4. MEND in Chicago Delivery Partner RFP
The MEND in Chicago Delivery Partner Grantee will:
• Recruit a minimum of thirteen (13) 7 to 13 year-old participants
(plus an adult care giver) for two 10-week MEND program
sessions
• Secure venue space for two 10-week MEND program sessions
• Achieve an 80% completion rate for all MEND participants
• Comply with evaluation and reporting requirements
5. Eligibility
In order to be eligible, applicants must:
• Be a for-profit or not-for-profit corporation or tax-exempt
youth serving organization providing services in low-income
Chicago communities
• Demonstrate experience providing obesity
prevention/health and wellness programming to youth
between the ages of 7 and 13 and their families.

8. Cover Page
• Provides basic information about grant and grantee
• The Implementing Agency Authorized Official is the
executive director of the agency seeking the grant
• The Program Financial Officer is the chief financial officer of
the agency seeking the grant
• The Program Agency’s Mailing Address should be the
address to which all grant materials will be mailed
9. Cover Page
• Fiscal Contact Person is the person completing the fiscal
reports
• Program Contact Person is the person responsible for
submitting all grant materials to ILAACP. This is the person
with whom ILAACP will be working and who will be the point
of contact for questions.
• Legislative District for both U.S. Congress and the Illinois
General Assembly are needed and may be obtained here

14. Points to Keep in Mind
• Spell out acronyms upon first use.
• Answer all questions.
• In the narrative, describe your work in a way that would help
people who are unfamiliar understand.
• Do not assume that the reviewer has an understanding of
your agency’s work.

16. Agency Capacity/Experience
Describe your organization’s:
• Mission, purpose and major program activities serving lowincome Chicago communities
• Experience working with overweight or obese children
between the ages of 7 and 13 in low-income Chicago
communities
• Experience recruiting and retaining program participants
• Capacity to provide space for two 10-week MEND sessions
• Experience delivering Parent Cafés
18. Goals and Objectives
Program objectives:
• Identify specific, measurable, and realistic changes
intended to be caused by the program
• Consist of actions taken to accomplish program goals
• Include benchmark measures used to determine program
outcomes

• Are measureable
• Identify the target of program activity
• Include timeframe for completion
• Indicate an expected direction of change (e.g., increase or
decrease)

Two kinds of objectives:
• Process (i.e., steps needed to implement the grant)
• Outcome (i.e., how will program impact be measured)

Goal: Recruit 13 MEND participants between the ages of 7 and
13
• Process Objective – Conduct 1 hour of outreach/day to
identify potential participants
– Performance Measure: Completed program recruitment strategy
• Outcome Objective – Qualified participants registered
– Performance Measure: Number of registered participants
23. Staffing and Management Plan
Should include:
• Plan for managing this grant
• Staffing plan and major functions of the staff positions
responsible
• List titles and FTE
• Do not list individual names
• Primary qualifications of each position. Include job
descriptions.
24. Implementation Schedule
• Complete chart with activity, month begun/completed, and
job title responsible.
• Add additional rows as necessary.

26. Budget
• A draft budget is attached to this RFP.
• Calculations are based on MEND Program model expenses.
• Amounts per line item reflect the maximum allowable
amount for that program expense.
• The ILAACP will work with grantees to finalize a budget after
award announcement.
27. Proposal Selection Process
• ILAACP reviews and scores proposals
• Site visits will be conducted week of February 10, 2014
• ILAACP reserves the right to reject proposals deemed
unsatisfactory
• ILAACP reserves the right to request applicants resubmit
amended proposals
• Awards will be announced by February 28, 2014
