အပြာွားသတ်မှတ်ထာွားတသာ အြ က်မ ာွားနှငဲ့်ပပညဲ့်ေံုကှိုက်ညီတနပါက normal occlusion လှို ဲ့ သတ်မှတ်တလဲ့ရှှိပါ
တယ်။ Normal occlusion ရှှိတဲ့ လူနာတတွေအမ ာွားေုကှို နှုှိင်ွားယှဥ်တလဲ့လာမှုတတွေ ပပြုလုပ်ပပီွား ေံပပအပဖေ် ideal
occlusion သတ်မှတ်ြ က်တတွေကှို ထာွားရှှိပါတယ်။ တကယဲ့်လက်တတွေွံ့ မှာတတာဲ့ ideal occlusion ရှှိတဲ့လူနာတတွေဟာ
အတရအတွေက်နည်ွားပါလှိမဲ့်မယ်။
Normal occlusion
Ideal occlusion ဆှိုတဲ့ ေံပပြုအတနအထာွားကတန လက်ြံနှိုင်တဲ့တှိမ်ွားတောင်ွားမှုအနည်ွားငယ်သာရှှိတဲ့ occlusion ကှို
normal occlusion လှို ဲ့ တြေါ်ပါတယ်။ သွောွားတတွေရ ဲ့အလှအပကှိုတရာ၊ လုပ်ငန်ွားတဆာင်တာကှိုတရာ မထှိြှိုက်တေဘ
အနည်ွားငယ်တှိမ်ွားတောင်ွားမှုတလွား ရှှိတနနှိင်ပါတယ်။ Class I Molar relationship, Class I incisor relationship, No
rotation, No spacing, Flat or mildly increased (≤ 1.5 mm) curve of Spee ပဖေ်တနသငဲ့်ပါတယ်။

4. Malocclusion
ေံပပြုသတ်မှတ်ြ က်တတွေကတန သှိသှိသာသာ တှိမ်ွားတောင်ွားတနပပီွား သွောွားန ဲ့
တမွားရှိုွားတတွေရ ဲ့ အလှအပန ဲ့ လုပ်ငန်ွားကှို
ြျွတ်ယွေင်ွားတေတဲ့အတနအထာွားတေ်ြုကှို malocclusion လှို ဲ့သတ်မှတ်နှိုင်ပါတယ်။
Malocclusion ပဖေ်တှိုင်ွား သွောွားညှိကုသဖှို ဲ့ မဟုတ်ပါဘူွား။ လှိုြ င်မှ လှိုပါလှိမဲ့်မဲ့ယ်။ သွောွားညှိကုသလှိုက်ရင်
လူနာအတွေက် ပှိုလှလာတာပဖေ်ပဖေ်၊ ပှိုပပီွား ောွားလှို ဲ့
ဝါွားလှို ဲ့ တကာင်ွားလာတာပပဖေ်ပဖေ် အက ှိြုွားရှှိတယ်ဆှိုမှ
သွောွားညှိကုသပါ။ သွောွားညှိကုသမယဲ့်လူနာဟာ သွောွားသန ဲ့

