4. Processing activities are authorized to enlist
selectees once notified in OTools of selection
and authority to enlist.
The authority to appoint a selected applicant for an
officer program is inherent in the appointment documents
provided by PERS-802 to the NAVCRUITDIST for
delivery, contingent upon compliance with any attendant
restrictions.
5. Prior to Executing the Enlistment/Commissioning
Change of Status Questionnaire
Pay attention to ANY positive answers
Completed in applicant’s own handwriting
Line-outs not recommended
All “YES” answers must be fully explained
Program Service Agreement
Annual Certificate of Physical Condition
Review and sign if all answers are “NO”
If there are any changes enlistment/commissioning will not be
executed. Forwarded updated DD2807-1 to NAVCRUITCOM
(N3M) via MEDWAIVE for final determination

Review Applicant’s File
Pay attention to any character and employer references or police
record checks not forwarded to NAVCRUITCOM prior to time of
selection
Notify the NAVCRUITDIST CO and appropriate
NAVCRUITCOM Director immediately if derogatory
information or any change would preclude
enlistment/commissioning
Ensure physical exam has not expired
Review Conditional Release for Navy Reserve or another
service
Expired: Obtain new DD 368 with new release date
Under no circumstance will applicant be enlisted prior to obtaining
new DD368

9. Determining Enlisted Paygrade
BDCP will be determined by using Exhibit 041001 of
COMNAVCRUITCOM 1131.2D
HSCP and NCP will be determined by the respective PA
NUPOC will be determined during the NUPOC Interview

Candidates required to maintain active bank account and initiate
direct deposit to their financial institution

Deposit Form and return it to the NAVCRUITDIST to be forward to
PSD
Command Urinalysis Brief
Active duty and reserve members will be briefed on the
Command Urinalysis Program
Enlistee/officer candidates will sign and date the Page 13s listed

14. After the oath has been administered

copies of the Oath and Acceptance of Office

Honorably discharge appointees day prior to the acceptance of
the permanent appointment
Notify the custodian of the enlisted service record of the type and

15. OPL/OP Responsibilities

collegiate ships to OCS
FEDEX service and health records to arrive at OCS no earlier than
2 weeks and no more than 2 days prior to report date
Provide candidate their active duty orders to be hand-carried to
OCS
19. Use Distribution of Commissioning Documents
Checklist to ensure all commissioning
documents are distributed properly after the
commission has been executed
OPL/OP will ensure
Commissioning documents are maintained in
accordance with COMNAVCRUITCOM 1131.2D Article

A copy of documents is given to the officer and he/she
signs the Distribution of Commissioning Documents
Checklist indicating receipt
Retain the original checklist in the officer’s residual file.
20. Review NAVCRUIT 1120/58
Ensure all restrictions have been adhered to prior to
allowing administration of the superseding oath
Administer Superseding Oath to Officer
Complete necessary sections of the form
