WHAT IS TETRALOGY OF FALLOT ?
 Tetralogy of Fallot (TOF) is a rare condition caused by the
combinations of four heart defects that are present at birth.
 These defects, affect the structure of the heart, causes oxygen-
poor blood flow out of the heart and to the rest of the body.
 Infants and children with Tetralogy of Fallot usually have
blue-tinged skin because their blood doesn't carry enough
oxygen.

The four abnormalities that make up the Tetralogy of Fallot

 Pulmonary stenosis: It is when the pulmonary valve (the
valve between the right ventricle and the pulmonary artery)
is too small, narrow or stiff. It is also called pulmonic
stenosis.
 Ventricular Septal defect: It is an abnormal opening in the
heart or hole between the two ventricles. This allows
oxygen-rich and oxygen-poor blood to mix.
 Right Ventricular Hypertrophy: It is an abnormal
enlargement or the thickening of the right ventricular
muscle.
 Overriding Aorta: When the aorta is positioned directly
over a ventricular septal defect instead of over the left
ventricle, in this aorta gets blood from the right ventricle
and causes mixing of oxygenated and deoxygenated blood.

TETRALOGY OF FALLOT SYMPTOMS
 A bluish color skin.
 Clubbed fingers, or growth of skin
and bone around the fingernails.
 Problems with eating.
 Heart murmur.
 Weakness.
 Prolonged crying.
 Failure to gain weight.
 Developmental delays or problems.
 Episodes of passing out.
 Tiring easily.

RISK FACTORS FOR TETRALOGY OF FALLOT
The following factors, during pregnancy, can result in a baby having Tetralogy of
Fallot:
 Consumption of excessive alcohol when pregnant.

TEST AND DIAGNOSIS FOR TETRALOGY OF FALLOT
ECG: Electrocardiogram is a test to detect and record heart’s
electrical activity and help to locate the heart problem.
Echocardiography: An electrocardiogram records the
electrical activity in the heart each time it contracts.
Chest X-ray: It show the structure of the heart and lungs. A
common sign of tetralogy of Fallot on an X-ray is a "boot-
shaped" heart, because the right ventricle is enlarged.
Pulse Oximetry: This test uses a small sensor that can be
placed on a finger or toe to measure the amount of oxygen in
the blood. Also called Oxygen level measurement.
Cardiac Catheterization: this test to evaluate the structure
of the heart and then plan surgical treatment.

TREATMENT FOR TETRALOGY OF FALLOT
Temporary Operation
• In some infants, a shunt operation may be done first to
provide adequate blood flow to the lungs. This is not open-
heart surgery and doesn't fix the inside of the heart. The
shunt is usually a small tube of synthetic material sewn
between a body artery and the pulmonary artery. The shunt
is closed when a complete repair is done later.
Complete repair
• Complete repair is to be done early in life. The surgeon
closes the ventricular septal defect with a patch and opens
the right ventricular outflow by removing some thickened
muscle below the pulmonary valve. Sometimes a tube is
placed between the right ventricle and the pulmonary
artery. This is called a Rastelli repair.
Tetralogy of Fallot is treated surgically. A temporary operation may be done at first if the baby is small or if
there are other problems. Complete repair comes later. Sometimes the first operation is a complete repair.

PRECAUTIONS AFTER SURGERY
 Pain medications, such as ibuprofen, may be
recommended to keep the child comfortable at home.
 Child's physician will discuss pain control before the child

 After surgery, older children usually have a fair tolerance
for activity.
 Child may become tired easily, and sleep more right after
surgery, but, within a few weeks, child should be fully
recovered.