Reasons for Orthodontic Treatment

မရှှိတတာဲ့ပါဘူွား။ ဒါ ဲ့
အပပင် သွောွားညှိလှိုက်တဲ့အတွေက် သွောွားနှငဲ့်ြံတွေင်ွား က န်ွားမာတရွားကှို ပှိုမှိုတကာင်ွားလာတေပါလှိမဲ့်မယ်။
သွောွားက ၊ သွောွားထပ်ပပီွား အောညပ်တနတာတတွေကှို ပပြုပပင်ပပီွား ပဖေ်သွောွားတေနှိုင်ပါတယ်။ တမွားရှိုွားအတနအထာွားမမှန်လှို ဲ့
တမွားရှိုွားအဆေ်န ဲ့ တမွားရှိုွားကကက်သာွားတတွေ နာတနတတ်တာမ ှိြုွားကှို အတှိုင်ွားအတာတေ်ြုအထှိ သက်သာသွောွားတေ
နှိုင်ပါတယ်။
မညီမညာတဲ့ သွောွားတတွေတကကာငဲ့် အတပေါ်တအာက်သွောွားတတွေကကာွားမှာ ထှိြှိုက်မှုပဖေ်တတ်ပါတယ်။ သွောွားတတွေ ညီသွောွားရင်
ဆက်လက်ပဖေ်တပေါ်လာနှိုင်တဲ့ ထှိြှိုက်မှုတတွေကှို ဖယ်ရှာွားပပီွား ပဖေ်သွောွားပါလှိမဲ့်မယ်။ သွောွားတြါတနတဲ့ လူနာတတွေဆှိုရင်
တရှွံ့သွောွားတတွေကှို မကကာြဏ ြှိုက်မှိတတ်ပါတယ်။ သွောွားညှိလှိုက်ရင် ထှိြှိုက်မှုပဖေ်တာ နည်ွားသွောွားနှိုင်ပါတယ်။
သွောွားတုတတွေ မတပ်ြင်မှာ မညီမညာပဖေ်တနတဲ့ သွောွားတတွေကှို ကကှိြုတင်သွောွားညှိကုသထာွားပြင်ွားအာွားပဖငဲ့် သွောွားတုေှိုက်
ရတာ ပှိုမှိုအဆင်တပပတေနှိုင်ပါတယ်။ သွောွားဖံုွားတရာဂါပဖေ်တနတဲ့ ထပ်တနတောင်ွားတနတဲ့ သွောွားတတွေကှို splinting

6. Orthodontic Treatment Options
၁။ No treatment - သွောွားတတွေက အနည်ွားငယ်ပ မညီမညာပဖေ်တနတဲ့အြါ သွောွားမညှိဘ တနြ င်ရင် တနလှို ဲ့
ရပါတယ်။
သွောွားညှိလှိုက်ရင် long-term stability and retention ကှိေစကှို ထညဲ့်ေဥ်ွားောွားရပါမယ်။ တကာင်ွားက ှိြုွားန ဲ့ဆှိုွားက ှိြုွားကှို
ြ ှိန်ွားထှိုွားကကညဲ့်ရပါမယ်။ ၂။ Extraction only - တေ်ြ ှိြုွံ့လူနာတတွေမှာ ထပ်တနတဲ့ သွောွားတတွေကှို နှုတ်တပွားလှိုက်ရံုန ဲ့
က န်သွောွားတတွေရ ဲ့ အတနအထာွားကှို အတှိုင်ွားအတာတေ်ြုအထှိ ပှိုမှိုတကာင်ွားသွောွားတေနှိုင်ပါတယ်။ အထူွားသပဖငဲ့်

appliance ကှို အသံုွားပပြုပပီွား ကုသနှိုင်ပါတယ်။ ဥပမာ - simple tipping movements of few teeth; to tip upper
incisor forward in anterior cross-bite
၄။ Fixed appliance - သွောွားတတွေ မညီမညာပဖေ်လွေန်ွားတနပပီွား three planes (antero-posterior, vertical and
lateral)မှာ လှိုအပ်သလှို တရွေွံ့တေဖှို ဲ့
အတွေက် fixed appliance ကှို အသံုွားပပြုနှိုင်ပါတယ်။
၅။ Functional appliances - တမွားရှိုွားက ပံုမှန်မဟုတ်ဘ အတှိုအရှည်မညီတဲ့ growing patients လူနာတတွေမှာ
အသံုွားပပြုနှိုင်ပါတယ်။
၆။ Orthognathic surgery - တမွားရှိုွားက ရှိုွားရှိုွားသွောွားညှိကုသရံုန ဲ့ လှိုြ င်တဲ့အတနအထာွားမတရာက်နှိုင်ဘူွားဆှိုရင်
ြွေေှိတ်ကုသပပီွား ပပြုပပင်နှိုင်ပါတယ်။ ြွေေှိတ်မကုသြင်မှာ သွောွားညှိကုသမှုပပြုလုပ်တာ၊ ြွေေှိတ်ကုသပပီွားတနာက်

