EXPERIENCE
Coder Level II (June 2006 – Present)

Currently coding medical charts in I-10 and PCS.
Responsible for coding Outpatient Ancillaries, Outpatient Preopts, Rehabs (Physical,
Speech, and Occupational Therapies, Radiation/Oncology, and Cardiology Records.
Trained in I-10 and PCS since June 2011, with weekly testing and audits.
Previously coded medical charts in ICD-9-CM and CPT Coding using 3-M Software.
Responsible for coding Outpatient Ancillaries, Outpatient Preopts, Rehabs (Physical,
Speech, and Occupational Therapies, Radiation/Oncology, Emergency Room Charts, and
Cardiology Records.
Successfully trained and coded Gastrointestinal Charts, with the usage of Modifiers 59,
70, PT, and CCE Edits.
Each Queue has to be coded from on a daily basis, maintaining a ninety-five percent
accuracy rate. In addition, the target goal must be maintained at coding fifteen charts per
hour. Audit weekly reports to see which queue is delinquent due to unsigned orders, and
unsigned plan of care. Evaluate charts to make sure Medicare requirements are met. In
addition, create coding queries to get proper documentation for diagnosis, procedures,
and signatures on all orders. Attend coding roundtables and seminars for education
regarding new coding guidelines and procedures for infusions and injections. Train and
educate students currently enrolled in HealthCare Management. Educate departments on
procedures, proper documentation and coding guidelines.
Coder Level I (June 2004 – June 2006)

Responsibilities included coding all Outpatient Ancillaries, Preopts, and Series charts.
Under limited supervision, interact with others in Health Information Management to
gather and properly document information needed to properly code. Provide correct and
proper documentation for other hospital staff.

Responsibilities included scanning, indexing, and verifying Medical Records on a daily
basis. Audit each record for deficiencies, such as torn paper, staples, loose tape, and light
copies. In addition, charts were properly analyzed for hazards such as blood spills. Lead
work activities through meetings, managing workloads, and communicating to other
hospital staff the flow of charts for the workday.

Responsibilities included pulling charts for next day appointments, auditing chart for
deficiencies, filing all lab, pathology reports, consults, and thinning charts for offsite
medical records storage.

filing all lab work, copying charts for release of information, and scheduling routine lab
