CFGS, 25-33 Bow Rd, London E3 2AE Mile End Park, Clinton Rd, London E3 4QY St. Stephen’s Road, London E3 5JL Dates / times: n/a Hazards2 List what could cause harm f rom this activ ity e.g. working at height, trip hazard, fire, etc. Who exposed List who might be harmed f rom this activ ity e.g. staff, contractors, contributors, public, etc. Risk3 For each hazard, decide lev el of risk as if y ou were to do the activ ity without y our controls Control measures For each hazard, list the measures y ou will be taking to minimise the risk identif ied e.g. appointing competent persons, training received, planning and rehearsals, use of personal protective equipment, provision of first aid, etc. Risk3 For each hazard, now decide lev el of risk once all y our controls are in place Cold Environment Cast & Crew LOW  Encourage all to bring w arm clothing and lots of layers  Put reminder in Location Notes on all Call Sheets  Ensure regular breaks are taken in a w armenvironment  Ensure hot drinks are available on location at all times VERY LOW Manual Handling Crew & Cast MEDIUM  Crew are trained how to safely lift and handle equipment and other items  Only those w ho are trained how to handle safely w illbe lifting things  Plan routes before moving objects  Equipment w illbe dropped as near to w here it needs to be placed as possible  Plan breaks w here objects can be put dow n  Team lifts w illbe encouraged for larger, bulky items VERY LOW Object Falling, Moving or Flying All LOW  Make sure all objects that are at height are secured to the rigging or w eighted down at the base  Make sure all crew and actors know the set and w hat is at height  Ensure that the vicinity surrounding the moving/flying object is clear VERY LOW Slippery Surface Cast & Crew MEDIUM Due to adverse w eather, Slippery Surface may become a risk. To limit this, w e w ill:  Avoid, w here possible, filming in particularly slippery areas  Encourage sensible, practical footw ear  Ensure sand/grit/sawdust is put dow n where necessary  Wipe or mop up any liquid spillages as soon as possible and notify other members of cast and crew VERY LOW Trip Hazard Crew , Cast & Public MEDIUM  Crew w illkeep cabling to a minimum w here possible  Kit w illbe stored neatly and w alkways willbe kept clear  Crew w illkeep cables tidy & secure any loose cabling  Cables laid across walkways, if unavoidable, to be covered w ith cables mats or securely taped dow n  Letters to residents have been VERY LOW

By Mariam Alim distributed to inform them of our intentions at each location w ith our contact details on  Locations w illensure public can get through the set in betw een takes and w illbe able to pass w ithout obstruction Vehicle Impact or Collision All MEDIUM  Ensure people do no w orktoo closely to the road and w hen w orking by the road have someone monitoring traffic forriskreduction  Wear high- visibility jackets w hen filming outside LOW Electric Shock Crew & Cast MEDIUM  Only equipment PAT Tested w illbe used  Ensure location of fuse boards is know n for each location  Check to make sure that sockets are not overloaded and equipment turned off w hen not in use  Those involved should do a visual checkbefore using any electrical devices  If anything is unfit for use, it w illnot be used further on the shoot until it repaired and re-tested LOW Fire Crew & Cast LOW  When setting up shots, the relevant departments w ill ensure electrical equipment is given adequate space to keep cool and not overheat  Strictly no smoking  Ensure a Fire Marshall is present, and as such, w illtake preventative steps w hilst on location  In the event of a fire, area to be left immediately and all cast and crew to assemble in pre-arranged safe location  Cast and crew to familiarise themselves w ith locations of fire extinguishers/blankets and locations of fire exits  Ensure that fire escape routes are not blocked VERY LOW Working in Confined Space Cast & Crew LOW  Cast and Crew to blockthrough moves  Constant communication to be kept up betw een everyone, so that everyone know swhat everyoneelse is doing and planning  Only necessary personneland equipment to be on location VERY LOW Noise Crew VERY LOW  Ensure noise levels monitored  If needed, w ear extraneousnoise cancelling headphones VERY LOW Lack of Food/Water Cast & Crew LOW  We w illensure the cast and crew have w ater and or hot drinks available as much as possible, to limit the possibility of dehydration or lack of food  Everyone involved w illtake meal breaks w herever possible to limit risk of lackof food/w ater VERY LOW Physical Fatigue Crew & Cast MEDIUM  All involved w illbe given as many breaks as time w illallow  The w orking day w illNOT exceed 6 LOW
