Methods: A pretested and validated questionnaire was used to collect details from practising dentists in Pondicherry .272 dentists responded to the questionnaire. Results: The data obtained was statistically analysed with SPSS Version 20 for calculating proportion and mean.84.9% (n=272) of respondents had some kind of musculoskeletal problem affecting different parts of the body.52.2% had low back pain and 50% had neck pain. Conclusion: The dental professionals are regularly exposed to various health hazards in their day to day practice. Chronic musculoskeletal disease is one of the common ailments affecting majority of dentists It is important for the dentists to be aware of the work related factors affecting their health and take adequate precautions or modifications in their working environment

This study compared outcomes of patients receiving a viscoelastic total disc replacement (VTDR) to patients receiving anterior lumbar interbody fusion (ALIF) for lumbar disc degeneration using data from two independent spine registries. Linear regression models showed that VTDR was associated with significantly greater back and leg pain relief compared to ALIF, with differences of 2.76 and 2.12 points respectively. Additional factors influencing pain relief included female sex, monosegmental surgery, higher preoperative pain levels, and surgical level. The results suggest that viscoelastic total disc replacement may provide superior pain relief compared to anterior lumbar interbody fusion for patients with degenerative lumbar disc disease.

Use of an Internet expert system provides diagnoses with a 96% correlation with diagnoses of former Johns Hopkins Hospital doctors, resulting in a 192% increase in interventional testing, and a 50%-63% increase in surgery in previously misdiagnosed patients, 93% of whom report good to excellent improvement after surgery. .
Proper patient selection, using a system from Johns Hopkins Hospital doctors, will allow an orthopedic or neurosurgeon to increase surgery rate by 50%-63% in their existing patient population.
This study aimed to assess the long-term outcomes of lumbar fusion versus non-specific physiotherapy for chronic low back pain. 294 patients were randomized to fusion or physiotherapy and followed for an average of 12.8 years.
Using various analytic approaches, success rates based on patient's global assessment of improvement were in the 65% range for fusion patients, compared to 31-37% for physiotherapy patients. However, secondary outcomes like disability and pain scales showed similar levels of improvement between groups. The discrepancies between primary and secondary outcomes means the study provides only Level II evidence regarding recommending fusion for non-specific low back pain.

Breast cancer in postmenopausal women with hormone receptor-positive (HR+) status is a common and complex condition that necessitates a multifaceted approach to management. HR+ breast cancer means that the cancer cells grow in response to hormones such as estrogen and progesterone. This subtype is prevalent among postmenopausal women and typically exhibits a more indolent course compared to other forms of breast cancer, which allows for a variety of treatment options.
Diagnosis and Staging
The diagnosis of HR+ breast cancer begins with clinical evaluation, imaging, and biopsy. Imaging modalities such as mammography, ultrasound, and MRI help in assessing the extent of the disease. Histopathological examination and immunohistochemical staining of the biopsy sample confirm the diagnosis and hormone receptor status by identifying the presence of estrogen receptors (ER) and progesterone receptors (PR) on the tumor cells.
Staging involves determining the size of the tumor (T), the involvement of regional lymph nodes (N), and the presence of distant metastasis (M). The American Joint Committee on Cancer (AJCC) staging system is commonly used. Accurate staging is critical as it guides treatment decisions.
Treatment Options
Endocrine Therapy
Endocrine therapy is the cornerstone of treatment for HR+ breast cancer in postmenopausal women. The primary goal is to reduce the levels of estrogen or block its effects on cancer cells. Commonly used agents include:
Selective Estrogen Receptor Modulators (SERMs): Tamoxifen is a SERM that binds to estrogen receptors, blocking estrogen from stimulating breast cancer cells. It is effective but may have side effects such as increased risk of endometrial cancer and thromboembolic events.
Aromatase Inhibitors (AIs): These drugs, including anastrozole, letrozole, and exemestane, lower estrogen levels by inhibiting the aromatase enzyme, which converts androgens to estrogen in peripheral tissues. AIs are generally preferred in postmenopausal women due to their efficacy and safety profile compared to tamoxifen.
Selective Estrogen Receptor Downregulators (SERDs): Fulvestrant is a SERD that degrades estrogen receptors and is used in cases where resistance to other endocrine therapies develops.
Combination Therapies
Combining endocrine therapy with other treatments enhances efficacy. Examples include:
Endocrine Therapy with CDK4/6 Inhibitors: Palbociclib, ribociclib, and abemaciclib are CDK4/6 inhibitors that, when combined with endocrine therapy, significantly improve progression-free survival in advanced HR+ breast cancer.
Endocrine Therapy with mTOR Inhibitors: Everolimus, an mTOR inhibitor, can be added to endocrine therapy for patients who have developed resistance to aromatase inhibitors.
Chemotherapy
Chemotherapy is generally reserved for patients with high-risk features, such as large tumor size, high-grade histology, or extensive lymph node involvement. Regimens often include anthracyclines and taxanes.
