The document discusses various treatment options for erectile dysfunction, including oral phosphodiesterase type 5 inhibitors (PDE5Is), intracavernosal injections, penile prostheses, and lifestyle modifications. It provides guidelines and recommendations from sources such as the American Urological Association and European Association of Urology on when to initiate certain treatments, how to administer PDE5Is effectively, and the role of treatments after procedures like prostatectomy. The treatment algorithm highlights PDE5Is as first-line treatment but notes alternatives like intracavernosal injections or topical therapies for some patients.

Editor's Notes
In one recent study, atenolol was associated with fewer sexual attempts, lower level of serum testosterone after 16 weeks, and a rate of ED of 18% compared with 0% with the angiotensin II inhibitor valsartan
Nebivolol is a beta-blocker with nitric oxide donor properties that may improve ed
3 Trials suggest that angiotensin receptor blockers may actually improve sexual function and may be the drug of choice in the ED patient newly diagnosed with hypertension

You should not have sexual intercourse for at least 6 weeks after your implant surgeryÂ
Antibioitocs hasta 48 hrs iv, y continuar al altaThe âno touchâ technique involves discarding all surgical instruments and changing all surgical gloves after an incision is made in the penoscrotal raphe and the dissection is carried down through the subcutaneous tissue and dartos to the level of Buckâs fascia.Â
The disadvantages of this approach are blind placement of the reservoir and increased scrotal swelling when compared to the infrapubic incision.Â
La limpieza es necesaria y debe ser excepcional, un dia antes se afeita la zona , y el dia de la cirugĂa se debe tener buena higiene, Antibiotic infusion is started in the holding area approximately 1 hour before the operationÂ
Proper exposure and retractor set-up are extremely important. Poor exposure obscures anatomy, making the case longer and more difficult and increases the chances of inadvertent injury or infection.Â
The patient is catheterized after draping is complete and the indwelling catheter is used to empty the bladder, An empty bladder is crucial for standard, retropubic placement of the reservoir to minimize the possibility of bladder injury the âSKWâ deep scrotal retractor from Boston Scientific and the âWilsonâ scrotal retractor from Coloplast.Â
Penis stretched and elevated over beaded strap with sharp hook.
An incision is made transversely about 1â2 cm below the penoscrotal junction to a total length of about 4-cm.Â
Incision and cautery through dartos creating a hole.
The incision is then held in place over the base of the penis and retracted open with blunt hooks.
First 2 hooks are placed superiorly at 11 and 1 oâclock with loose tension, then 2 hooks are placed inferiorly at 5 and 7 oâclock with medium tension, then 2 hooks are placed laterally at 3 and 9 oâclock with tight tensionÂ
Using the index and middle fingers, with blunt dissection the scrotal contents are dissected away, revealing the proximal corpora cavernosa and urethra
the âcobra kai maneuver according to Dr. KĂśhler;(qula) the âbarrido de Henryâ according to Dr. HenryÂ
The overlying dartos tissue overlying the corpora cavernosa is then sharply dissected, revealing the âsqueaky cleanâ, gleaming white tunica albugineaÂ
After blunt and sharp dissection, the tunica should be âsqueaky cleanâ with no overlying tissue.Â
cleaning off the dartos and then cleaning off Buckâs fascia.
Both hands can be used in opposite directions, for more direct power, continuing until the corpora cavernosa and corpus spongiosum are cleaned off anteriorly.
Here the scrotal septum is demonstrated prior to and after sharp dissection of the septum off the urethra. Note the exposure afforded by the cowboy blanket roll and additional rake retractors.Â
Corpotomia, Right corpora is exposed with urethra pulled toward left corpora.Â
The corporotomies should be proximal enough on the corpus cavernosum to ensure the tubing exits the corporotomy into the scrotum, not the penile skin.Â
1.5-cm corporotomies are made with a 15-bladeÂ
The 2-0 vicryl stay sutures are placed around the intended corporotomy on each corpus cavernosumÂ
For a Coloplast device, corporotomy should just accept the 12 Fr Brooks dilatorÂ
Distal dilation is begun with scissors or the suction tip to about 50â75% of the way to the glans penis.
The instrument should deform the lateral wall of the penis to prevent crossoverÂ
Distally we ensure the dilator points slightly laterally to avoid crossover throughout the dilation.
Squeezing the glans prevents distal perforationÂ
In order to check for urethral injury after the distal dilation, forceful injection of fluid with a bulb syringe is done on each side distally while observing for urethral dischargeÂ
The proximal dilation is done following the path of the crura of the corpora as they flare laterally onto the pubic bone.Â
âfield-goal testâ checks the proximal dilation by placing a dilator on each side proximally. The dilator handles should be parallel and of equal depth penetration (Paired dilators can then be inserted in both corpora simultaneously to ensure an equal dilatation with no posterior crural perforation.
A subdartos pouch is made for the pump in the middle of the scrotum or the hemiscrotum.
A small incision is made through the dartos fascia and a long-blade nasalspeculum or ring forceps is used to create the space
The external ring is identified using a small Deaver retractor and the index finger is used to palpate the spermatic cord, pushing it medially to protect it.
A closed Metzenbaum scissors is then used to puncture the transversalis fascia.
This permits access to the retropubic space.Â
The index finger is inserted again to create a space.Â
The reservoir is then filled with normal saline up to 5 mL above its manufactured capacity.Â
With the tubing to the filling syringe left open, the syringe should spontaneously fill to 5 ml.Â
If it fills more than this, the position of the reservoir space needs to be checked again to ensure that there is no pressure around the reservoir.
A finger is surprisingly effective with proper retraction. In our hands, if a finger cannot pierce the fascia, then the reservoir is placed ectopicallyÂ
Coloplast 125 cc cloverleaf reservoir which we intentionally under-fill (to minimum cylinder requirements) to prevent an obvious bulge.Â
All overlying tissue is cleaned off of the pubic tubercle with a finger or Metzenbaum scissors. The tubercle must be free of all overlying Scarpaâs fascia to avoid reservoir placement superficial to the musculatureÂ
Otherwise the reservoir will be placed in a subcutaneous location instead of submuscular, creating a visible and palpable bulge in the abdominal wallÂ
Cadaver demonstration of ectopic reservoir location in abdominal esta subcutaneo
The device is then inflated to seat the cylinders correctly and to test the device.
The guide strings help position the distal ends of the cylinders underneath the glans if pulled during the initial inflation (Figure 12).
When satisfied with the position, we keep the device inflated to about 60% for 24 hours to aid in hemostasis.Â
A 2-0 vicryl is used to close the deep dartos layer with a gathering stitch, with careful attention is to keep the device tubing in the scrotum and preventing its migration under the skin of the penis.Â
A âfigure of eightâ type of method is used to wrap the entire external genitalia until the dressing roll is finished

