This document summarizes a child's portfolio and interests. It describes activities the child engages in, such as drawing, writing her name, and playing with babies. The child has success being creative, writing her name with different materials, and focusing on activities. Her goal is to learn to say her name and the names of her friends. The teacher sets up activities to encourage the child to spell and say her name, such as exploring with play dough and shaving cream. After two weeks of practice, the child can now say the names of two friends.

9. Yanitzel’s Goal
Progress
My teacher team and I set a short
goal for Yanitzel. After we did the
ASQ, we found out that she can’t say
her name and she didn’t know her
peer's names.
11. Enjoying all the activities
Yanitzel was exploring painting with baking
soda and vinegar. This activity allowed her to
talk about what she has been seing and make
predictions. She said, teacher “fumay”
“bubuing”.
Yanitzel loves working on the computer.
Her favorite activity is making puzzles.
12. Yanitzel’s artwork
During exploration time, Yanitzel was
exploring with shaving cream, I asked
her “tell me about your painting” she
said “birds” and she got the color
“green” when she asked for yellow and
blue food coloring.
Then she decided to do another
one, but this time she asked for
red and yellow and she said
teacher “fumey” orange and she
did it using the “sign language”
During exploration time Yanitzel
was writing her name by using
playdough. Teacher encouraged
her to say each letter of her
name.
13. What are you doing to assist the child/family
with attaining this goal? How is the process
being communicated to the family?
When I let yanitzel’s mom know that I chose her
daughter for my CDA class. We planned about a short
goal. Yanitzel has an IEP and she will be starting
speech therapy next week. Our goal was to encourage
Yanitzel to say her name and learn some friend's
name. Mom asked me for all her friends names to
practicing with Yanitzel at home.
During circle time we use the transition to pick a
friend and we asked them “what is her/his name” if
they don’t know we encourage them to asked her/his
friend “what’s your name”.
After 2 weeks she knows and can pronounce perfectly
her two girls friend names “kendra” and she can say
clearly the name “Tony”. She also knows what are
her/his friends names letters.
