BASIC LIFE SUPPORT
(ADULT)

ā€¢ Basic Life Support, or BLS, generally refers to the type of care that
first-responders, healthcare providers and public safety professionals
provide to anyone who is experiencing cardiac arrest, respiratory
distress or an obstructed airway.
ā€¢ It requires knowledge and skills in cardiopulmonary resuscitation
(CPR), using automated external defibrillators (AED) and relieving
airway obstructions in patients of every age.
ā€¢ In addition to the physical skills necessary for providing BLS care to
those in need, those working in basic life support also need to have
critical-thinking and problem-solving skills, and be able to work as
part of a team to deliver the best care possible and improve patient
outcomes.
General Concepts of Basic Life Support
ā€¢ According to the Centers for Disease Control (CDC), heart disease
continues to be the leading cause of death in the United States and is
responsible for over 600,000 deaths every year.
ā€¢ Research continues to improve how we respond with life-saving
techniques to emergencies. These techniques are based on the most
current research and are organized into a systematic response called
the Chain of Survival, which begins with Basic Life Support (BLS).
ā€¢ The Chain of Survival provides the victim the best chance to receive
the care needed and return to a healthy life.
General Concepts of Basic Life Support
General Concepts of Basic Life Support
ā€¢ The heart pumps blood through the lungs, where blood takes in
oxygen and releases carbon dioxide.
ā€¢ This blood then returns to the heart where it is pumped out to vital
organsā€”the heart and brainā€”as well as the rest of the body.
ā€¢ When the heart stops, blood flow stops, and the person quickly
becomes unconscious.
ā€¢ Without blood flow, the heart and the brain quickly become damaged
due to a lack of oxygen.
ā€¢ The actions that make up BLS try to prevent or slow the damage until
the cause of the problem can be corrected.
ā€¢ BLS improves a personā€™s chance of surviving until advanced care
becomes available.
ā€¢Taking the right action quickly and
confidently can make the difference
between life and death for a person dealing
with cardiac arrest.

Initiating Chain of Survival
ā€¢ Early initiation of BLS has been shown to increase the probability of survival for
a person dealing with cardiac arrest.
ā€¢ To increase the odds of surviving a cardiac event, the rescuer should follow the
steps in the Adult Chain of Survival.

2020 BLS Guideline Changes
ā€¢ Approximately every five years the International Liaison Committee on
Resuscitation (ILCOR), updates the guidelines for CPR and ECC (Emergency
Cardiac Care).
ā€¢ The content contained herein is based on the most recent ILCOR publications on
BLS. Recommendations for adult basic life support (BLS) from the 2020
Guidelines for CPR and ECC include the following:
1. The importance of early initiation of CPR by lay rescuers has been re-
emphasized. The risk of harm to the patient is low if the patient is not in
cardiac arrest. Bystanders should not be afraid to start CPR even if they are not
sure whether the victim is breathing or in Cardiac Arrest.
2. A sixth link, Recovery, was added to the Chains of Survival for both Pediatric
and Adults.

