Pharmaceutical Industry:
 A pharmaceutical industry, or drug manufacturing, is a commercial business that
focuses on research, develop, market and/or distribute drugs, most commonly in
the field of healthcare.
 They can deal in generic and/or brand medications (innovator). They are subject
to a variety of laws and regulations regarding the patenting, testing and
marketing of drugs.
 The pharmaceutical industry is now one of the most successful and influential
industries.
 In 2006, global spending on prescription drugs was $643 billion.
 Pfizer's cholesterol pill “Lipitor ®” remains the best-selling drug in the world for
the fifth year in a row. Its annual sales were $12.9 billion.
History of G.M.P.
 What is G.M.P.?
 G.M.P. or Good Manufacturing Practice (also referred to as
“cGMP” or “current Good Manufacturing Practice”)
They are the guidelines for the control and management of
manufacturing and testing of foods and pharmaceutical products.
GMP resulted from a long history of the need for consumer
protection.
 History of G.M.P.
 1900s
1902: 12 children died of tetanus. (diphtheria antitoxin contaminated with live tetanus bacilli)
Congress responded by passing the Biologics control Act of 1902: Inspections and testing of
biologics manufacturers’.
1905: The Jungle
A story catalyzed public opinion for change. Sinclair wrote about the Chicago meat packing
industry.
1906: Pure Food and Drug Act
Creates one of the first government regulatory agencies (now known as FDA).
FDA: Food and Drug Administration
For the first time:
 Illegal to sell contaminated (adulterated) food or meat
 Labelling had to be truthful
 1935: Sulfa drugs were introduced
 One company used Diethylene glycol (a poisonous solvent & chemical analog of
anti-freeze) in an oral elixir of sulfanilamide
 Before the problem was discovered, 107 people died, many of them children.
 1940s
 1941: Nearly 300 deaths and injuries from distribution of Sulfathiazole*
tablets cross contaminated with Phenobarbital*.
 Sulfathiazole: A short-acting sulfa drug, used as topical antimicrobial.
 Phenobarbital: Anticonvulsant.
 FDA revises manufacturing and quality controls rules for the beginning of what
will later be called GMPs.
 1955: Jonas Salk discovered a way to vaccinate against polio
 Many manufacturers began making his poliovaccine
 One company failed to inactivate the virus completely
 About 60 individuals developed polio
 Another 89 of their family members contracted polio from them
 1960s
1962: Thalidomide (used to decrease vomiting of pregnancy ”morning sickness”) caused birth defects in
thousands of European babies (Phocomelia).
 Manufacturers must prove efficacy of products before marketing them and ensure stricter control over
drug testing.
1963 GMPs for Drugs
 Good manufacturing practices for manufacturing, processing, packing, or holding finished
pharmaceuticals were first published.
 1970s
1975: cGMPs for Blood and Blood Components Final Rule: Establishes minimum current good
manufacturing practices for blood establishments in the collecting, processing, compatibility testing,
storing, and distributing of blood and blood components.
1976: Medical Device Amendments
 Intra Uterine Devices seriously injures many patients.
 New law strengthens FDA authority to oversee medical devices.
1978: cGMPs for Drugs and Devices (21 CFR* 210–211 and 820)
A major rewrite for the drug GMPs and GMPs for medical devices were
published.
 These regulations establish minimum cGMP regulations for manufacturing,
processing, packing, or holding drug products and medical devices.
1979: GLP*s (21 CFR 58) Final Rule
 Establishes good laboratory practices for conducting nonclinical laboratory
studies that support applications for research or marketing permits for: human and
animal drugs, medical devices for human use, and biological products.
 CFR: Code of Federal Regulations
 GLP: Good Laboratory Practice
 Packaging Regulations1980s:
 1980: Infant Formula Act
 100 children reported seriously ill linked by a lack of chloride in two soy-based
formulas.
 Congress gives FDA authority to set and enforce nutritional and quality
standards.
 OTC* Products
 Acetaminophen-capsule poisoning by cyanide causes 7 deaths.
