2. Lung Segment: UPPER LOBES – Apical Segment
Location of Auscultation:
• I’m not sure about posterior??
Postural drainage position:
• Bed or table flat with pt leaning on pillow at 30° angle
Area to percuss/shake vibrate:
• Between clavicle and top of scap on each side
Exercise:
• Upper lobe expansion – active (pt hands on chest)
• Upper lobe expansion – active (therapists hands on chest)

3. Lung Segment: Upper Lobes: Posterior Segment
Location of Auscultation:
• Just above scapula and upper/medial scap on both sides (see picture)
Postural Drainage Position:
• Bed flat w/ pt leaning over folded pillow at 30° angle.
Area to percuss/shake/vibrate:
• Standing behind pt, claps over upper back on both sides
Exercise:

4. Lung Segment: Upper Lobes – Anterior Segments
Location of Auscultation:
• Inferior to clavicle on either side of manubrium (See picture)
• Right: Medial of nipple line just lateral to sternum
• Left: Just medial to left nipple
Postural Drainage Position:
• Bed flat with pt on supine with pillow under knees
Area to percuss/shake/vibrate:
• Therapists claps between clavicle and nipple on each side
Exercise:
• Upper lobe expansion

5. Lung Segment: Right Middle Lobe
Location of Auscultation:
• See picture
Postural Drainage Position:
• Table elevated 16”
• Pt lies head down on Left side, rotates ¼ turn backwards
• Knees flexed and pillow on back from shoulder to hip
Area to percuss/shake/vibrate:
• Therapists claps over Right nipple area. For women, use cupped hand
with base of hand in the armpit and fingers extending beneath breast.
Exercise:
• Mid-chest expansion

6. Lung Segment: Left Upper Lobe – Singular Segments
Location of Auscultation:
• Just under clavicle b/t 1st and 2nd rib
• Basically on LEFT nipple, tad medial
Postural Drainage Position:
• Table elevated to 16”
• Pt on Right side rotated ¼ turn backwards
• Pillow behind shoulder to hip, knees flexed
Area to percuss/shake/vibrate:
• Therapist claps w/ moderately cupped over Left nipple area
• For females w/ breast development/tenderness cupped hand w/ heel under armpit
and fingers extended forward beneath breast
Exercise:
• Upper lobe expansion – active
• Breathing - active

7. Lung Segment: Lower Lobes – Left Anterior basal segment
Location of Auscultation:
Postural Drainage Position:
• Foot of table elevated 20”
• Pt Right SL, head down and pillow under knees w/ right arm
out of way of percussion area
Area to percuss/shake/vibrate:
• Therapists claps w/ over lower ribs
Exercise:
• Lateral costal expansion – active
• Breathing - active

8. Lung Segment: Lower Lobes – Right Lateral basal segment
Location of Auscultation:
• Right just under inferior angle of scapula
Postural Drainage Position:
• Foot of table elevated 20”
• Pt lies prone then rotates ¼ turn upward
• Upper leg flexed over pillow (support)
Area to percuss/shake/vibrate:
• Therapist claps over uppermost portion of lower ribs
Exercise:
• Lateral costal expansion – active
• Breathing - active

9. Lung Segment: Lower Lobes – Posterior basal segment
Location of Auscultation:
• See picture
Postural drainage position:
• Foot of table elevated 20”
• Pt prone, head down with pillow under hips
Area to percuss/shake/vibrate:
• Therapists claps over lower ribs close to spine on each side
Exercise:
• Posterior basal expansion – active
• Breathing - active

10. Lung Segment: Lower Lobes – Superior Segment
Location of Auscultation:
• Between spine and medial border of scapula, bilaterally
Postural drainage position:
• Table flat
Area to percuss/shake/vibrate:
• Therapist claps over middle back at tip of
scapula on either side of spine
Exercise:
• Breathing – active
• Posterior basal expansion