3. Care of the patient after the return of spontaneous circulation (ROSC) requires
close attention to oxygenation, blood pressure control, evaluation for percutaneous
coronary intervention, targeted temperature management, and multimodal
neuroprognostication.
4. Because recovery from cardiac arrest continues long after the initial
hospitalization, patients should have formal assessment and support for their
physical, cognitive, and psychosocial needs.
5. After a resuscitation, debriefing for lay rescuers, EMS providers, and hospital-
based healthcare workers may be beneficial to support their mental health and
well-being.
6. Management of cardiac arrest in pregnancy focuses on maternal resuscitation,
with preparation for early perimortem cesarean delivery if necessary to save the
infant and improve the chances of successful resuscitation of the mother.
2020 BLS Guideline Changes
ā€¢ Updates in the ratio of rescue breath in pediatrics:
ā€¢ The 2015 guidelines recommended traditional CPR cycles of 30 chest
compressions to two rescue breaths for one-rescuer CPR in all age
groups and for two-rescuer CPR in adults.
ā€¢ The 15:2 ratio of compressions to breaths that was presented in the
2015 guidelines only for use in two-rescuer CPR for children and
infants is now the recommended assisted ventilation rate for all
pediatric resuscitation scenarios (rescue breathing or advanced
airway). This will increase the respiratory rate to 20-30 breaths per
minute by having 1 breath every 2 to 3 seconds.
BLS for Adults
ā€¢ BLS for adults focuses on doing several tasks simultaneously.
ā€¢ In previous versions of BLS, the focus was primarily on one-rescuer
CPR.
ā€¢ In many situations, more than one person is available to do CPR.
ā€¢ This simultaneous and choreographed method includes performing
chest compressions, managing the airway, delivering rescue breaths,
and using the AED, all as a team.
ā€¢ By coordinating efforts, a team of rescuers can save valuable seconds
when time lost equals damage to the heart and brain.
Simple Adult BLS Algorithm
One Rescuer BLS/CPR for Adults
ā€¢ Be Safe
ā€¢ Make sure the scene is safe before proceeding.
ā€¢ Move the person out of traffic.
ā€¢ Move the person out of water and dry the person. (Drowning persons
should be removed from the water and dried off; they should also be
removed from standing water, such as puddles, pools, gutters, etc.)
ā€¢ Be sure you do not become injured yourself.
ā€¢ Assess the Person
ā€¢ Shake the person, tap their shoulder hard, and talk to them loudly.
ā€¢ Check to see if the person is breathing. (Agonal breathing, which is
occasional gasping and is ineffective, does not count as breathing.)
One Rescuer BLS/CPR for Adults
ā€¢ Call EMS
ā€¢ Send someone for help or to call your emergency number and to get an
AED.
ā€¢ If alone, call for help while assessing for breathing and pulse. (The
ILCOR emphasizes that cell phones are available everywhere now and
most have a built-in speakerphone. Call for help without leaving the
person.)
ā€¢ CPR
ā€¢ Begin sets of compressions and rescue breaths.
ā€¢ Defibrillate
ā€¢ Attach the AED pads when available.
CPR STEPS
1. Check for the carotid pulse on the side of the neck. Keep in
mind to not waste time trying to feel for a pulse; feel for no
more than 10 seconds.
ā€¢ If you are not sure you feel a pulse, begin CPR with a cycle of
30 chest compressions and two breaths.

2. Use the heel of one hand on the lower half of the sternum in
the middle of the chest.

3. Put your other hand on top of the first hand.
4. Straighten your arms and press straight down.
Compressions should be 2 to 2.4ā€³ (5 to 6 cm) into the personā€™s
chest and at a rate of 100 to 120 compressions per minute.

5. Be sure that between each compression you completely stop
pressing on the chest and allow the chest wall to return to its
natural position.
Leaning or resting on the chest between compressions can keep
the heart from refilling in between each compression and make
CPR less effective.

6. After 30 compressions, stop compressions and open the airway by tilting the
head and lifting the chin.

ā€¢ Put your hand on the personā€™s forehead and tilt the head back.
ā€¢ Lift the personā€™s jaw by placing your index and middle fingers on the lower jaw;
lift up.
ā€¢ Do not perform the head-tilt/chin-lift if you suspect the person may have a neck
injury. In that case, the jaw-thrust is used.
ā€¢ For the jaw-thrust maneuver, grasp the angles of the lower jaw and lift it with
both hands, one on each side, moving the jaw forward. If their lips are closed,
open the lower lip using your thumb.

7. Give a breath while watching the chest rise. Repeat while
giving a second breath. Breaths should be delivered over one
second.
8. Resume chest compressions. Switch quickly between
compressions and rescue breaths to minimize interruptions in
chest compressions.
Two-Rescuer BLS/CPR for Adults
ā€¢ Many times there will be a second person available that can act as a
rescuer.
ā€¢ The ILCOR emphasizes that cell phones are available everywhere now
and most have a built-in speakerphone.
ā€¢ Direct the second rescuer to call 911 or Emergency Medical Services
(EMS) without leaving the person while you begin CPR.
ā€¢ This second rescuer can also find an AED while you stay with the
person.
ā€¢ When the second rescuer returns, the CPR tasks can be shared.
Two-Rescuer BLS/CPR for Adults
ā€¢ The second rescuer prepares the AED for use.
ā€¢ You begin chest compressions and count the compressions out loud.
ā€¢ The second rescuer applies the AED pads.
ā€¢ The second rescuer opens the personā€™s airway and gives rescue
breaths.
ā€¢ Switch roles after every five cycles of compressions and breaths. One
cycle consists of 30 compressions and two breaths.
Two-Rescuer BLS/CPR for Adults
ā€¢ Be sure that between each compression you completely stop pressing
on the chest and allow the chest wall to return to its natural position.
Leaning or resting on the chest between compressions can keep the
heart from refilling in between each compression and make CPR less
effective. Rescuers who become tired may tend to lean on the chest
more during compressions; switching roles helps rescuers perform
high-quality compressions.
ā€¢ Quickly switch between roles to minimize interruptions in delivering
chest compressions.
ā€¢ When the AED is connected, minimize interruptions of CPR by
switching rescuers while the AED analyzes the heart rhythm. If a
shock is indicated, minimize interruptions in CPR. Resume CPR as
soon as possible.
Adult Mouth-to-Mask and Bag-Mask Ventilation
ā€¢ ADULT MOUTH-TO-MASK VENTILATION
ā€¢ In one-rescuer CPR, breaths should be supplied using a pocket mask, if
available.
1.Give 30 high-quality chest compressions.
2.Seal the mask against the personā€™s face by placing four fingers of one hand
across the top of the mask and the thumb of the other hand along the bottom edge
of the mask.

