Older people are more likely to get stuck in hospitals for several reasons: they are more prone to illness which requires hospital care, they are less likely to recover fully to independence, and they often require more care arrangements to be made upon discharge. Frailty, assessed using a frailty index measuring health deficits, is independently associated with risks of death, longer hospital stays, and increased likelihood of discharge to a nursing home rather than home. Identifying frailty and implementing targeted interventions could help address challenges with older patients experiencing delays in discharge from hospitals.

research presents an online National Health Insurance Scheme (NHIS) Outpatient Medical Appointment
Booking System where NHIS patients can access and view any available personnel or doctor schedule in
order to book an appointment with the corresponding time as specified by the available doctor. The system
was developed using PhP, macromedia dreamweaver, apache and MYSQL. This is to ensure that the
application is robust, cheap and is able to run on different platforms. The system provides the platform to
facilitate the booking and management of patients’ appointment bookings. Patients can also view their
appointment reports. It also provides the healthcare workers an easy access to manage patients’
appointments and to generate relevant reports.

The burden of cancer is also raising: more than 10 million EU citizens are affected by cancer, incidence and prevalence are growing, and the socio-economic impact of cancer heavily burdens both healthcare systems and cancer patients.
The recent political agreement on the text the General Data Protection Regulation and the production of the Privacy Code of Conduct mHealth apps further stir the debate regarding the use of new mHealth technologies within cancer care pathways.

The Imperial Centre for Endocrinology is renowned worldwide for diagnosing and treating rare and complex endocrine conditions. Their multidisciplinary team has extensive expertise in conditions affecting the adrenal, thyroid, parathyroid and pituitary glands, infertility, neuroendocrine tumors, hypertension and genetic endocrine disorders. They utilize state-of-the-art diagnostic testing and see a high volume of complex cases, which has made them a leading center for many endocrine diseases. People travel from overseas to receive second opinions and treatment for conditions that others were unable to diagnose or treat effectively.
This randomized controlled trial evaluated the effectiveness of low-level laser therapy (LLT) for treating chronic knee pain. 126 patients with knee osteoarthritis were randomly assigned to receive either active laser treatment or sham laser treatment over 12 sessions within 4 weeks. The primary outcome was pain level measured using a visual analog scale (VAS) from 0-10. Results showed that the active laser treatment provided significant pain relief and osteoarthritic improvements compared to the sham laser, with statistical significance of p<0.01 in reduced VAS scores from baseline to 30-day follow-up. The laser treatment was thus found to be an effective adjunctive therapy for chronic knee pain.

The management of painful bone metastases requires multidisciplinary care, with external beam radiation therapy (EBRT) providing relief that is effective and time efficient.
In our study the efficacy of external beam irradiation in the palliation of bone metastasis-related symptoms is confirmed by this study, even with short treatments and single-dose administrations. This is important for both patient expectations and the necessity for improved resource allocation with reference to the territorial distribution and waiting lists of radiotherapy centers. The issue of their efficacy in combination with antiblastic drugs (Bisphosphonates drugs such as Zoledronic acid) and/or external beam irradiation(EBRT) remains open and will be clarified only with further randomized clinical trials.

Massachusetts General Hospital's Pre-Admission Testing Area (PATA) was struggling with long patient wait times and inefficiencies. PATA was responsible for completing pre-operative work-ups for outpatient surgical patients, but faced challenges including limited capacity, lack of clear prioritization guidelines for surgeons, and shared ownership between departments. This resulted in patients spending hours in the clinic with minimal face time with providers, delays in surgeries, and overworked staff. A task force was formed to address these challenges, and brought on an MBA intern to conduct an assessment of PATA's processes.

