The goal of the resource fair is to offer
attendees the most up-to-date, state-of-the-

to the elderly along with general information. 5:00pm - 7:00pm (set-up at 4:30pm)
Come participate in this rare opportunity to meet local area
social workers, discharge planners and case managers!
Attendees are interested in learning what’s new in services available to their
elderly patients. This is your chance to share your expertise and educational
materials with them. You will meet established and potential clients and referral
sources in a friendly and receptive atmosphere where they will expect to hear
about your company, products, points of difference, and services.
o Must bring a door prize valued at $25.00+, educational handouts and a food or beverage item.
o Deadline to register is Wednesday, May 15, 2013! – It’s first come, first serve, so if you wait too
long to mail in your Registration Form and $50.00 Entry Fee, you may not get in. Space is limited.
No walk-ins on the day of the event!
o Set-up begins promptly at 4:30pm. You must check-in at the registration table before setting-up.
o No free-standing or table-top displays! On the day of the event, just bring collaterals and
educational materials. You are encouraged to bring your own tablecloth. Chairs and table will be
provided.
o How it works: When the Resource Fair begins, attendees will visit your booth, at which time you will
give your spiel, and then place your initials next to your company name on their sign-off sheet.
Complete the attached Registration Form and mail it along with the $50.00 Entry Fee to:
