    • Defining Medical Home  in Kansas• Stakeholder driven process• Began in 2007 and occurred during 2008  Legislative Session• Culminated with the passage of Senate Bill 81 – Codified the definition of medical home in statute – Directed KHPA to establish a medical home delivery  model for Medicaid, children’s health insurance  program (CHIP), and State Employees Health Plan – Directed the agency to develop systems and  standards for implementing medical home model of  care 2
    • NASHP Consortium to  Advance Medical Home• Kansas selected to participate in the  Consortium in 2009 – Develop a plan for statewide implementation of  medical home – Implementation to occur when funds available – Kansas team working with Kansas Primary Care  Physician Coalition to develop standards for  medical home 3
    • Drafting Medical Home  Recognition Standards• KHPA – Transform health care system in Kansas – Medical Home statute with physician directed team – Coordination of patient’s health care needs across the system – Payment reform to help support the model• Kansas Primary Care Physicians Coalition – Three tiered medical home function/payment structure – Entry level – Advanced – Optimal• Kansas Department of Health and Environment – Additional provider category inclusions/integration Chronic Disease Electronic Management System (CDEMS)• Medicaid Managed Care Health Plan 4
