But we're different…
"Human Healthcare"
"Multi-species Medicine"
Start from need
The Current State
2015 AVMA Report on Veterinary Compensation
Median Professional Income of Private Practitioners
2011: $100,000 2013: $94,000
Mean Professional Income of Private Practitioners
2011: $118,726 2013: $129,031
Skewed distribution due to a few of the highest incomes pulling the
mean above the median
2015 AVMA Report on Veterinary Compensation
But, in terms of real (2003) dollars
Mean income in 2013 was less than 2007 and 2009
Bayer Veterinary Care Usage Study
Economy
Improving now, but tough 5-8 years ago
Cycles
Not if, but when
Lean organizations use slow times to get “leaner”
Bayer Veterinary Care Usage Study
“Dr. Google”
39% of pet owners report looking on-line first
15% report relying less on their veterinarian
If clinical signs resolve within 1-2 days,
vets never get to see the pet
Bayer Veterinary Care Usage Study
“Sticker Shock”
26% would switch veterinarians if a
less expensive service was found
One of the two lowest scores practices received was
for value of services
The benchmark for retained clients is 60% at 18 months
“Give your Practice a Physical Exam”, Mark Opperman, Veterinary Economics, Nov., 1, 2013
Bayer Veterinary Care Usage Study
Fragmentation of Veterinary Services
Low cost spay/neuter clinics
Low cost vaccination clinics
Clinics/Hospitals located within pet stores
Specialty practices
“Dr. Y”
A fictional story
Inspired by actual events
Any similarity to actual persons, living or deceased,
or my current employer, is completely coincidental!
• 2000 Graduate
• Owned SA hospital in growing, suburban area of
large city for 10 yrs.
• 1.5 FTE vets and 12 PT and FT staff
• Free standing with 3 exam rooms; typically
furnished and equipped
• Progressive and quality medicine oriented
• Practice website, client portal, social media
presence
• Fees are above average for area and not comfortable
in raising them at this time
• Expense continue to rise, however
• Practice is growing, but leveling off
• Returning clients may be slightly down?
• Management becoming more frustrating and starting
to affect physical and emotional health
• Recurring staff and client issues
• Friendly staff, but turnover higher than colleagues
• Difficulty getting staff engaged, to take the
initiative, and be more self motivated
• Training new employees seems more difficult
than it should be
• Communication between doctors and staff
difficult
• “We just don’t seem to be on the same page.”
• Wonders if there is some new paradigm that will
help solve problems, and improve the systems
and culture now and into the future!?!
Starting to be talked about
But, still in the embryonic stages
The Future State
• 1 to 2 day workshops around the country (world)
• Corporate sponsorship(?)
• Private practices embracing the lean mindset and sustaining the
culture
• Enough data to compare Lean practices to traditionally managed
practices
• Lean lexicon part of veterinary management lexicon
• An annual Lean Veterinary Summit
• A community of Lean practioners sharing ideas & successes in order
to bring the profession closer together for the continuous
improvement of all stakeholders
