This document contains a general risk assessment form used by the BBC. It includes a hazard list and risk matrix to determine risk levels for identified hazards. The form is to be completed in two parts: Part A requests basic information about the production/event being assessed such as location, team members, and a risk assessment summary. Part B involves using the hazard list and risk matrix to identify specific hazards, their likelihood and severity of harm in order to determine an appropriate risk level. The completed form requires authorization by a designated person.
