Make sure our blank from looks like this

Add title to our form by selecting Untitled form
Next select enter
We have successfully added a title to our form

8. Form Editing can:
Duplicate the question
We select the button to duplicate the question we made.
Delete the question
If we do not want the question to appear, wecan select this
button to delete the question.
Mark the question as required
If we want all of our respondents to answer the question, we
select the slider.
Change the answer type
This involves changing the answer type.

9. Answer type What it does
Short answer This ask the user for text input for an answer
Paragraph Text input for answer, but it can be longer than a short
Multiple Choice This is a default setting for new questions.
Checkboxes User can pick all the answer that apply to him or her
Drop-down This is same as multiple choice but list in drop down format
Linear scale This ask the user to rate a question with a certain scale degree
(1-5)
