1. HOW TO UPDATE STUDENT’s DATA
2nd STEP - Click “Update profile” button
1st STEP - Go To your LIS Class then CLICK “View Profile” on the right-end of the Student’s Name

2. Data to update:
• Parents/Guardian Names
• Spelling of Names
• Current Residence
 Province “BULACAN”
 City (ex. “City of SJDM”)
 Barangay (Know from your Student)
• Permanent Residence
 CLICK “Same as Current..”
3rd STEP – Start UPDATING

Then CLICK “SAVE”

4. 2nd STEP - Click “Correction of basic profile” button
1st STEP - Go To your LIS Class then CLICK “View Profile” on the right-end of the Student’s Name
HOW TO REQUEST CORRECTIONS on STUDENT’s DATA

5. Data to Check for correction:
• Last Names
• First Names
• Middle Name (If Applicable)
• Extension Name (If Applicable)
• Gender
• Birthdate
Type REASON (Required) choose below:
• Typographical Error
• Wrong Initial Information
Then CLICK “Continue”
Final CHECKING of corrected data.
Then CLICK “Submit Request”
Request will be forwarded to Division Office
or the School Head for Approval
3rd STEP - Start Encoding the correct data (Base your correction on the student’s PSA/NSO Birth Cert.)