9. The Timing of Orthodontic Treatment
The deciduous dentition - ဒီအဆငဲ့်မှာ သွောွားညှိကုသမှုကှို ပပြုလုပ်ြပါတယ်။ သွောွားအပှိုသွောွားညပ်တကကာငဲ့်
သွောွားမညီမညာပဖေ်တနရင်ပဖေ်တေ၊ malposition tooth or premature contact တကကာငဲ့် mandibular displacement
leading to unilateral posterior crossbite ပဖေ်တနရင်ပဖေ်တေ လှိုအပ်တဲ့ကုသမှုတတွေကှို ပပြုလုပ်တပွားနှိုင်ပါတယ်။
The early mixed dentition - ဒီအဆငဲ့်မှာ developing malocclusion ပဖေ်တနတာတတွေရှှိြဲ့ရင် ကကှိြုတင်ကုသမှု
ပပြုလုပ်တပွားနှိုင်ပါတယ်။ ဥပမာ - planned extraction of intensively carious first permanent molars (8.5 – 10
years old); balancing extractions of deciduous teeth; serial extractions, space maintainers may be fitted;
Simple orthodontic treatment to correct in-standing incisor or to eliminate a mandibular displacement;
reverse headgear to correct retrognathic maxilla in Skeletal Class III

The late mixed and early permanent dentition (10-13 years old) - ဒီအရွေယ်မှာ သွောွားညှိကုသမှုကှို အမ ာွားဆံုွား
ပပြုလုပ်ကကပါတယ်။ တမွားရှိုွားက ဥ်ွားတနရင် တမွားရှိုွားကှို ြ ွံ့တပွားပြင်ွားအာွားပဖငဲ့် သွောွားထပ်ပြင်ွားကှို ကုသနှိုင်ပါတယ်။
တမွားရှိုွားကကီွားထွောွားတနဆဲ့အရွေယ်ပဖေ်တာတကကာငဲ့် ဒီအဆငဲ့်မှာ လှိုအပ်ရင် functional appliance ကှို တပ်ဆင်ပါတယ်။
The late permanent dentition (>16 years old) - ဒီအဆငဲ့်မှာ သွောွားညှိကုသတဲ့အြါ လူနာတေ်ဦွားြ င်ွားေီရ ဲ့
အတနအထာွားအရ treatment planning and mechanics ကှို လှိုအပ်သလှို မွေမ်ွားမံတရွားဆွေပပီွား ကုသမှု တပွားရပါတယ်။
Adult patients တတွေကှို ကုသမှုတပွားရတာက growing patients တတွေကှို ကုသရတာထက် အြ ှိန်ပှိုကကာပါလှိမဲ့်မယ်။
ကကီွားထွောွားမှုရပ်သွောွားတဲ့လူနာတတွေမှာ severe skeletal discrepancy ပဖေ်တနရင် ြွေေှိတ်ကုသတပွားနှိုင်ပါတယ်။