2. Using the fingers of your hand on the bottom of the mask, open the airway
using the head-tilt/chin-lift maneuver. (Do not do this if you suspect the person
may have a neck injury).
Press firmly around the edges of the mask and ventilate by delivering a breath over
one second as you watch the personā€™s chest rise.

ā€¢ ADULT BAG-MASK VENTILATION IN TWO-RESCUER CPR
ā€¢ If two people are present and a bag-mask device is available, the second rescuer
is positioned at the victimā€™s head while the other rescuer performs high-quality
chest compressions. Give 30 high-quality chest compressions.
1.Deliver 30 high-quality chest compressions while counting out loud.

ā€¢ ADULT BAG-MASK VENTILATION IN TWO-RESCUER CPR
2. The second rescuer holds the bag-mask with one hand using the thumb and
index finger in the shape of a ā€œCā€ on one side of the mask to form a seal between
the mask and the face, while the other fingers open the airway by lifting the
personā€™s lower jaw.

3. The second rescuer gives two breaths over one second each as you watch the
personā€™s chest rise.
4. Practice using the bag-valve-mask; it is essential to forming a tight seal and
delivering effective breaths.

ā€¢ ADULT MOUTH-TO-MOUTH VENTILATION
ā€¢ When a pocket mask or bag-mask is not available, it may be necessary
to give mouth-to-mouth breaths during CPR.
ā€¢ Mouth-to-mouth breathing is very effective in delivering oxygen into
the personā€™s lungs without putting the rescuer at a high level of risk.
ā€¢ The rescuerā€™s exhaled air contains approximately 17% oxygen and 4%
carbon dioxide.
ā€¢ This is in contrast to the 100% oxygen available with ventilation with
100% high flow oxygen and the 21% oxygen that is available in room
air that we breathe.

ā€¢ ADULT MOUTH-TO-MOUTH VENTILATION
ā€¢ Do not give breaths too rapidly or too forcefully. Doing this may cause air to be
forced into the stomach, resulting in distention and less room for lung expansion.
It may also cause vomiting.
ā€¢ To deliver mouth-to-mouth breaths, do the following:
1.Open the airway using the head-tilt/chin-lift maneuver.

2. Pinch the personā€™s nose closed with your hand on the personā€™s head.

3. Create a seal when using your lips to surround the personā€™s mouth.

4. Blow into the personā€™s mouth for one full second and watch for chest to
rise. Tilt the victimā€™s head further back if the chest does not rise.
5. Give an additional breath for over one second.
6. If you cannot see the chest rise in two breaths, continue giving chest
compressions.

Use of Automated External Defibrillator (AED)
ā€¢ Ventricular fibrillation, caused by disorganized electrical activity in the
main pumping chambers of the heart, is a common cause of cardiac
arrest. The treatment for ventricular fibrillation is defibrillation or the
delivery of an electric shock to the heart through the personā€™s chest
wall. This shock attempts to stop the disorganized electrical activity
and allow the heartā€™s normal rhythm to resume.
ā€¢ The automated external defibrillator (AED) is a device that recognizes
ventricular fibrillation and other dysrhythmias and delivers an electric
shock at the right time. The AED has become a common sight in
public buildings. The AED is nearly foolproof and will not allow you
to make a mistake. It is safe for anyone to use. In a witnessed cardiac
arrest, where the person is observed to suddenly collapse, the most
common cause is likely to be ventricular fibrillation and a defibrillator
should analyze the personā€™s cardiac rhythm as soon as possible.
Use of Automated External Defibrillator (AED)
ā€¢ Using the team concept, one rescuer should coordinate all available
rescuers so that one rescuer performs chest compressions while the
second rescuer prepares the AED for use. Although there are many
different brands of AEDs, all are utilized in a similar way. Be sure to
move the person and yourself to a safe place before using the AED.
Electricity and water can be lethal when combined. Ensure that the
person is not wet (quickly wipe dry) or in close proximity to water
before using the AED. It is safe to use an AED if the person is lying in
the snow. If the person has an implanted device, such as a pacemaker,
you will see a bulge over their chest. Place the defibrillator pads as
close to the correct position as possible without being directly over the
device. For persons with medication patches, remove the patch, wipe
the skin dry, and apply the AED pad.