first description in 1973. The specific objective of the surgery is to increase the erection (cavernosal) artery blood inflow in patients with blood flow related ED secondary to trauma.
Indications
<55 years old
Better outcomes for younger patients
Recently acquired ED from focal atrial occlusive disease
Absence of other risk factors (e.g. smoking, DM, etc.)
All young patients with a history suggestive of trauma-associated impotence (pelvic fractures and perineal trauma) undergo routine hormonal evaluation to ensure adequate circulating levels of testosterone.
The patients undergo a night-time penile erection test to rule out nerve damage or psychological ED. Finally, they undergo blood flow assessment, using either duplex Doppler ultrasound of the penis or dynamic infusion cavernosometry/cavernosography (DICC)
Doppler ultrasonography, if the peak systolic velocity is less than 25 cm/s. Angiography, whilst being the benchmark for all arterial investigations, is reserved with the aim of concurrent embolisation or angioplasty if an arteriovenous fistula or stenosis is suspected
They devised PRS using the anastomosis of the inferior epigastric artery (IEGA) to the CC. Subsequently, DDV arterialisations, either with no extensive venous ligation or with venous ligation,
the Furlowâ Fisher procedure (Fig. 4 ), in which IEGA was made in an end-to-side anastomosis to the DDV, while several ligations were made at the lateral circumflex veins, in proximal and distal locations

The vacuum chamber creates an erection by generating negative pressure within the penis, increasing blood flow, and distending the corporeal sinusoids,
Then, constriction rings are applied to maintain an erection sufficient for penetration.
This can be maintained for up to 30 minutes
The 2008 AUA best practice policy on the use of parenteral antibiotics for broad spectrum coverage prior to penile prosthesis surgery recommends use of vancomycin or a first- or second-generation cephalosporin as well as an aminoglycoside 1 hour before surgery and up to 24 hours after surgery.Â
vascular endothelial growth factors (VEGF) and their receptor Flt-1