10. Development of Normal Occlusion (by Nay Aung, BDS, PhD)
Introduction
ကလလေးတစ်ဥေး လမေးဖာေးလာမယ်။ အမ ာေးအာေးဖဖင် အသက်(၆)လလလာက်မာ deciduous dentition ရ
ငယ်သာေးတစ်ခ ျို့ စလ ေါက်လာမယ်။ ကလလေးရအသက် (၆)နစ်အရယ်မာဆိုရင် ကကေးသာေးတစ်ခ ျို့ စတင်ထက်လာ
နိုင်ပ ေး mixed dentition ဖဖစ်လာ ေါလမ်မယ်။ အသက် (၁၂) နစ်လ ာက် ိုင်ေးကို လရာက်တအခေါ
ငယ်သာေးလတလ ရာမာ ကကေးသာေးလတက အစာေးထိုေးသာေးပ ေးဖဖစ်လို permanent dentition ဖဖစ်သာေး ေါလမ်မယ်။
လူတစ်ဥေးရ occlusal development ကို အဆင်(၅)ဆင် ိုင်ေးဖခာေးကကည်မယ်ဆိုရင် အဆင်(၁) Birth to
establishment of deciduous dentition အဆင်(၂) Deciduous dentition to early mixed dentition အဆင်(၃)
Early mixed dentition to late mixed dentition အဆင်(၄) Late mixed dentition to permanent dentition
အဆင်(၅) Permanent dentition ဆိုပ ေး ိုင်ေးဖခာေးကကည်နိုင် ေါတယ်။
Stage 1 Birth to the establishment of deciduous dentition
ကလလေးလမေးဖာေးလာပ ေးလ ာက် maxilla mandible ကို ကကည်လိုက်ရင် ငယ်သာေးလလေးလတ လ ေါက်လာမယ်
လ ရာမာ သာေးဖိုေးသာေးလလေးလတ ဖိုလ တတ် ေါတယ်။ အဒဖိုလ တ gum pads လလေးလတ အခို(၂၀)ကို လ ေါက်လာမယ်
ငယ်သာေးအလခ ာင်ေး(၂၀)အတက် လတျို့ရ ေါလမ်မယ်။ maxilla ရ ိုသဏ္ဍာ ်က horseshoe-shaped
ဖမင်ေးခာ ိုစဖဖစ်ပ ေး palatal vault က very shallow တမ်လ ေါလမ်မယ်။ mandible ရ ိုသဏ္ဍာ ်က U-shaped
အင်္ဂလ ်အကခရာ U ိုစ ဖဖစ်လ ပ ေး လရျို့ ိုင်ေးရ gum pads လလေးလတက slightly everted labially ဖဖစ်လ ေါလမ်မယ်။
Mandible ကို physiological rest position မာ ထာေးကကည်လိုက်ရင် လ ာက maxilla mandible မာရတ gum
pads လတကို ဖခာေးလ ေးထာေးပ ေး နှုတ်ခမ်ေးလတကို လရျို့ဘက်သို လရာက်လ လစ ေါတယ်။ Occlusal relationship of the

11. teeth အရ ိုမာ ဖ ထာေးတအတိုင်ေး လမေးရိုေးလတကို overlapping လို ်ကကည်ရင် Maxilla မာရတ gum pad
လလေးလတက mandible မာ ရတ gum pad လလေးလတရ buccally and labially ဘက်မာ ရလ ေါလမ်မယ်။
The deciduous dentition
ကလလေးရအသက် (၆)လလလာက်မာ lower incisors လလေးလတ စလ ေါက်လာ ေါလမ်မယ်။ သာေးလ ေါက်တအခ ်ဟာ
ိုမ ်ထက် (၆)လခ ် လစာလ ေါက်တာ၊ (၆)လခ ် လ ာက်က လ ေါက်တာလတ ဖဖစ်တတ် ေါတယ်။ အမ ာေးအာေးဖဖင်
ကလလေးအသက် (၂)နစ်ခခ ်မာ ငယ်သာေးလတအာေးလိုေး လ ေါက်လာ ေါလမ်မယ်။ ငယ်သာေး incisors လတဟာ
လ ာင်တစ်ခ ်မာ အစာေးထိုေးလ ေါက်လာမယ် ကကေးသာေး incisors လလေးလတထက် ိုမိုလထာင်မတ်လ တတ်တာမို
ငယ်သာေးလတကကာေးမာ spacing လလေးလတ ရလ တတ် ေါတယ်။ Spacing လလေးလတ မရဘူေးဆိုရင် ကကေးသာေးလတ
လ ေါက်လာတတ်တအခေါ သာေးထ ်တာလတ ဖဖစ်လာနိုင် ေါတယ်။
ငယ်သာေး lower canines လတရ distal ဘက်မာ upper canines လတရ mesial ဘက်မာ spacing ရနိုင်ပ ေး အဒေါကို
primate spacing လို လခေါ် ေါတယ်။

ဥ မာ - Utilization of existing spacing between the deciduous incisors; A growth increase in inter-canine
width; The upper permanent incisors are more proclined and thus form a larger arch than the deciduous
incisors.

ကလလေးအသက် ၈-၉နစ်အရွေယ်မော lower permanent incisors လလေးလတွေဟော lingually positioned, mildly

Lip competency
A. Competent lips: a lip seal is produced with minimal muscular effort when the mandible is at
rest position
B. Potentially competent: positioning of the upper incisors prevents a comfortable lip seal
C. Incompetent: excessive muscular activity is required to achieve lip seal [the interlabial distance
at rest is >4mm]
Signs of excessive muscular activity
Flattening of the labio mental fold when the lips are held together
