2. Patient profile
• Name -Siyaram
• Sex -male
• Age -30 yr old
• Time of admission -2.00pm, 17/4/2020

3. Complaints
• Fatigue
• Fever and dry tongue
• vomiting
• Itchy skin and pale skin
• yellow skin
• Joint pain
• Dark urine
• Tan colored stool (diarrhea)

4. History of patient illness
• General weakness began 1 week ago
• Intensity of headache increased since then
• Loss of sleep from last 5 days.
• Flu like symptoms prior to coming to hospital.

5. Personal history
• No history of previous hospitalization
• No history of antibiotics use from last 3 month
• Not any travel history
• Visited a new restaurant 2 week ago.
• No such symptoms in past
• No previous surgeries

6. Family History
• No history of severe disease
• No cancer
• No liver disease

7. Further examination
• Bilateral yellow discoloration of eye
• Distended abdomen with mild tenderness in right
upper quadrant .

8. Examination
• Palpation
• CBC
• Urine Test
• Liver Function Test
• Biochemical
• Serological(ELISA)
• Ultrasound

9. Ultrasound
• Liver size increased by 1.5 cm and has no mass .
Splenomegaly is also seen. No stones seend in
cystic duct. pancreas are also normal.

10. Lab findings
• prothrombin time -increased
• ALT - increased
• AST - increased
• Total Bilirubin - 90microM/l (increased)
• HBs antigen - negative
• HAV RNA - positive
• Anti HAV - positive(IgM predominance)
• Thymol turbidity test - mild increased

11. Diagnosis confirmed
• Acute Hepatitis A, typical form, icteric period, mild
severity, Dehydration degree 2

12. Treatment
• Bed rest
• Exclude heavy fats from the diet
• Loperamide to treat diarrhea
• Intravenous Detoxification therapy
0.9%NACL
• Hepatoprotectors
Heptral 3times a day

13. Prophylaxis
• Vaccine
• After exposure to virus , a single dose of vaccine
within 2 weeks of contact with the virus has
protective effects.
