- 2. Solution Overview
High Level Solution Description:
Treat’em is a patient-centered private and secure social network and health
information exchange. It allows users to benefit from the support and
wisdom of the crowd while maintaining the confidentiality of their medical
data. It provides a central location for healthcare providers to integrate
efforts and engage with patients across the continuum of care.
Healthcare/Business problem addressed:
Social networks could play a significant role in the management of patient
with chronic disease if only it was possible to ensure privacy.
Health information exchange is key for the management of chronic patients,
but connecting patient records across institutions is problematic in lack of a
unique identifier.
The application developed addresses the following category:
Applications that improve management of high cost chronic diseases.
Innovative approaches to addressing value-based care imperatives.

- 3. Usability and Design
At the core of the system is the TreatmentLine – a timeline that includes the
treatment plan for the patient.
Healthcare providers can use the TreatmentLine to integrate treatment
efforts; to share information and files; to follow up and developments in the
treatment and to monitor compliance.
Users can involve followers in the management of their medical condition.
A User can exchange information and support with fellow users with similar
conditions treated by the same healthcare providers (micro-communities).
Users, with the aid of healthcare providers, can manage their medical
information in a central, HIPAA compliant location.
The micro-communities allow for generation of personalized knowledge-
bases.
Users can upload any medical data to share with their healthcare providers
and followers including bluetooth based data collection from home
monitoring devices (e.g., blood pressure, weight, glucose etc.,).

- 5. Integration Description
The main integration with AllScripts is for the upload of patient documents.
In the future it will be possible to add functionalities such as downloading
and saving of documents into AllScripts including home monitoring data (the
latter is problematic due to liability concerns – i.e., a provider failing to
follow up an abnormal result).

- 6. Go to Market Plans
The business model - B2C (patients) and B2B (providers).
B2B will make use of the Enterprise and Professional EHRs.
The system will require significant marketing efforts in regular
channels. We hope to engage providers in disseminating the
system by providing the B2B services for a nominal fee in the first
phase.
The greatest advantage of a patient centered social network is the
ability to identify and characterize homogenous subpopulations.
Accordingly the main expected source of revenue is advertising
(without compromising user privacy, of course).

This video shows the Treat’em social network under two
scenarios. The first, a patient with cancer across the continuum of
care. The second of a patient with congestive heart failure using
the home monitoring integration.

- 8. Why this solution be selected!
This is a patient centered solution that has an actual potential of
improving patient outcomes by allowing effort integration and
harnessing of social tools to help patients manage their condition.
It makes medical social networking accessible by maintaining
user confidentiality.
It moves the center of health information exchange to patients,
thus reducing privacy concerns and the challenge of identity-
reconciliation.
It is a platform that can be extended to other services such as
the collection of home monitoring data.
It separates the continuous follow-up process from the actual EHR
to reduce concerns of liability on the part of providers.

Set alerts for readings that require
immediate attention. For example, a blood

Set alerts for patient-specific abnormal
readings that will remind the patient to
repeat measurement at pre-specified
timeframe. Set when to notify the provider.
* The system always requests a repeated
measurement to verify abnormal readings.
