accident. Follow these instructions carefully to protect you and the University.
Steps to Follow at the Accident Scene
1. Aid the injured; do not move the person unless life-threatening; let the
emergency staff deal with the medical situation.
2. Do not make any statements concerning the assumption of liability;
only give information required by authorities; do not sign any statement
except for an authorized Risk Management representative or OU legal
counsel.
3. Call the Police – 911; give exact location and advise if medical help is
needed; write down the Name and Badge # of police officer; write down who
received traffic violation
4. Gather vehicle facts: Company Car & Dept; Driver’s Name; Dept Phone
#; Vehicle Make and Year; Vehicle Tag #; Vehicle #; location of damage

5. Gather other vehicle facts: Driver’s Name; Phone #; Driver Address; Make/year; Tag #; Driver’s License #;
Insurance Co; location of damage (Picture is best).
6. Gather Info on other people involved in accident: Name, Age, Address, Injured Party – Vehicle, Pedestrian;
other vehicle
7. Gather Facts on Property Damage: Owner’s Name, Phone, Address, Property damaged; Nature of damage
(Picture is best).
8. Gather Other Witness Contact Info: Name, Phone, Address
9. What Photographs: Your damaged car and other damaged cars from several angles; car tags; street signs or
address location; signs of weather conditions
10. Fill out the Accident Form at the accident