The Imperial Centre for Endocrinology is renowned worldwide for diagnosing and treating rare and complex endocrine conditions. Their multidisciplinary team has extensive expertise in conditions affecting the adrenal, thyroid, parathyroid and pituitary glands, infertility, neuroendocrine tumors, hypertension and genetic endocrine disorders. They utilize state-of-the-art diagnostic testing and see a high volume of complex cases, which has made them a leading center for many endocrine diseases. People travel from overseas to receive second opinions and treatment for conditions that others were unable to diagnose or treat effectively.
This randomized controlled trial evaluated the effectiveness of low-level laser therapy (LLT) for treating chronic knee pain. 126 patients with knee osteoarthritis were randomly assigned to receive either active laser treatment or sham laser treatment over 12 sessions within 4 weeks. The primary outcome was pain level measured using a visual analog scale (VAS) from 0-10. Results showed that the active laser treatment provided significant pain relief and osteoarthritic improvements compared to the sham laser, with statistical significance of p<0.01 in reduced VAS scores from baseline to 30-day follow-up. The laser treatment was thus found to be an effective adjunctive therapy for chronic knee pain.

The management of painful bone metastases requires multidisciplinary care, with external beam radiation therapy (EBRT) providing relief that is effective and time efficient.
In our study the efficacy of external beam irradiation in the palliation of bone metastasis-related symptoms is confirmed by this study, even with short treatments and single-dose administrations. This is important for both patient expectations and the necessity for improved resource allocation with reference to the territorial distribution and waiting lists of radiotherapy centers. Massachusetts General Hospital's Pre-Admission Testing Area (PATA) was struggling with long patient wait times and inefficiencies. PATA was responsible for completing pre-operative work-ups for outpatient surgical patients, but faced challenges including limited capacity, lack of clear prioritization guidelines for surgeons, and shared ownership between departments. This resulted in patients spending hours in the clinic with minimal face time with providers, delays in surgeries, and overworked staff. A task force was formed to address these challenges, and brought on an MBA intern to conduct an assessment of PATA's processes.

It was held under the auspices of the ‘smart villages’ initiative, a three - year project to advance sustain able energy provision for development in off - grid villages in Africa, Asia and Latin America.
Impact investing aims to generate financial returns while solving social or environmental challenges by investing in small- and medium-sized enterprises that offer innovative but high-risk solutions, providing them with flexible, patient funding and support beyond what traditional sources offer; examples include the Low Carbon Enterprise Fund which invests in global businesses reducing carbon emissions. However, impact funds also face limitations in their ability to support entrepreneurs and achieve scale due to constraints of their market-based approach, focus on commercial returns alongside impact, and limited capital and expertise.

• VA physicians earn 26 days of paid annual (vacation/personal)
leave starting their first year and may accumulate up to 86 days

11. HEALTH AND LIFE INSURANCE

RETIREMENT
VA physicians are covered by the Federal Employees Retirement System (FERS).
FERS is a three-tier retirement plan composed of Social Security benefits, FERS
basic benefits, and the Thrift Savings Plan (TSP). Similar to the 401(k) savings
plans widely available in the private sector, the TSP allows employees to tax
defer as much as $17,000 - $22,500 (depending on age) of their income each
year, subject to the requirements of the Internal Revenue Service. The Federal
Government also provides a basic contribution and matching, depending on
the amount of the employee contribution. For retired military personnel, these
benefits are in addition to full monthly retirement pay or pension. Retirement
health care insurance is also available.

12. MOBILITY AND LICENSURE
Where can a career at VA take you? How about anywhere you’d like to live
in all 50 states, the District of Columbia, Puerto Rico, and the Philippines. An
employee may seek employment at any VA location where there is a vacancy
and, if hired, transfer without loss of benefits. Only one active state medical
license is necessary to practice in any VA facility.
LIABILITY PROTECTION
The United States Government accepts responsibility and liability for the
actions of its employees during the exercise of their official duties. VA
physicians, therefore, are protected by the Federal Government in instances
of alleged malpractice or negligence resulting from the performance of their
duties in or for the Veterans Health Administration. There is no fee and tail
coverage is included.

QUALIFICATIONS
Minimum qualifications for appointment as a VA physician are:
• U.S. citizenship (non-citizens may be hired when it is not possible to
recruit qualified citizens)
• Degree of Doctor of Medicine or an equivalent degree from an institution
approved by the Association of American Medical Colleges or the American
Osteopathic Association for the year in which the degree was granted
• Full, current, and unrestricted license to practice medicine or surgery
in any U.S. State, Commonwealth, the District of Columbia, or Territory
(e.g., Puerto Rico)
• Appropriate professional record for experienced physicians
• Satisfactory physical examination (may include drug screening)
• English language proficiency
