members of all cultures have the goal of protecting self-image following failures, differences exist among cultures in terms of the attributions made for the failure and success of a task. Thus, while the self-serving bias is universally applied, the specific attributions made differ cross-culturally. In some cultures, it is assumed that failure is attributable to situational factors while others assume dispositional factors.
Differences also exist in how the failure or success of another individual is attributed. Consider the relevance of attributions for success and failure for the scholar-practitioner working in a multicultural environment or in a global company. How would knowledge of how individuals’ attribute their own or others failure impact a team, classroom, or organization?

Across these years, I have practiced nursing in the states of New Jersey, Pennsylvania, and my home state of Massachusetts, to which I returned two years ago. When I graduated from a hospital-based nursing program, I took a job as a Post-Acute/Rehab Care staff nurse. After two years as a staff nurse, I was promoted to a clinical management position in a Post-Acute Care (PAC) facility. Since then, I have worked in a variety of milieus, including both free-standing and hospital-based facilities and served as a corporate nurse, a traveling nurse, and a private consultant. My nursing specialty has grown exponentially over the years. My role now encompasses reporting to many governmental agencies, on both a state and federal level, and most recently, I am involved in mandated Quarterly Reporting Program (SNF QRP). I am currently working as a Regional Clinical Reimbursement Specialist for a major Post-Acute Care Company.
I have been in the healthcare field since I was a junior in high school, working as a candy striper, certified nursing assistant, and home health aide before becoming an RN. So, I have been both exposed to and used critical thinking for a lot of years. Nevertheless, I am excited to broaden my understanding, experience, and critical thinking skills with new and fresh information on the best practices approach of nurse leaders in today's healthcare environment.
How do you see current trends and changes in healthcare models impacting the delivery of healthcare? In the recent past, the fee for service healthcare trend generated money through the volume of patients or census levels and viewed the patient as a source of revenue. Whereas, the new trend of value-based healthcare has changed healthcare, particularly in my industry of post-acute care. Our patients have now become an expense versus a source of revenue. This change has made it necessary for different types of healthcare providers to stop working in silos and instead communicate and collaborate with care providers across the continuum. Each provider will need to leverage each other’s strengths to provide the best possible outcomes for the patient. Also, with this new value-based payment model system, providers, and doctor’s need to begin to see cradle to grave care within a fixed premium payment.

An area of interest for a future career is hospital administration; therefore, the most relevant topic is hospital departments and allied professions. This profession includes interacting with all kinds of professions within the hospital, dentistry, pharmacy, security, and many others. Anytime an issue occurs in any of these areas, hospital and medical administrators must deal directly with the issue and solve the problems through the help of committees, boards, and potentially stakeholders. Overall, “healthcare professionals are held to the prevailing standard of care required in their profession, which include proper assessments, reassessments, diagnosis, treatment, and follow-up care” (Pozgar, 2016, p. 292). Due to high standards and expectations such as this, hospital administrators must be ready to assume responsibility when necessary, and work diligently to educate staff. Overall, there are many legal and ethical issues within this spectrum.
A current legal issues faced by hospital systems is HIPAA and data breaches (Becker’s Hospital Review, 2011). Due to the shift from hard copies of records to electronic record, there is a rise in the breach of personal data. Sharing electronic information between systems also increases the potential for more security breaches, directly impacting the patients served by hospitals and health systems (Becker’s Hospital Review, 2011). As a result of data breaches, lawsuits inevitably occur costing hospitals millions of dollars. One such case occurred in Maryland in 2011, and cost Cignet Health $4.3 million (Becker’s Hospital Review, 2011). While many benefits exist to using electronic sharing, there are also great risks and legal issues that can derive from it as well.
Another legal issue, and potentially the most common, is medical malpractice. When a patient feels they have been injured by their physician, they have the right to sue (Sabatino, 2017). While this may seem like a legal issues for the physician, and not the hospital, it can quickly become the hospital’s issue. Many cases like this result in a lawsuit against a hospital or health care system, who back their physician. Malpractice can include inappropriate treatments, unnecessary mistakes in surgeries, inappropriate transfers, and inappropriate discharge (Pozgar, 2016). These mistakes can directly cause or lead to the causation of personal injury in the patient, therefore resulting in legal issues. Many cases of medical malpractice are dismissed or found to be erroneous; however, as a hospital administrator there is the added pressure of protecting the image and reputation of the hospital or health care system.
There are also many ethical issues to consider as a hospital administrator. According to Larson, one of the top five ethical issues is balancing care quality and efficiency (2013). The e.

