2. THORACENTESIS

4. Introduction = Thoracentesis refers to the
aspiration of fluid , pus , air from thoracic
cavity .
THORA = related to thoracic cavity
CENTESIS= aspiration of fluid, pus or air
Thus
Removel of air ,pus or fluid from
transthoracic is called thoracentesis

5. INDICATION:
1- PNEUMOTHORAX
2- HEMOTHORAX
3 - PLEURAL EFFUSION
4-HYDROTHORAX
● TRANSUDATE FLUID= not infectious
in nature as seen in hydrothorax
● EXUDATE FLUID = infectious in nature
as seen in pleural effusion

6. ● Fluid present in pleural space is
called “pleural fluid”
● Normal amount of pleural fluid is “15-
20” ml
● FUNCTIONS = prevent friction
during inspiration and expiration and
act as lubricator
● NOTE- respiratory conditions are
develop if fluid is excess more than
20 ml

7. ::Equipment ::
● Needle - 18gauze, also used in Blood
transfusion and paracentesis
● Tincture benzoin - it is an solution
antiseptics (tinctures) in nature , used
to clean surgical site .
● Anti cogulant - sodium citrate used to
prevent blood clotting at surgical site
during procedure .

● Smallest ribs : 1st ribs
● Longest ribs: 7th ribs
● 1-7 ribs are called true ribs because
they directly attached to sternum .
● 8,9,10 number ribs are called false
ribs because they attached
indirectly with sternum , firstly they
attached with hyline cartilages.
● Immovable ribs - first and second
ribs
● 11th and 12th ribs are called

12. “flotting ribs” because they are flotting.

● Procedures :
● Obtain consent / take inform
consent
(1) If patient is more than 18 year of age
- self consent .
(2) if patient below 18 year or old age-
legal guardians
(3) psychiatrist patient. - court order ,
without Court order patient can refuse
to treatment

13. ● Assessment of vital sign .
Position -
1- when patient able to maintain
sitting upright position
: Provide sitting upright
position with shoulder and
hand supported by bed aide
table
2- when patients not able to
maintain upright sitting position

14. : lying down position towarads
unaffected side
Instruct the patients that not
breath deep during procedure.

17. SITE SELECTION ÷
● X - RAY - to detective location of
affected lungs / exact location .
● Percussion - full sound indicate
accumulation of fluid or body mass
● Site : 7th or 8th inteecostal space
between the interior angel of specula
.
● For air removel 2nd intercostal
space

20. Note - during whole procedures
monitor the insertion of needle
visibly through x ray

THORACENTESIS
● Pneumothorax - air collection
● Hemothorax -blood collection
● Tracheal deviation : trachea
deviated from normal position