 Revision of GMPs to require tamper-resistant packaging
OTC: Over The Counter
 1982: Tamper – Resistant (Poisoned acetaminophen capsules)
 31 million bottles of Tylenol were recalled
1983:
 The “Guide to the Inspection of Computerized Systems in Drug Processing” initiates tighter
controls on computers and computer validation.
 Federal Anti-Tampering Act makes it a federal crime to tamper with packaged consumer products.
1987: Guideline on General Principles of Process Validation*
 Agency expectations regarding the need for process validation are outlined.
Validation: The actions taken to make confidence that the product will consistently meet
predetermined specifications and attributes.
 1990s:
 1990: Safe Medical Devices Act
 Some types of heart valves caused serious problems to patients.
 FDA given authority to cGMP to add pre – production design controls and tracking of critical or
implantable devices.
 1996
 Proposed revision to U.S. cGMP for Drugs and Biologics
 (21 CFR 210–211) adds detail for validation, blend uniformity, prevention of cross
– contamination, and handling out of specification results (OOS).
 “ICH (International conference for Harmonization) Guidance for Industry: E6,
Good Clinical Practice: Consolidated Guidance” becomes the standard for
conducting human clinical trials.
 1997 cGMP for Medical Devices (Quality System Regulation) Final Rule
 Major revision to current good manufacturing practices for medical devices.
 1997 Electronic Records Final Rule
 (21 CFR 11) Requires controls that ensure security and integrity of all electronic
data.
 1998 Draft Guidances
 “Manufacturing, Processing, or Holding Active Pharmaceutical
Ingredients (API)” and “Investigating Out of Specification (OOS) Test
Results for Pharmaceutical Production.”
 2001 ICH Q7AAPI Guidance
 ICH’s “Good Manufacturing Practice Guidance for Active Pharmaceutical
Ingredients (APIs)”: is adopted by the United States, Europe, and Japan,
and becomes the manufacturing standard for APIs.
2002 Drug Manufacturing Inspections Compliance Manual
 New FDA technique for routine drug manufacturing inspections focuses on two or
more systems, with mandatory coverage of the quality system.
 Other systems are: facilities and equipment, materials, production, packaging and
labeling, and laboratory controls.

Pharmaceutical Industry:  A pharmaceutical industry, or drug manufacturing, is a commercial business that focuses on research, develop, market and/or distribute drugs, most commonly in the field of healthcare.  They can deal in generic and/or brand medications (innovator). They are subject to a variety of laws and regulations regarding the patenting, testing and marketing of drugs.  The pharmaceutical industry is now one of the most successful and influential industries.  In 2006, global spending on prescription drugs was $643 billion.  Pfizer's cholesterol pill “Lipitor ®” remains the best-selling drug in the world for the fifth year in a row. Its annual sales were $12.9 billion.
  • 4. History of G.M.P.  What is G.M.P.?  G.M.P. or Good Manufacturing Practice (also referred to as “cGMP” or “current Good Manufacturing Practice”) They are the guidelines for the control and management of manufacturing and testing of foods and pharmaceutical products. GMP resulted from a long history of the need for consumer protection.
  • 5.  History of G.M.P.  1900s 1902: 12 children died of tetanus. (diphtheria antitoxin contaminated with live tetanus bacilli) Congress responded by passing the Biologics control Act of 1902: Inspections and testing of biologics manufacturers’. 1905: The Jungle A story catalyzed public opinion for change. Sinclair wrote about the Chicago meat packing industry. 1906: Pure Food and Drug Act Creates one of the first government regulatory agencies (now known as FDA). FDA: Food and Drug Administration For the first time:  Illegal to sell contaminated (adulterated) food or meat  Labelling had to be truthful
  • 6.  1935: Sulfa drugs were introduced  One company used Diethylene glycol (a poisonous solvent & chemical analog of anti-freeze) in an oral elixir of sulfanilamide  Before the problem was discovered, 107 people died, many of them children.  1940s  1941: Nearly 300 deaths and injuries from distribution of Sulfathiazole* tablets cross contaminated with Phenobarbital*.  Sulfathiazole: A short-acting sulfa drug, used as topical antimicrobial.  Phenobarbital: Anticonvulsant.  FDA revises manufacturing and quality controls rules for the beginning of what will later be called GMPs.