Outcomes can be measured at the individual, group, or organizational level, and in the short, intermediate, or long term. Outcomes research establishes evidence-based practices and evaluates care delivery. It is important for value-based care, where reimbursement is based on patient outcomes. The Agency for Healthcare Research and Quality and the Patient-Centered Outcomes Research Institute are two organizations that fund outcomes research to improve healthcare quality, safety, and value. Research in clinical suicidology continues to rapidly expand, much of it with implications
for day-to-day clinical practice. Clinicians routinely wrestle with how best to integrate
recent advances into practice and how to do so in efficient and effective fashion. This
article identifies five critical domains of recent research findings and offers examples
of simple questions that can easily be integ.

Pathological Conditions in Older Adults
As one goes through the natural aging process, the body's capacity to defend itself against infections diminishes. The immune system's ability to offer protection is reduced, and the individual becomes susceptible to conditions that affect them more than other age groups (Haynes, 2020). This population also experiences other symptoms impairing other aspects of their lives as time passes. For instance, their skin and bones lose their integrity and become more prone to abrasions and breakage. This assignment module will examine the pathological conditions that affect the sexual response in older adults and how and why nutritional and psychological factors, drugs, and other alternative and complementary medications affect the immune system of the populations.
Pathological Conditions that Affect Sexual Response in Older Adults
Sexuality is an essential aspect of life, irrespective of the age group one is in—the older population and the younger generation alike need to explore sexuality to maintain health and well-being. Exploring sexuality is also a mixture of biological, psychological, social, and religious factors, all of which have plenty to do with aging. Genitourinary Syndrome of Menopause
These are the changes experienced in the genitourinary pathway as one age. The individual can feel a burning sensation, dryness, or irritation. This can lead to painful sexual encounters, which can, in turn, reduce their desire to engage and their response.

This is a degenerative disorder of the mental faculties, predominantly among the elderly (National Institute on Aging, n.d.). Their judgment diminishes, making them disinterested or utterly unaware of their sexual experiences. Some forms of the condition have been shown to increase sex or closeness, but the individual may fail to recognize what is appropriate and what is not.

While the move toward regional economic integration is generally seen as a good thing, some observers worry that it will lead to a world in which regional trade blocs compete against each other.
In this possible future scenario, free trade will exist within each bloc, but each bloc will protect its market from outside competition with high tariffs.
The specter of the EU and NAFTA turning into economic fortresses that shut out foreign producers with high tariff barriers is worrisome to those who believe in unrestricted free trade. If such a situation were to materialize, the resulting decline in trade between blocs could more than offset the gains from free trade within blocs.
The Opening Case: Regional Trade Pacts Give the Mexican Auto Industry an Edge describes the growth of the Mexican auto industry due to a wave of foreign direct investment spurred by regional trade agreements. Beginning with the establishment of the North American Free Trade Agreement (NAFTA) in 1994, Mexico has established over 40 different free trade agreements that provide Mexican automakers duty-free access to markets throughout the world. These agreements, along with Mexico’s relatively low labors costs, have attracted investments from many of the world’s largest car companies and auto parts suppliers, making Mexico the sixth-largest car producer in the world.

MAPPING AND ANALYZING VALUE STREAMS
What It Means
The mapping and analysis of value streams is an undertaking focused on gaining insight into the current
state of workflow. It includes creating diagrams that show the various steps a product or service
undergoes from request to delivery. The format of the maps should not be overly complex, and they do
not require any specialized software to create. Their purpose is to help management and workers “see”
the flow of work tasks that are often difficult to visualize, and to analyze that flow from a customer-centric
perspective that is explicitly focused on adding value.
Why It Matters
• It aligns stakeholders around a common understanding of the starting point before any
operational change initiatives are undertaken.
• It focuses on value as the critical metric of operational excellence and competitive advantage,
and it identifies steps where no value is added and/or waste occurs.
• It requires the team to apply quantifiable performance metrics to the current state so that
improvement efforts can be measured and validated.

