We are pleased to offer you the Trek Excellence Scholarship for Continuing Students in the amount of $750.00 for the
2016 Winter session.
Your Scholarship is scheduled to be paid to your tuition and other student fees* as follows (actual payment date
depends on when the award is accepted):
$750.00 will be paid on October 13, 2016
*Your registration deposit is not covered by this Scholarship.
If you have not signed up for direct deposit, please go HERE for instructions.
Any remaining funds will be returned to you by direct deposit or cheque. An 'Award Refund' will appear on your SSC
account, under Financial Summary. We will email you when your direct deposit is processed or when your cheque is
available for pickup.
Please ensure that your address and Social Insurance Number (SIN) or Individual Tax Number (ITN) are correct and
updated on the SSC. We need this information to process your T4A tax form.
This Scholarship is being provided through University of British Columbia funds.

2. LEGAL AGREEMENT AND TERMS AND CONDITIONS
I understand that awards are available in accordance with policy and regulations approved by the University Senate
and the Board of Governors.
Further, I understand that if my circumstances change during the academic term such that I am no longer eligible for
this Scholarship, it may be cancelled and I may be required to repay all or a portion of this Scholarship to the
University.
CONSENT TO DISCLOSE
By accepting this award, I authorize The University of British Columbia to: (i) release to award donors and University
faculty and administrative staff my name, contact information (mailing address, telephone number and email address),
year and program of study, academic average, and other information relevant to the requirements of the award; (ii)
prepare a brief biographical profile to be used for award presentation ceremonies, publications and the Internet; and
(iii) prepare lists of scholarship and prize winners to be published or included in graduation ceremonies.
