Austin conducts the “Basic Fire Protection” training to the students of a Vocational High School
which is located near to Austin’s premises.

• On Wednesday 09 January 2019, at 06:50 Mr. Martin Ichsan (QC Manager, the IP) fell
from the H-beam with his left hand hitting the floor.
• The incident could have not occurred if the IP did not take risk to use the H-beam
supports as an access.
16. 7. Learnings from LTI / HPI / significant incidents – last 12 months
• On Wednesday 10 April 2019, two Mechanical Technicians were about to carry out six-monthly
maintenance of overhead cranes using scissor lift as the means of. The technicians attempted
to move the overhead crane to the front end of the workshop, its hook stroke the handrail of
the scissor lift causing the scissor lift toppled.
• The incident could have not occurred if the scissor lift was lowered when the technicians
arrived at the platform of the overhead crane.
17. 7. Learnings from LTI / HPI / significant incidents – last 12 months
• On Thursday, 27th of June 2019, the IP (Painting and Blasting Operator) was vacuum cleaning
the steel shots on the floor in front of the lip mount at the rear end of the blasting chamber. At
04:10 hrs the lip mount fell from the two cross beams incurring a crush injury to the IP. He was
taken to the Soedharsono Dharmosoewito hospital to obtain medical treatment and passed
away as result of his injury.

• Every Austin Employee should adhere
to Austin’s OH&S Policy, Standard,
Golden Rules, Procedures and Safe
Work Instructions.
• The challenge that my HSE Officers
and I struggle is instilling Austin’s
OH&S culture among the new hired
employees. They come from various
companies of different safety cultures.
Most of them are not familiar with
Austin’s practices –for example-
completing Take Five. While many
other new employees whose discipline
are poor due to previous job’s habits.