Datril: Pioneering the Acetaminophen Market
Marvin Koslow, vice president for marketing services at Bristol-Myers, had just finished a
strategic planning meeting with his staff. The meeting was supposed to delineate a strategy for
Bristol-Myers’ newest offering: a non-prescription acetaminophen1 drug branded as Datril, which
was to be launched later in the spring of 1975. Despite many hours of deliberation, a clear solu-
tion failed to emerge. The main unresolved issue was how to position Bristol-Myers’ new drug to
the general public and, in particular, how to price and promote Datril.
Bristol-Myers was one of the nation’s leading marketers of pharmaceutical and consumer prod-
ucts, with a portfolio of well-known brands such as Bufferin, Excedrin, Windex, Drano, Vitalis, Be-
hold, and Clairol. Bristol-Myers was comprised of three major product groups: the Consumer Prod-
ucts Group, which accounted for 25.8% of the company’s total sales in 1974; the Clairol Group; and
the Pharmaceutical, Health Care, and International Group, which accounted for 51.8% of the compa-
ny’s total sales in 1974. In 1974, Bristol-Myers reported net earnings exceeding $120 million on $1.6
billion in net sales. The cost of goods sold accounted for more than $600 million of the expenditures:
marketing, sales, and administration expenses were in excess of $420 million, advertising and pro-
motion expenses were nearly $300 million, and research and development expenses were close to
$50 million.
The $680 million analgesic market was divided into two types of pain-relievers: aspirin and ace-
taminophen. Aspirin-based products, such as the market leader Bayer Aspirin, distributed by Sterling
Drug Company, and Bufferin and Excedrin, both distributed by Bristol-Myers, comprised 90% of the
analgesic market. Most of the aspirin-based products were proprietary drugs and were typically mar-
keted with heavy consumer advertising and extensive point-of-purchase displays. Bristol-Myers spent
more than $30 million advertising Bufferin and Excedrin in 1974 (see Exhibits 1, 2 for examples of
print advertisements). In contrast, acetaminophen-based analgesics, initially available only by pre-
scription, were advertised primarily to medical professionals.

- 1. From the perspective of the integrative medical care consultant. What type of evidence would you propose to show that an integrative approach produces the best medical outcomes in medical care? As the consultant, how would you address the criteria put forth by your peers? Considering Integration For the purpose of this discussion, I have been asked to meet with an integrated medical care consultant whose job is to propose that my consortium incorporate CAH practitioners into its clinics. As a Chief Medical Officer of a specialty clinic consortium that is dedicated in delivering the highest level of cancer care treatment, I would have several concerns and questions for the consultant. First, what types of therapies will we be offering to our patients? Are there scientific studies available that prove both the efficacy and safety of these therapies? What is the internal cost of making the transformation to integrative care? What are the licensure requirements for the CAH practitioners who will be joining our staff? Are they responsible for the cost of maintaining licensure, or is that our responsibility? This is the most important information I would like to gain from the consultant’s presentation. Moreover, the most important clinical criteria that I would want to consider before I made a contractual decision to integrate CAH practitioners into the consortium clinics are as follows: An outcome assessment of this integrative clinical model that includes concurrent measurement of biologic, psychologic, social, spiritual, and contextual dimensions of whole-person health as well as patterns of change in medical conditions, risk factors, and/or health behaviors that occur during the integrative medical treatment (Greeson, Rosenzweig, Halbert, Cantor, Keener, & Brainard, 2008, p. 4) An assessment of current studies that prove both efficacy and
- 2. safety in CAH therapies to be integrated An assessment of current policies and procedures, as well as any adjustments that would need to be made for integration An assessment of key concerns of integration for patients and staff alike A risk assessment and liability analysis (Ventola, 2010, p. 3) An assessment of the integrative model of care’s scope of practice An assessment of the ethical considerations of integration I feel that next to efficacy and safety, cost effectiveness and ethical implications are the most important elements to consider when discussing making the transformation to an integrative care facility.
