rehabilitation service
5. Follow up at Out Reach Clinics and
Homs visits
Aim- To promote inclusive development
of SB/H Children(Health, Social,
Education, Livelihood, and Empowerment)
Follow up at Out Reach Clinics and Homs
visits is part of activities of CBR.
But why inclusive development important
in implementing CBR?
6. Consider a case of example of a Spina bifida
child identified deep from the Villages of Isingiro
during the home visit:
Jane is a beautiful 5 years old child except that
she was born with Spina bifida and club foot.
Shortly after two weeks her head increased
the size abnormally. Her mother was chest
from the husband’s family saying she was
cursed and she returned to her poor parent’s
family deep in the village. On identification
Jane was a smiling four years old Child.
However she already had contractures,
pressure sores all over the body, Urinary tract
infections, and smelling urine. She was always
left at home alone and could not interact with
peers thus excluded from the main stream of
development.

Homs visits
From the example above all the areas of
child development are affected in one way
or another.
There for Holistic Rehabilitation is not
possible without promoting inclusive
development through Follow up.
8. What do we need to prepare for
follow up
Have all your materials and equipments
for implementing all areas of inclusive
development in one package.
Have a folder of all documents to assess
and evaluate the progress of the Client
Be Therapeutic; Have a flexible mat and
some toys.You may find the house
closed!!
Remember to pack Folic acid
9. What do we need to prepare for
follow up
Have you teaching materials
Have a folder of list of all SB/H clients
Have a map of your area of coverage
List clients from the same sub county find
their treatment/management files.
List the clients on a cord; name, parents,
village and local leader, contact if any.
10. What do we need to prepare for
follow up
Keep all the documents in your bag and
travel with a card in your hands
Travel to the furthest stage towards the
clients stay.
Meet Motorists/boda boda at the stage
and discuss with them the need of what
your doing.
11. Use village structures
In case they cannot located the Clients
family, ask them to take you to the Local
leader (chair person of the village)
Introduce yourself to the family/ local
leader and why your there.
Initiate relationship with the client/care
giver and sit down with them
12. Attend to the Client
Curry out your assessment and compare
the results with previous clients status.
Areas of assessment; Home situation,
Presentation of the client, Home
rehabilitation, Environmental situation,
Intervention, Recommendations
13. Intervention
Base on the positives (Abilities) to deal
with the Limitations
Involve other members of the family. This
make SB/H child included in daily life
activities and support the main care giver
Bring Fathers on board to support
mothers
Agree with the Client/care giver on
treatment plan and what next/ review
14. Community involvement & net
working
Visitservice providers in the area; Health
centers, schools to get the child included
Use the groups you have involved to
identify the new clients in the area and to
support them.
Give them information materials

Wright down the summary of your activities
In the clients file and Clients record book
which acts as their reference.
Leave the family with a different attitude

smile of Jane now!!!!
