This document provides guidance for dental nurses on taking a patient's dental history. It discusses collecting biographic data, chief complaint, history of present illness, past history, and psychosocial data. Biographic data includes name, address, occupation, marital status. Assessing chief complaint involves exploring the pain's features, location, character, aggravating/relieving factors, duration and severity. Evaluating history of present illness requires asking when the problem began, how often it occurs, and what makes it better or worse. Past history addresses prior hospitalizations, medications, and allergies. Psychosocial data involves understanding a patient's ideas, concerns, and expectations about their condition.
