A spot report is a written, narrative report that informs the higher office about
the accident / incident that happened.
It will answer the following questions:
What accident / incident took place? (Type of Accident/Incident)
What Date and Time it happened?
Where did it happened? (Location)
Who or What is involved? (person/equipment/environment)
What is the Scenario during the time of accident? (a Short story)
Who are the witnesses? (If applicable)
GENERAL RULES:
1. Provide one (1) copy of this report to the Head Office within 24 hours after the incident.
2. A detailed accident/incident report shall follow after the investigation within 72 hours.
3. Indicate time & date of accident, name of person or property involved, and narrate the
incident.

A rule of thumb in case of an accident / incident:
Frasec Ventures Corporation requires all employees to immediately report
all accidents and incidents resulting to an injury or property damage, and all near
misses with the potential for serious injury or property damage. This report shall be
informed to their supervisor, immediate superior, safety inspectors for further action
or emergency response as needed.
Each incident shall undergo a thorough analysis to determine causes and
contributing factors and the analysis will be used to reduce or eliminate the risk of
future incidents.
As a deputized safety inspector, it is your duty to comply the required
accident/accident report in order to document such event. This report will serve as a
guide for the management to review such accident/incident in order to develop
solutions, revise procedures, create a policy for the purpose of preventing the
occurrence of such accident/incident.

What should you report and investigate in case an
accident/incident occurs?
FVC requires that injuries and certain categories of accidents
must be reported. Such report should specify the following:
what is to be reported
to whom it will be reported
how it is reported
which incidents are investigated
who will investigate them
what forms are used
what training investigators will receive
what records are to be kept
what summaries and statistics are to be developed
how often reports are prepared

Accidents and incidents are investigated so that
corrective actions will be provided for the purpose of
preventing a recurrence of a similar event. Investigation
represents an "after-the-fact" reaction for any particular
mishap.
However, under a thorough investigation conducted
by a team of inspectors & safety committee members, we
may uncover hazards, potential risks or problems that can
be eliminated "before-the-fact" for the future.
After identifying the causes, a prompt follow-up
action is required to achieve or complete the purpose of the
investigation.
