2. Documentary
Requirements
All interested Teacher I applicants shall submit the following documentary
requirements to their selected school on or before the deadline.

3. A. Letter of Intent addressed to the
Schools Division Superintendent

4. B. Duly accomplished Personal Data
Sheet (CSC Form No. 212, Revised
2017) with Work Experience Sheet,
if applicable;

5. C. Photocopy of valid and updated
PRC License / ID, if applicable;

6. D. Photocopy of Certificate of
Eligibility / Rating, if applicable;

7. E. Photocopy of scholastic /
academic record such as but not
limited to Transcript of Records
(TOR) and Diploma, including
completion of graduate and post-
graduate unit / degrees, if available;

8. F. Photocopy of Certificate/s of
Training, if applicable;

9. G. Photocopy of Certificate of
Employment, Contract of Service, or
duly signed Service Record,
whichever is / are applicable;

10. H. Photocopy of latest appointment,
if applicable;

11. I. Photocopy of the Performance
Rating in the last rating period(s)
covering one (1) year performance
in the current / latest position prior
to the deadline of submission, if
applicable;

12. J. Checklist of Requirements and
Omnibus Sworn Statement (Annex C)

13. K. Other documents as may be
required by the HRMPSB for
comparative assessment

20. Important Reminders:
- Take a screenshot of your online registration
- Present the screenshot to your selected school upon submission of your
application documents
- The School Selection Committee shall not accept the application documents if the
applicant failed to present a screenshot of his/her online registration
- Application documents must be filed in long white folder, properly labeled with tabs
(3 copies)
- Applicants who failed to submit complete mandatory requirements on the set
deadline shall not be included in the pool of official applicants
- No additional documents shall be accepted after the deadline
