PERSPECTIVE 13.5 FINANCIAL BENEFITS FROM IT Investment in IT is considered a driving factor in improving patient care; the critical question asked by CEOs, CFOs, and board members is whether hospitals achieve a financial return from their IT investments. The inability to measure and account for financial benefits from IT investments has led some hospital administrators to avoid making these types of capital investments that can provide more efficient, higher-quality care and also improve patient and provider satisfaction. One health care system that decided to make this investment is Banner Health, which implemented an IT system at its newest hospital, Banner Estrella Medical Center in Phoenix, Arizona. By investing in IT and receiving input from physicians, pharmacists, nurses, informaticists, and other clinicians, the hospital was able to design new work flows and establish evidence-based order sets. The improved IT design and standard order sets were implemented across departmental functions such as care management, clinical documentation, computerized provider order entry (CPOE), nursing order management, and medication administration and scheduling, and also across processes in departments such as health information management, emergency services, obstetrics, pediatrics, pharmacy, and surgery. This IT investment resulted in a $2.6 million cost savings from greater retention of nurses, which lowered the costs of recruiting, hiring, and training new nurses, and from reducing accounts receivable billing by one day, which resulted in faster payment; decreasing the incidence of adverse drug events, length of stay, and pharmacy costs; decreasing overtime expenses for nurses as a result of improving their efficiency in charting and shifting tasks; and reducing forms costs because information was digitized.

While these hospitals are evolving as world-class care providers, not many of them are able to evolve as profitable and sustainable businesses. The generalization of its use includes the transformation of paper patient charts and protected health information (PHI) into electronic forms and from there the options to revolutionize HIT began. The development of the EHR has brought a multitude of advantages and challenges with its implementation in the HIT world. Although the functions of EHR have allowed for both public and private sectors to utilize its possibilities, the private sector will be focused on in this situation. The ability for an electronic system to safely document, record, save, integrate and share with other instantly has allowed HIT to expand rapidly to all corners of the world.
EHR functionalities
In health information technologies, electronic health records have been identified by the Institute of Medicine as having eight core components which include the following [1]: recording patient health information and data, result management, physician order management, decision support, electronic communication and connectivity, patient support, administrative process and reporting, reporting requirements, and population health. Table 1 allows a visual representation of the core components along with other variable options available through the EHR [2]:

THE MID-1990S, EVERYONE knew that health care orga-
nizations across the United States were plagued by
wasteful spending. The question for Intermountain
Healthcare, which serves residents of Utah and Idaho,
was where to start looking for savings internally.
Data analyses quickly identified the most promis-
ing targets: 104 of the 1,440 clinical conditions that
Intermountain treated accounted for 95% of the care
it provided, and two services—newborn delivery and
treatment of ischemic heart disease—accounted for
21% of its work.
Quality-improvement teams focused first on those
two services. Armed with a sophisticated electronic
health record (EHR) system and a separate informa-
tion technology system that detailed the costs of ac-
tivities, the teams used evidence-based guidelines
and the experience of Intermountain’s physicians to
redesign clinical workflows. The top executives, the
board of trustees, physicians, and nurses all worked
together to support the drive to improve care. Today
more than 60 services have been revamped, and
Intermountain is recognized as a national leader in
quality improvement and cost management. None of
it would have been possible without its IT systems.
This example is impressive. Unfortunately, it is
still a rarity. The more common story in health care
is one of large IT investments but little to show for

The administrative burden of quality improvement should never fall on physicians and other care providers.This exact problem is why I founded Able Health, which is focused on building software that simplifies quality reporting and improvement for all stakeholders. I have written about the need to meet the needs of clinical users in quality improvement through the use of 'design thinking' methods:
Papers can be misplaced; forms can be incorrectly filled, with up to 40% of forms becoming obsolete due to changes; and the sheer number of paper forms means that much storage space is needed. All these can affect patient service delivery and patient care quality within the organisation.
While healthcare technology has improved, the current forms management workflow continues to be a laborious process. PERSPECTIVE 13.5 FINANCIAL BENEFITS FROM IT Investment in IT is considered a
driving factor in improving patient care; the critical question asked by CEOs, CFOs, and board
members is whether hospitals achieve a financial return from their IT investments. The inability
to measure and account for financial benefits from IT investments has led some hospital
administrators to avoid making these types of capital investments that can provide more efficient,
higher-quality care and also improve patient and provider satisfaction. One health care system
that decided to make this investment is Banner Health, which implemented an IT system at its
newest hospital, Banner Estrella Medical Center in Phoenix, Arizona. By investing in IT and
receiving input from physicians, pharmacists, nurses, informaticists, and other clinicians, the
hospital was able to design new work flows and establish evidence-based order sets. The
improved IT design and standard order sets were implemented across departmental functions
such as care management, clinical documentation, computerized provider order entry (CPOE),
nursing order management, and medication administration and scheduling, and also across
processes in departments such as health information management, emergency services,
obstetrics, pediatrics, pharmacy, and surgery. This IT investment resulted in a $2.6 million cost
savings from greater retention of nurses, which lowered the costs of recruiting, hiring, and
training new nurses, and from reducing accounts receivable billing by one day, which resulted in
faster payment; decreasing the incidence of adverse drug events, length of stay, and pharmacy
costs; decreasing overtime expenses for nurses as a result of improving their efficiency in
charting and shifting tasks; and reducing forms costs because information was digitized.