  • 7.  1955: Jonas Salk discovered a way to vaccinate against polio  Many manufacturers began making his poliovaccine  One company failed to inactivate the virus completely  About 60 individuals developed polio  Another 89 of their family members contracted polio from them
  • 8.  1960s 1962: Thalidomide (used to decrease vomiting of pregnancy ”morning sickness”) caused birth defects in thousands of European babies (Phocomelia).  Manufacturers must prove efficacy of products before marketing them and ensure stricter control over drug testing. 1963 GMPs for Drugs  Good manufacturing practices for manufacturing, processing, packing, or holding finished pharmaceuticals were first published.  1970s 1975: cGMPs for Blood and Blood Components Final Rule: Establishes minimum current good manufacturing practices for blood establishments in the collecting, processing, compatibility testing, storing, and distributing of blood and blood components. 1976: Medical Device Amendments  Intra Uterine Devices seriously injures many patients.  New law strengthens FDA authority to oversee medical devices.
  • 9. 1978: cGMPs for Drugs and Devices (21 CFR* 210–211 and 820) A major rewrite for the drug GMPs and GMPs for medical devices were published.  These regulations establish minimum cGMP regulations for manufacturing, processing, packing, or holding drug products and medical devices. 1979: GLP*s (21 CFR 58) Final Rule  Establishes good laboratory practices for conducting nonclinical laboratory studies that support applications for research or marketing permits for: human and animal drugs, medical devices for human use, and biological products.  CFR: Code of Federal Regulations  GLP: Good Laboratory Practice
  • 10.  Packaging Regulations1980s:  1980: Infant Formula Act  100 children reported seriously ill linked by a lack of chloride in two soy-based formulas.  Congress gives FDA authority to set and enforce nutritional and quality standards.  OTC* Products  Acetaminophen-capsule poisoning by cyanide causes 7 deaths.  Revision of GMPs to require tamper-resistant packaging OTC: Over The Counter  1982: Tamper – Resistant (Poisoned acetaminophen capsules)  31 million bottles of Tylenol were recalled
  • 11. 1983:  The “Guide to the Inspection of Computerized Systems in Drug Processing” initiates tighter controls on computers and computer validation.  Federal Anti-Tampering Act makes it a federal crime to tamper with packaged consumer products. 1987: Guideline on General Principles of Process Validation*  Agency expectations regarding the need for process validation are outlined. Validation: The actions taken to make confidence that the product will consistently meet predetermined specifications and attributes.  1990s:  1990: Safe Medical Devices Act  Some types of heart valves caused serious problems to patients.  FDA given authority to cGMP to add pre – production design controls and tracking of critical or implantable devices.
  • 12.  1996  Proposed revision to U.S. cGMP for Drugs and Biologics  (21 CFR 210–211) adds detail for validation, blend uniformity, prevention of cross – contamination, and handling out of specification results (OOS).  “ICH (International conference for Harmonization) Guidance for Industry: E6, Good Clinical Practice: Consolidated Guidance” becomes the standard for conducting human clinical trials.  1997 cGMP for Medical Devices (Quality System Regulation) Final Rule  Major revision to current good manufacturing practices for medical devices.  1997 Electronic Records Final Rule  (21 CFR 11) Requires controls that ensure security and integrity of all electronic data.
  • 13.  1998 Draft Guidances  “Manufacturing, Processing, or Holding Active Pharmaceutical Ingredients (API)” and “Investigating Out of Specification (OOS) Test Results for Pharmaceutical Production.”  2001 ICH Q7AAPI Guidance  ICH’s “Good Manufacturing Practice Guidance for Active Pharmaceutical Ingredients (APIs)”: is adopted by the United States, Europe, and Japan, and becomes the manufacturing standard for APIs.
  • 14. 2002 Drug Manufacturing Inspections Compliance Manual  New FDA technique for routine drug manufacturing inspections focuses on two or more systems, with mandatory coverage of the quality system.  Other systems are: facilities and equipment, materials, production, packaging and labeling, and laboratory controls.