1. Retrieve the AED.
ā€¢ Open the case
ā€¢ Turn on the AED

ā€¢ If wet, dry chest.
ā€¢ Remove medication patches.

3. Open the AED pads.
ā€¢ Peel off backing.
ā€¢ Check for pacemaker or internal defibrillator

4. Apply the pads.
ā€¢ Apply one pad on the upper right chest above the breast.
ā€¢ Apply the second pad on the lower left chest below the armpit.

5. Ensure the wires are attached to the AED box.

6. Move away from the person.
ā€¢ Stop CPR.
ā€¢ Clear the person. Tell others not to touch the person.

7. Let AED analyze the rhythm.
8. If the AED message reads ā€œCheck Electrodes,ā€ then:
ā€¢ Ensure electrodes make good contact.
ā€¢ If the chest is hairy, pull off the pad and replace it.
9. If the AED message reads ā€œShock,ā€
ā€¢ Be sure the person is ā€œclearā€ by making sure no one is touching them.
ā€¢ Press and hold the ā€œshockā€ button until the AED delivers the shock.

10. Resume CPR for two minutes starting with chest compressions.
Repeat steps 1 to 10.

  • 3. ā€¢ According to the Centers for Disease Control (CDC), heart disease continues to be the leading cause of death in the United States and is responsible for over 600,000 deaths every year. ā€¢ Research continues to improve how we respond with life-saving techniques to emergencies. These techniques are based on the most current research and are organized into a systematic response called the Chain of Survival, which begins with Basic Life Support (BLS). ā€¢ The Chain of Survival provides the victim the best chance to receive the care needed and return to a healthy life. General Concepts of Basic Life Support ā€¢ The heart pumps blood through the lungs, where blood takes in oxygen and releases carbon dioxide. ā€¢ This blood then returns to the heart where it is pumped out to vital organsā€”the heart and brainā€”as well as the rest of the body. ā€¢ When the heart stops, blood flow stops, and the person quickly becomes unconscious. ā€¢ Without blood flow, the heart and the brain quickly become damaged due to a lack of oxygen. ā€¢ The actions that make up BLS try to prevent or slow the damage until the cause of the problem can be corrected. ā€¢ BLS improves a personā€™s chance of surviving until advanced care becomes available.
  • 5. ā€¢Taking the right action quickly and confidently can make the difference between life and death for a person dealing with cardiac arrest.
  • 6. Keys for BLS: ā€¢ Quickly start the Chain of Survival. ā€¢ Deliver high-quality chest compressions to circulate oxygen to the brain and vital organs. ā€¢ Know when and how to use an Automated External Defibrillator (AED). ā€¢ Provide rescue breathing. ā€¢ Understand how to work with other rescuers as part of a team. ā€¢ Know how to treat choking.

Initiating Chain of Survival ā€¢ Early initiation of BLS has been shown to increase the probability of survival for a person dealing with cardiac arrest. ā€¢ To increase the odds of surviving a cardiac event, the rescuer should follow the steps in the Adult Chain of Survival. ā€¢ Adult Chain of Survival
  • 8. 2020 BLS Guideline Changes ā€¢ Approximately every five years the International Liaison Committee on Resuscitation (ILCOR), updates the guidelines for CPR and ECC (Emergency Cardiac Care). ā€¢ The content contained herein is based on the most recent ILCOR publications on BLS. Recommendations for adult basic life support (BLS) from the 2020 Guidelines for CPR and ECC include the following: 1. The importance of early initiation of CPR by lay rescuers has been re- emphasized. The risk of harm to the patient is low if the patient is not in cardiac arrest. Bystanders should not be afraid to start CPR even if they are not sure whether the victim is breathing or in Cardiac Arrest. 2.   • 10. 2020 BLS Guideline Changes ā€¢ Updates in the ratio of rescue breath in pediatrics: ā€¢ The 2015 guidelines recommended traditional CPR cycles of 30 chest compressions to two rescue breaths for one-rescuer CPR in all age groups and for two-rescuer CPR in adults. ā€¢ The 15:2 ratio of compressions to breaths that was presented in the 2015 guidelines only for use in two-rescuer CPR for children and infants is now the recommended assisted ventilation rate for all pediatric resuscitation scenarios (rescue breathing or advanced airway). This will increase the respiratory rate to 20-30 breaths per minute by having 1 breath every 2 to 3 seconds.

