Student Life will:
• Accept
Club/Organization
Renewal and
Recognition paperwork
during designated
periods
• Route the completed
packet for approval
The Requestor will:
• Complete the 125-291
*If this is a new
club/organization
• Complete the 125-296
*If the club has been
active in the past 2
years

Student Life Club/Organization Recognition Procedure
1. Complete
the 125-291
or the 125-
296 as
applicable
2. Submit the
completed
packet to
Student Life
within the
renewal window
3. Student Life
submits the packet
to:
•Dean of Students
•College-Wide Student
Activities Coordinator
•Associate Vice
President for Student
Services and Enrollment
Management
4. Student Life will
email the
Club/Organization
President and Advisor
when official
recognition is
approved

2. Student Life Club/Organization Recognition Policy Student Life will: • Accept Club/Organization Renewal and Recognition paperwork during designated periods • Route the completed packet for approval The Requestor will: • Complete the 125-291 *If this is a new club/organization • Complete the 125-296 *If the club has been active in the past 2 years

3. Student Life Club/Organization Recognition Procedure 1. Complete the 125-291 or the 125- 296 as applicable 2. Submit the completed packet to Student Life within the renewal window 3. Student Life submits the packet to: •Dean of Students •College-Wide Student Activities Coordinator •Associate Vice President for Student Services and Enrollment Management 4. Student Life will email the Club/Organization President and Advisor when official recognition is approved
