My name is Dave Myers, I am the northern representative for Solcom Ltd who have created Whzan.
Solcom were established in 1998 as a developer of bespoke software working in the nuclear medicines and pharma industries.
We have worked with various NHS teams over a 7 year period developing the Whzan Digital Health system
It is a targeted, robust system based upon the clinical expertise of the NHS and Solcom’s robust systems development
The system is mature and fully scalable
Whzan is used to collect and analyse health data from individuals outside of a clinical environment.
For example people with LTCs living at home or care home residents.
Whzan is a cased kit comprising of a tablet PC and wireless instruments is used to take physical measurements.
The tablet contains a range of automated assessment tools and questionnaires including the national early warning score, nutrition and hydration tracking, falls risk assessment, photo history for wound care and so on.
The tablet exchanges information with a secure database accessed via the Microsoft cloud system.
Whzan provides real time information, reports, charts, history, alerts and data interchange facilities.
Patients living at home with LTCs have their own Whzan kit take measurements and are remotely monitored by a clinician.
Residential care homes, carers use the tablet system to monitor individuals both routinely and when a health issue is suspected.
Carers receive immediate traffic light feedback showing the National Early Warnings Score NEWS and have protocols for amber and red situations.
On line tabular weekly data, reports, historical charts, use and intervention records are reviewed by care managers and clinicians.
Whzan is being directly deployed by the NHS, Councils across the UK.
We are just starting deployments with private sector residential care companies, in care homes, with traveling carers and in walk in centres.
We develop the product in close partnership with the users, with NHS Isle of Wight and Sunderland leading the way.
Whzan can be configured to match each individual patient situation.
We are developing new tools for many regions, as each tool is completed it becomes freely accessible to all Whzan users.

The data shown in the slide is supported by reports from separate NHS regions and multiple testimonials.
It is why several regions have made the decision to deploy Whzan to all care homes.
Savings are spread across multiple departments and agencies, making it difficult to obtain complete information, the AHSN is helping some NHS regions with the data review.
Saving patients from admissions, giving them the tools to manage their health and providing carers a means to accurately communicate with clinicians has a huge impact on peoples lives.
