Cause of Dental Caries
• Streptococcus mutans (S. mutans) bacteria is
  the leading cause of tooth decay.
• Bacteria like S. mutans feast on sugars and
  starches found on teeth.
• Bacterial metabolism produce
  acids that “eat away” teeth
  surfaces causing weak spots
  that can become cavities.

What is Xylitol?
• Naturally occurring sweetener in the same
  class as sorbitol, maltitol, mannitol.
• Found in fibers of trees, fruits,
  and vegetables.
• The human body produces
  several grams of xylitol a day.
• Pure xylitol is a white crystalline substance
  that looks and tastes like sugar .


• On food labels, xylitol is classified broadly
  as a carbohydrate and more narrowly as a
  polyol or sugar alcohol; however, xylitol is
  considered a “sugar-free” sweetener.
• Chemically xylitol differs from other
  sweeteners such as sorbitol, fructose and
  glucose because its molecule has five,
  instead of six, carbon atoms.
• It is much sweeter than sorbitol.
• It contains about 40% fewer calories than
  sugar.
• xylitol's sweetening power is the same as
  that of sucrose (table sugar).
How Xylitol Works
• Cannot be fermented by plaque bacteria.
  – Microorganisms like S. Mutans do not
    metabolize xylitol .
• Inhibits growth and metabolism of
  S. mutans and reduces dental plaque.
• Thus reduces S. mutans levels in plaque
  and saliva .
  – Reduction in acid production
  – Reduction in dental caries
Where Can You Find Xylitol?
Xylitol paste
Xylitol chewing gum
How Much Xylitol?
• Research suggests 5 to 10 grams of xylitol
  per day for effectiveness.
• Humans tolerate up to
  approx. 45 grams/day .
Adults: 6 to 12 g/ day
Children: 3 to 8 g /day
INDICATIONS
•   Hydrated Mouth
•   Against Cavities
•   Diabetics
•   Smoking Cessation
•    Otitis media .
•   lung and nasopharyngeal infections.
Contraindications
• Mild luxative effect

•contraindicated in those who are
hypersensitive to any component of
a xylitol-containing product.
Can dIabeTICs use
       XYLITOL ?
• Yes, because body dose not require
  insulin to metabolize xylitol.
• Low glycemic response than sucrose
  and glucose.

This event takes place during time period when
 the child’s first tooth erupts(6to30 months)
 Because bacteria require tooth surface as a
                     habitat

          inhibit the growth of
   bacteria


    Long term use-
       - permanent reduction
      - change in type of bacteria
in mother’s mouth.
Because of inhibition of growth and
 reduction in number of bacteria in
 mother’s mouth it is difficult for
 these bacteria to be successfully
 transmitted to their children
5 EFFORTS TO KEEP THE MOUTH
                 HEALTHY
1.Use Xylitol toothpaste, mouthwash, and
       nasal spray upon waking up.
2.After breakfast use Xylitol gum, mints or
                   candy.
  3. After lunch use Xylitol gum, mints or
                   candy.
 4. After dinner use Xylitol gum, mints or
                   candy.
5.Use Xylitol toothpaste, mouthwash, and
      nasal spray upon going to bed.
Conclusion
• Xylitol-fluoride toothpaste appears to
  enhance protection from dental caries.
• It is feasible to integrate twice-daily tooth
  brushing into the program.
• Families need to be encouraged to use
  xylitol at home.

