14 Lift-All Company Wellness 2011 Health Screening & Completion of the Wellness Profile Who: covered employees & covered spouses  How much: $300 per employee & $300 for spouse (per year)  Where: On-site screenings in April or go to your own doctor (handout for the doctor)  What:‘TAKE THE PLEDGE’ & complete the paper profile (distributed in April) or on-line profile When: complete the profile by May 1st
15 Lift-All Company Wellness 2011 Participate in Healthy Activities Who: covered employees  How much: up to $300 per employee   Where: On-site, On-line or at your local center or facility, at home  When: complete the activities by September 30th What: THREE OPTIONS
16 Lift-All Company Wellness 2011 Healthy Activities: Three Options On-Site Programs ($150 for each) ,[object Object]
10,000 steps – through the summerOff-Site Programs  ,[object Object]
($100 for each)  Self-Certification – ($300) for those who currently have a personal fitness plan
17 Lift-All Company Wellness 2011 On-Site Programs ($150 for each) Drop 10 in 10 – end of Jan through mid April 10,000 steps – during the summer ,[object Object]
Individuals complete ‘journal’ or ‘tracking forms’
Individuals attend a weekly short meeting (during lunch or at break) to ‘check-in’ ,[object Object]
Diet and/or Weight Management – local hospitals or private company like Weight Watcher or Jenny Craig  On-line through Highmark –  ,[object Object],[object Object]
Defined as 2 and ½ hours a week (30 minutes for 5 days a week) in ‘moderate activity’ – walking, running, weights, stretching, biking, swimming, aerobic videos, yoga….etc. ,[object Object]

  • 33. Participation is Optional. Employees who are NOT covered on our medical plan, may participate. However, only those that are on our plan will see a financial benefit in the 2012 payroll contributions.
