Anterior shoulder dislocation

1. Case Conference

2. Case: ชายไทยอายุ22ปี
• Chief complaint: ปวดหัวไหล่ขวาขยับไม่ได้ 2 ชั่วโมงก่อนมารพ.

3. Present illness
• 2 ชั่วโมงก่อนมารพ. ขณะเล่นวอลเล่บอล ล้มใช้มือขวายันพื้นในท่าเหยียด
แขนไปด้านหลัง เจ็บหัวไหลขวา ขยับแล้วเจ็บ ไม่มีศรีษะกระแทกพื้น ไม่
บาดเจ็บบริเวณอื่น
• ที่ห้องฉุกเฉิน ผู้ป่วยเดินมาในท่าพยุงแขนงอข้อซอกไว้ข้างลาตัว ปวดมาก
pain score 8/10

4. Past history
• ปฏิเสธโรคประจาตัว
• ปฏิเสธประวัติแพ้ยา แพ้อาหาร
• ดื่มเหล้า social drinking
• สูบบุหรี่วันละ 5 มวน นาน 5 ปี
• ปฏิเสธโรคติดต่อในครอบครัว

5. Physical examination
• Vital sign: BT 36.7°C PR 108bpm RR 18/min BP 128/78mmHg
• GA: Young Thai male, alert, not pale, no jaundice
• HEENT: not pale conjuctivae, anicteric sclerae
• Heart: pulse full and regular, normal s1s2 sound, no murmur
• Chest&lungs: equal breath sound, clear
• Abdomen: soft, normoactive bowel sound, not tender

6. Physical examination
• Extremities: Flatten right deltoid, right arm in slightly
abduction and external rotation, Duga’s test positive,
Hamilton ruler test positive
• Neuro examination: Axillary nerve – intact sensation and
motor

7. Investigation
• Film right shoulder AP, scapular Y view

9. Diagnosis
• Right Anterior shoulder dislocation (subcoracoid type)

10. Management
• Sedate ด้วย Morphine 4mg v stat, Valium 10mg v stat
• Close reduction ด้วย Zero position  failed
• Traction - countertraction  success
• Pain control  Paracetamol(500) 1 tab PO prn for pain q 6hr
 Naproxen(250) 1x2 PO pc
• Film right shoulder(AP, lateral, transcapular) after reduction
• On arm sling and swath
• F/U OPD 1 wk

11. Physical examination (after reduction)
• Joint stability
• Neurovascular

14. Anterior shoulder dislocation
• Epidemiology: shoulder เป็น mojor joint ที่ dislocation บ่อยสุดประมาณ
45% โดยเป็น anterior dislocation 84%, recurrence 75%
• Etiology: indirect force เมื่อแขนอยู่ในท่า abduction external rotation หรือ
hyperextension
• Types:
90%

• Sign & symptoms:
1. Flatten right deltoid
2. Axillary nerve injury
3. Anterior fullness at deltopectorial groove
4. Right arm in slightly abduction and external rotation
5. Duga’s test
6. Hamilton ruler test

16. Anterior shoulder dislocation
• Associated injury:
1. กระดูกยุบทางด้าน posterolateral ของ humeral head เรียกว่า
Hill-Sachs lesion พบได้ 25%
2. กระดูกหักบริเวณ anteroinferior glenoid labrum เรียกว่า Bony
Bankart lesion
3. กระดูกหักบริเวณ proximal humerus
4. กระดูกหักของ greater tuberosity

19. Treatment
• Close reduction
1. Zero position ดึงในท่า abduction 165° flexion 45°

3. Stimson’s technique: นอนคว่า ถ่วงน้าหนัก 10 ปอนด์ รอจน auto-
reduction

22. Treatment
• After reduction
– Immobilizationด้วย interlocking sling นาน 1-3 สัปดาห์ (ในผู้สูงอายุไม่เกิน
1 สัปดาห์)
– ห้ามทา abduction and external rotation ในระยะแรก
– Pain control
– Rehabilitation (Acute, Recovery, Maintenance)

23. Rehabilitation
• Acute phase: <1 wk

24. Rehabilitation
• Recovery phase: 1-6 wks

25. Rehabilitation
• Maintenance phase: >6 wks
