10. TIPS FOR DOCUMENTATION
Corrections in the Proposal Forms
• Any corrections in the PPF should be countersigned either by proposer /
Life Assured not by the advisor or MST.
Age Proof
• Do double check the Age Proof whether standard or non-standard (e.g.
Driving License which is issued before March’93 is non-standard).
Ensure that the extra premium is charged and consent letter is obtained
from the customer
• If the Age Proof is in vernacular language, Do translate this to English
either by the MST or Advisor
• If a person is a graduate or S.S.L.C, & submitting non-std age proof is
not accepted. If he are she submits the non-std age proof, then a valid
reason in the letter format should be enclosed

• Please try to collect atleast one telephone no if possible
Occupation Details
• If occupation is mentioned as business – Details of the business
should be descriptive
Family History
• Do double check the family history column, if any of the family
members died below the age of 50 the reason for death should
be filled in & it is mandatory
Existing or Simultaneous client
• If the customer is already an existing client, Do mention his/her
details in the columns provided in the proposal form without fail.
Also please write it in the top corner of the first page. (Help us to
evaluate the Sum at Risk)

House Wives
• Husband details is mandatory also check for husband insurance.
Health History
• Health related questions should have to be answered as either
yes or no. N.A., dash etc is not accepted
Minor Life & Student
• Check for proposer consent to pay premium
• Declaration in the proposal form should be signed by the proposer
and not by the life assured (for minor life only)
• School studying proof should be given apart from Age Proof (Eg.
School Id., Fees paid receipt etc.)
• MHR by MST is mandatory for all minor life cases

• Check for DOB written
• Check for Nomination & relationship (even though there is no
specific column in the proposal form
• Check for match in the vesting age (Minimum vesting age
should be 45 & minimum accumulation should be 5 yrs)
• Check for regular or lump sum contribution
• If regular premium is opted, then the column for lump sum
should be left blank & if Lump sum is opted, then the column for
regular contribution should be left blank
Signed Date & place
• Signed date and place to be filled at the time of filling up the
proposal form & not at the time of submitting the proposal form.