BLS for Adults ā€¢ BLS for adults focuses on doing several tasks simultaneously. ā€¢ In previous versions of BLS, the focus was primarily on one-rescuer CPR. ā€¢ In many situations, more than one person is available to do CPR. ā€¢ This simultaneous and choreographed method includes performing chest compressions, managing the airway, delivering rescue breaths, and using the AED, all as a team. ā€¢ By coordinating efforts, a team of rescuers can save valuable seconds when time lost equals damage to the heart and brain.

  • 13. One Rescuer BLS/CPR for Adults ā€¢ Be Safe ā€¢ Make sure the scene is safe before proceeding. ā€¢ Move the person out of traffic. ā€¢ Move the person out of water and dry the person. (Drowning persons should be removed from the water and dried off; they should also be removed from standing water, such as puddles, pools, gutters, etc.) ā€¢ Be sure you do not become injured yourself. ā€¢ Assess the Person ā€¢ Shake the person, tap their shoulder hard, and talk to them loudly. ā€¢ Check to see if the person is breathing. (Agonal breathing, which is occasional gasping and is ineffective, does not count as breathing.)

In that case, the jaw-thrust is used. ā€¢ For the jaw-thrust maneuver, grasp the angles of the lower jaw and lift it with both hands, one on each side, moving the jaw forward. If their lips are closed, open the lower lip using your thumb.

Two-Rescuer BLS/CPR for Adults ā€¢ The second rescuer prepares the AED for use. ā€¢ You begin chest compressions and count the compressions out loud. ā€¢ The second rescuer applies the AED pads. ā€¢ The second rescuer opens the personā€™s airway and gives rescue breaths. ā€¢ Switch roles after every five cycles of compressions and breaths. One cycle consists of 30 compressions and two breaths.

If a shock is indicated, minimize interruptions in CPR. Resume CPR as soon as possible.

Adult Mouth-to-Mouth Ventilation ā€¢ ADULT MOUTH-TO-MOUTH VENTILATION ā€¢ When a pocket mask or bag-mask is not available, it may be necessary to give mouth-to-mouth breaths during CPR. ā€¢ Mouth-to-mouth breathing is very effective in delivering oxygen into the personā€™s lungs without putting the rescuer at a high level of risk. ā€¢ The rescuerā€™s exhaled air contains approximately 17% oxygen and 4% carbon dioxide. ā€¢ This is in contrast to the 100% oxygen available with ventilation with 100% high flow oxygen and the 21% oxygen that is available in room air that we breathe.

Use of Automated External Defibrillator (AED) ā€¢ Ventricular fibrillation, caused by disorganized electrical activity in the main pumping chambers of the heart, is a common cause of cardiac arrest. The treatment for ventricular fibrillation is defibrillation or the delivery of an electric shock to the heart through the personā€™s chest wall. This shock attempts to stop the disorganized electrical activity and allow the heartā€™s normal rhythm to resume. ā€¢ The automated external defibrillator (AED) is a device that recognizes ventricular fibrillation and other dysrhythmias and delivers an electric shock at the right time. The AED has become a common sight in public buildings. The AED is nearly foolproof and will not allow you to make a mistake. It is safe for anyone to use. In a witnessed cardiac arrest, where the person is observed to suddenly collapse, the most common cause is likely to be ventricular fibrillation and a defibrillator should analyze the personā€™s cardiac rhythm as soon as possible.

Use of Automated External Defibrillator (AED) ā€¢ Using the team concept, one rescuer should coordinate all available rescuers so that one rescuer performs chest compressions while the second rescuer prepares the AED for use. Although there are many different brands of AEDs, all are utilized in a similar way. Be sure to move the person and yourself to a safe place before using the AED. Electricity and water can be lethal when combined. Ensure that the person is not wet (quickly wipe dry) or in close proximity to water before using the AED. It is safe to use an AED if the person is lying in the snow. If the person has an implanted device, such as a pacemaker, you will see a bulge over their chest. Place the defibrillator pads as close to the correct position as possible without being directly over the device. For persons with medication patches, remove the patch, wipe the skin dry, and apply the AED pad.
