This document outlines a scoring model called the W.F.O. Questions model for setting and achieving well-formed outcomes. The model involves asking 16 questions about an outcome, including what the outcome is, why it is important, where and when it will take place, who will be involved, what actions need to be taken, and how progress and success will be monitored. The goal of the model is to help people set clear, intentional outcomes and develop strategies and plans to achieve their goals.