  2. Liz presents: So how is a dental cavity actually formed? The occurrence of tooth decay is affected by several factors that may be related to the structure of dental enamel, immunologic responses to cariogenic bacteria, and the composition of saliva. Researchers also believe that genetic variations of these factors may also contribute to increased risks for dental caries. Although everyone can get cavities, some people are at greater risk than others. However, the greatest risk factor remains a sugary diet that fuels the cariogenic bacteria. Tooth enamel is in a constant state of degradation or breaking down and repair (you may have also heard the terms demineralization and remineralization). A balance of this demineralization and remineralization state keeps the teeth healthy and strong. The term “mineral” is from the fact that the teeth structure or enamel is made up of minerals in a complex chemical structure. The most important minerals for the natural repair or remineralization process are calcium and phosphate but this process is slow. Fluoride can act as a replacement for calcium in this complex structure. Although fluoride does not produce as strong a complex structure as calcium its ability to easily and quickly bind to complex makes it a good complement to the natural repair process. Teeth enamel naturally degrade or demineralize with time. However, certain conditions will speed up the demineralization process. Bacteria in the mouth (such as Streptococcus mutans) feed on carbohydrates (sugars and starches) that are found on teeth biofilm (plaque) and produce acids as part of the metabolic by-products i.e. their waste products. The sticky film (biofilm) is actually plaque and it helps bacteria stick to teeth allowing the acid by-products to be released on teeth surfaces. This acid “eats away” the teeth enamel, that is it dissolves the complex structure. Frequent and high sugary foods leads to high levels of bacterial “feasting” activities leading to a degradation process that overwhelms the repair process, the enamel gets damaged and holes or hollow spaces begin to form in teeth. This is call cavities or dental caries and what we common call tooth decay.
  3. Liz presents: Another factor that we have working against healthy mouths is the typical human diet and the large amounts of table sugars present in commercial foods (and often low-cost foods). In the last 20 years, sugar consumption in the U.S. has increased from 26 pounds to 135 lbs. per person per year! That is 2 to 3 pounds of sugar per person each week. Our body is like a complex machine. The foods we choose as fuel and how often we “fill up” affect our general health and that of our teeth and gums. Many dentists are concerned that their patients are consuming record numbers of sugar-filled sodas, sweetened fruit drinks, and non-nutritious snacks that affect their oral health. These items generally have little if any nutritional value and over time they can take a toll on oral health and teeth. Eating patterns and food choices among children and teens are important factors that affect how quickly youngsters may develop tooth decay. Diet and nutrition affects oral health in many ways. We are most familiar with nutrition affecting dental diseases through the development of dental caries, but it can also contribute to dental erosion and periodontal disease. If your diet lacks certain nutrients, it may be more difficult for tissues in your mouth to resist infection and to repair effectively. These may contribute to oral diseases such as periodontal (gum) disease, a major cause of tooth loss in adults. Nutrition also influences: -the proper development of the cranio-facial structures, -proper development of tooth enamel and gum tissues and -risks for disease such as oral cancer and oral infections.
  4. Liz presents: What is Xylitol? We are most familiar with natural sugars such as sucrose, glucose, frucose, and lactose. All of these sugars are 6-carbon based and can be used by cariogenic bacteria and therefore bad for teeth. Many of us also know a bit about the intensely sweet artificial sweeteners such as aspartame ( uh - spahr -teym) (Nutri Sweet) and saccharin (Sweet N’ Low) and Sucralose (Splenda). Xylitol, however, is a natural five-carbon sweetener which falls into the same family of polyols or sugar alcohols as sugar substitutes like sorbitol and maltitol. Sugar alcohols other than xylitol are widely used in sugar-free products such as chewing gums, candies, and toothpastes because they are cheaper. All polyols/sugar alcohol are considered to be noncariogenic. That is, consuming these sweeteners will not cause more tooth decay as regular table sugar would. What is unique about xylitol compared to other sugar alcohol is that it has been scientifically shown that consuming xylitol can actually prevent and reduce tooth decay. Furthermore, xylitol has a natural refreshing taste and is as sweet as table sugar (sucrose) but contains only about half calories. As a naturally occurring sweetener, xylitol is found in the fibers of many fruits and vegetables, including various berries, corn husks, oats, and mushrooms. It can be extracted from hard wood trees such as birch and beech as well as corn fiber, strawberries, raspberries, plums, and corn. As part of human metabolic processes, our bodies actually produce several grams of xylitol each day. The US Food and Drug Administration allows “sugar-free” labeling of products sweetened only with sugar alcohols, artificial sweeteners, or a combination of these.
  5. Liz presents: Microorganisms do not readily metabolize xylitol into energy sources (in other words they do not use xylitol for energy creation thus encouraging microorganism growth). As a five-carbon sugar alcohol, Xylitol cannot be digested by bacteria, as opposed to natural sugars and sweeteners that are fermentable. The thinking behind Xylitol’s inability to ferment is that Xylitol interferes with the metabolism of  Streptococcus mutans  when it is transported into the cell, where it probably stays bound to the transport protein. This bond is unbreakable by the usual enzymes so the transport protein is essentially tied up, the protein can’t go back out to get more glucose to provide the cell with energy. The reduced number of functioning transport proteins is the process thought to reduce acid production in  S. mutans. This reduction in acid production then reduces the creation of dental caries. An additional theory about xylitol is that it makes the bacteria in the mouth less able to produce the sticky extracellular polysaccharides that bind bacteria together and, as a result, they can’t adhere as readily to the tooth surface.
  6. Dr. Ly presents: Do you recognize any of these products or use any of these products? Xylitol is found as a sugar substitute (sweetener) in gums, mints, candies, lozenges, and soft drinks. As well as dental caries prevention products like we have been discussing such as toothpaste, oral sprays, teeth wipes, etc. Xylitol is not effective for dental caries prevention when ingested, exposure must be in the mouth.   7. Dr. Ly presents: So how much xylitol do you need to make a difference? On average, we use about 0.5 grams of toothpaste, which is a pea-size amount. If a xylitol toothpaste contains 31% xylitol then that is only 0.15 grams of xylitol per brushing. Research suggests that may need 5 to 10 grams of xylitol per day to be effective. However, these studies were conducted using chewing gum, lozenges, gummy bears, etc. There is debate that the level needed for effectiveness depends on the what is used to deliver xylitol to the oral cavity and the amount and contact time xylitol has with teeth enamel. As such, different delivery methods may have different effectiveness thresholds. Similar to other sugar alcohols and sugar substitutes, the human body can only tolerate a general amount before sometimes experiencing side effects. Many of us have experienced this after eating too many sugar-free gummi bears, or something similar. For xylitol this amount seems to be about 45 grams per day. Side effects may include cramps and loose stools. Generally if we start slowly with a new substance, our bodies will adapt and symptoms will go away. We do know that side effects for sorbitol is worse than for xylitol.
  8. Liz presents: So from this we can conclude… (Explain slide) Because we were so excited about the results from this initial school study and because the parent response was so positive, we were very willing to help with any future studies. (change slide)
